Tolvaptan is a highly selective vasopressin receptor 2 antagonist that regulates cyclic adenosine monophosphate levels to inhibit both epithelial cell proliferation and chloride ion excretion, two mechanisms known to induce cyst expansion in autosomal dominant polycystic kidney disease (ADPKD). Tolvaptan is currently the preferred treatment of rapidly progressive disease ADPKD in adult patients; however, since cyst formation in ADPKD begins early in life, (frequently in utero), and significant disease progression with cyst expansion occurs in the first decade, tolvaptan may be advantageous as a preemptive treatment in children with ADPKD. Tolvaptan has already been used to successfully treat refractory edema or hyponatremia in children; this literature review provides insight into the biochemical basis of its action to contextualize its use in the pediatric population.

The presence of hypoxic cells in solid tumors has long been considered a problem in cancer treatment such as in radiation therapy or treatment with some anticancer drugs. It has been suggested that hypoxic cells are involved in the development of a more aggressive phenotype and contribute to metastasis. In this study, as an attempt to understand how tumor cells adapt to hypoxic stress, we investigated the regulation of the hypoxia-induced expression of proteins that control essential processes of tumor cell survival and angiogenesis. We first examined whether hypoxia induces stress protein gene expression of murine solid tumor RIF cells. We also examined hypoxia-induced changes in angiogenic gene expression in these cells. Finally, we investigated the association of the elevated levels of stress proteins with the regulation of hypoxia-induced angiogenic gene expression. Results demonstrated that hypoxia induced the expression of the erythropoietin (EPO) gene and at least two major members of stress proteins, heat shock protein 70 (HSP70) and 25 (HSP25) in RIF tumor cells. Evidence that the expression of EPO gene was greatly potentiated in TR cells suggested that the elevated levels of HSPs may play an important role in the regulation of the hypoxia-induced EPO gene expression. One of the RIF variant cell lines, TR, displays elevated levels of HSPs constitutively. Taken together, our results suggest that a hypoxic tumor microenvironment may promote the survival and malignant progression of the tumor cells by temporarily increasing the level of stress proteins and expressing angiogenic genes. We suspect that stress proteins may be associated with the increase of the angiogenic potential of tumor cells under hypoxia.

A method for processing and the drug releasing behavior of 5-Fluorouracil(5FU)/ Polyglycolic Acid(PGA) composite, applicable to medical devices, was investigated. A preliminary results indicated that the method utilizing melt processing could be a simple means by which the composite can be processed without referring to more involved and difficult-to-achieve solvent processing method. It is shown that the effectiveness of 5-FU was maintained during tests, even after the processing temperature of 230℃. The release patterns from the various sized samples have indicated that the observed release may have occurred in one or two stages, depending on the sample thickness and that a longer experimental times may be required to elucidate the complete release pattern. Although a further and more detailed analysis need to be performed to better understand the controlled release behavior of 5FU/PGA composite, the current results have provided the possibility of utilizing melt processing as a tool to produce medical devices for successful drug delivery applications,

In this paper we describe streamer propagations characterizing underwater pre-breakdown and breakdown behaviour under impulse voltages. Streamer corona branches in lower resistivities are much thicker and brighter than those in higher resistivities. The negative streamer channels not only have more branches but also widely spread out than the positive streamer channels. The succession of streamer coronas during pre-breakdown produces a stepwise propagation. The pre-breakdown currents consist of the conduction current and streamer corona current pulses.

This paper describes touch and step voltage in the reinforced concrete and steel poles. Ground surface potential rises were measured as a function of the separation between pole and ground rod. Touch and step voltage are calculated on the basis of the distribution of ground surface potential rises. As a result, touch and step voltages strongly depend on the position of installation of ground rod.
