The effectiveness of outpatient pulmonary rehabilitation for chronic lung diseases

Objective: To determine whether functional ability of patients with chronic lung diseases could be improved after pulmonaryrehabilitation (PR). Methods: Patients in Regional Respiratory Center were allocated to an eight-week PR program. The PR program consisted of aerobic training, resistance training, upper extremity mobilization and respiratory muscle training. Maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP), peak cough flow (PCF) for pulmonary function, six minute walk test for physical activity, Modified medical research council (MMRC) dyspnea scale, and COPD assessment test (CAT) were tested and followed after the PR program. Results: Sixteen of fifty-three patients actively completed PR programs and pre- and post-evaluations without follow up loss. Among these 16 patients, eight patients attended the hospital based PR, four attended the home based PR, and the others attended the hospital & home based PR. All of these patients improved in MIP by 7.25 cm H2O, MEP by 9.5 cm H2O, PCF by 23.18 L/min, six minute walking distance by 30.21 m without statistical significance. Conclusion: The results showed that the compliance of out-patient PR programs was approximately 30.3%. Although there was no statistical significance in the pre-post evaluations, the out-patient PR program could produce positive response in chronic lung diseases. Therefore, this study supports the necessity of the active pulmonary rehabilitation for the improvement of physical function and pulmonary function of various chronic lung diseases in Korea.

N/A Background/Aims: Liver biosy is usually used to assess the expect of hepatic fibrosis, however, it is limited hecause serious complications have been reperted accasionally and frequent examinations are not adequate, Recently, the serum level of type IV collagen-7S domain is known to reflect the activity of collagen synthesis in the process of fibrosis. We evaluate the clinical usefulness of serum type IV collagen as an indicator of hepatic fibrosis. Methods: Serurn levels of type IV collagen 7S domain were evaluated in 88 subjects including 20 healthy persons, 40 patients with chronic active hepatitis confirmed by liver biopsy and 28 patients with liver cirrhosis. The patients with chronic active hepatitis were classified as mild(18 cases), moderate(11 cases) and severe degree(11 cases) according to the histologic findings and the patients with liver cirrhosis were classified as Child A(10 cases), B(10 cases), C(8 cases) according to modified Childs classification. Serum level of type IV collagen-7S domain was measured by radioimmunoassay (RJA). Results: Serum type IV collagen-7S domain concentrations in patients with chronic active hepatitis and liver cirrhosis were significantly higher than those in healthy persons(p<0.05). And significant correlations between the patients with chronic active hepatitis and with liver cirrhosis were also found(p<0.05). When chronic active hepatitis was divided into mild, moderate and severe degree there was a statistical significances(p<0.05). And significant correlations were found between the serum type IV collagen and the grade of the modified Childs classification(p<0.05). Conclusions: These results suggest that the measurement of serum type IV co]lagen may be a simple, noninvasive method and sensitive rnarker of hepatic fibrosis and also may reflect tbe status of progression in cirrhosis. Exoskeleton-assisted Gait Training in Spinal Disease With Gait Disturbance

Objective: Gait impairment reduces a patient’s quality of life. Exoskeletons and wearable robotics enable patients with gait disturbance to stand up and walk. An exoskeleton was developed for use in patients with stroke and spinal cord injuries. This study aimed to evaluate the effectiveness of overground exoskeleton-assisted gait training (OEGT) in spine diseases with gait disturbance. Methods: This was a single-group preliminary study. Five participants with gait disorders because of root dysfunction accompanying spinal stenosis were included in this study. All participants underwent surgical treatment and an exoskeleton training protocol scheduled for 2 or 3 days per week for 4 weeks. Each session was 60 minutes. Clinical tests were performed before (T1) and at the end of the training (T2). Results: One patient dropped out of the study because of medical issues that were not associated with the exoskeleton. Exoskeleton-assisted rehabilitation was feasible for all participants. All participants showed positive changes in gait performance, balance, proximal muscle strength, psychological state, and satisfaction with the rehabilitation. However, there was no significant improvement in neurological deficits. Conclusion: OEGT is a feasible rehabilitation method for patients with gait disorders caused by degenerative spinal disease.
Evaluation of Hydration Effect on Human Skin by $^1H$ MRS at 14.1T

Purpose: We achieved high resolution MR imaging and spectra of human skin in vitro with using a 14.1 T MRI/MRS system, and evaluated the hydration effect of various cosmetic products by measuring the skin's. moisture concentration. Materials and Methods: We used the Bruker 14.1 T MRI/MRS system with a vertical standard bore that was equipped with a DMX spectrometer gradient system (200 G/cm at a maximum 40 A), RF resonators (2, 5 and 10 mm) and Para Vision software. Spin echo and fast spin echo pulse sequences were employed for obtaining the high resolution MR images. The 3D-localized point resolved spectroscopy (PRESS) method was used to acquire the MR spectra. Results: The high resolution MR images and spectra of human skin in vitro were successfully obtained on a 14.1 T system. The water concentration of human skin after applying a moisturizer was higher than that before applying a moisturizer. Conclusions: The present study demonstrated that the high-resolution MR images and spectra of human skin from a high field NMR instrument could be applicable to evaluating the hydration state of the stratum corneum.

Lemierre syndrome is a rare disease that`s characterized by internal jugular vein thrombosis and septic emboli. These symptoms typically develop after acute oropharyngeal infection by Fusobacterium necrophorum. Although this syndrome is less frequently seen in modern times due to the availability of antibiotics, physicians must be aware of the syndrome in order to initiate prompt antibiotics therapy, including coverage of the anerobic organisms. We discuss here the case of an 18-year-old female with Lemierre syndrome and we review the relevant literature on this syndrome.
Development of Automatic Region Segmentation Methods For Brain MR Images

The purpose of the present research was to evaluate an automatic brain-region division system as a first step to comparative and quantitative brain MR analysis. Separating brain regions has been used for a long time to get certain goal-directed information from MR images. Without an automatic system, many researchers have to spend time for the laborious work. During the process, the users' intervention often hampers objectivity and could bias the results. As a method to divide brain regions automatically from MR brain images, we suggest that brain masks and iterative selection algorithm give excellent results.
