      • 경두개 직류전기자극 기기의 안전관리 이슈에 대한 고찰

        경두개 직류전기자극(Transcranial Direct Current Stimulation, 이하 “tDCS”)는 현재 우울증, 뇌졸중, 파킨슨 등의 적응증 뿐만 아니라 비의료 건강응용분야에도 활용되고 있으며 관련 연구가 진행되고 있다. tDCS와 관련한 추가적인 연구들이 진행됨에 따라 tDCS 기기의 허가 및 안전관리 측면에서도 중요성이 증대되고 있으며 이에 대한 연구가 필수적이다. 본 논문에서는 tDCS의 개요와 한국, 유럽, 미국의 인허가 및 안전관리 제도를 알아보았으며 인허가 단계와 인허가 후의 사용 단계로 구분하여 tDCS 안전관리 이슈를 제시하고, 각 tDCS 안전관리 이슈에 대한 해결방안을 제안하였다. tDCS 기기의 장기적 사용 문제의 해결방안으로 뇌 질환 병변의 변화, 뇌 가소성 변화양상 등의 연구를 포함하여 임상연구를 통한 장기적 사용의 안전성 정보 확보를 제안하였다. tDCS 규제감독 범위에 대한 문제의 해결방안으로 의료기기 품목분류 대상 지정 방안, 소비자 tDCS 기기에 대한 문제의 해결방안으로 의료기기 사후관리 제도 도입 방안을 제안하였다. 개인의 tDCS 기기 오남용 문제를 해결하기 위해 사용자 교육 프로그램 및 가정용 원격 모니터링 안전관리 시스템 구축 방안을 제안하였으며, 취약한 대상자의 tDCS 기기 사용 문제의 해결방안으로 tDCS 기기의 안전한 자극 수준 설정, 프로토콜을 명시하고 권장하는 취약한 대상자의 tDCS 사용 가이드라인 마련을 제안하였다. 본 논문에서 제안한 바와 같이 tDCS의 안전관리 방안을 구체적으로 수립함에 따라 안전한 사용 및 관리를 이룸으로써, tDCS 기기의 안전성 확보를 강화하며 tDCS 기기를 비롯한 비침습적 뇌자극기기 산업을 활성화하는데 기여할 수 있기를 기대한다. Transcranial Direct Current Stimulation(tDCS) is being used not only for various adaptations but also for non-health applications. As studies progress in this regard, the licensing and safety management aspects of tDCS are also becoming important. In this study, I looked at the overview of tDCS and the safety management systems of Korea, Europe and the United States, and presented safety management issues. A solution to tDCS safety management issues was proposed. As a solution to the long-term use of tDCS, safety information through clinical research was proposed. and a method for designating medical device item classification as a solution to the problem of the scope of regulatory supervision was proposed. To solve the problem of consumer tDCS, a plan for introducing a medical device follow-up management system was proposed, As a solution to the problem of personal tDCS abuse, a user education program and a home remote monitoring safety management system were proposed. As a solution to the problem of tDCS use by vulnerable subjects, I proposed the establishment of a use guideline specifying the stimulus setting and protocol. It is expected that this study will strengthen the safety of tDCS and contribute to invigorating the non-invasive brain stimulation device industry, including tDCS devices.

        HER2 양성 간세포양 위선암 1예

        간세포양 위선암은 매우 드물며 일반적인 위선암에 비해 예후가 불량하다. 진단 당시 림프절 전이 및 간 전이를 동반한 경우가 많아 초치료로 항암화학요법을 시행하는 경우가많지만 현재까지 정립된 항암화학요법은 제시된 바 없다. 또한 문헌 보고에 따르면 그 효과가 위선암에 비해 좋지 않다. 본 증례는 국내에서 HER2 양성을 보인 간세포양 위선암의 첫 증례이다. 기존 항암화학요법에 HER2 표적치료제인 tras-tuzumab을 추가하여 사용하였고 현재 복부 전산화단층촬영 및 AFP 검사상 호전 소견을 보이고 있다. Gastric hepatoid adenocarcinoma is a rare adenocarcinoma that develops in the stomach. The prognosis of gastric hepatoid adenocarcinoma is poorer than that of ordinary gastric adenocarcinoma. Here, we report the first case of human epidermal growth factor receptor 2 (HER2)-positive gastric hepatoid adenocarcinoma in Korea. A 57-year-old male presented with abdominal distension and underwent endoscopic gastric biopsy and percutaneous core needle liver biopsy. The pathological findings were consistent with HER2-positive gastric hepatoid adenocarcinoma. He received six cycles of chemotherapy with cisplatin-capecitabine plus trastuzumab, which is a HER2 targeted agent. After chemotherapy, a follow-up abdominal computed tomography scan showed a partial tumor response. This case emphasizes the importance of using trastuzumab in a patient with HER2-positive gastric hepatoid adenocarcinoma. (Korean J Med 2016;91:211-215)

      • 17세기 조선의 동전유통정책

        This study focuses on the governmental regulation in coin production and circulation in Seventeenth-Century Choson. In preparation for famine, policy makers minted and circulated coin to conserve and store food resources. Policy makers and intellectuals insisted on coin circulation as the solution to conserve food resources, rather than good exchange. Rice and fabric, particularly ramie and cotton, were the traditional mediums of exchange in Choson. Since the majority of governmental finance consisted of rice and fabric, famine would cause financial shortage. Policy makers and intellectuals feared that famine would reduce trade due to the depletion of rice and fabric. The appropriation of good exchange for crops as currency would also aggravate starvation and intensify financial crisis. Therefore, policy makers consistently exhorted for coin mintage and distribution. On the one hand, the government in the early seventeenth century could not issue coins extensively. Copper, the primary material in coin mintage, was expensive because it was a major import from Japan and was a diversely demanded commodity. Evidently, coin mintage became a challenge for the Choson government particularly during and after the two major wars (Japanese Invasion and Manchu Invasion). On the other hand, the government of King Sukchong (1661~1720) obtained substantial fiscal afford for minting large quantities of coin by profiting from the intermediary trade between Qing and Japan and by implementing positive fiscal policies. However, the government of King Sukchong strictly limited coin production in order to prevent excessive coin distribution. The government also limited the period of coin distribution and the import volume of copper. Excessive coin distribution could lead to monopolization of profit for merchants and further exacerbate impoverishment of public welfare. Therefore, the government allownned coin mintage primarily for the central administration to prevent illegal production and authorized coin circulation mostly during famines and castle constructions to relieve financial shortage.

        This thesis attempts to review the "historical restoration" that now goes indiscriminately everywhere in korea and that in fact deletes individual memories of the city, to set up a theoretical and experimental frame with 「Rejuvenation with a slight gesture」, the theme of contest in 2015 Korean Architecture Award, and to propose a design idea by which the sound urbanity is recovered and public interest is acquired as the competition gropes for. In consideration of humor in architecture, the study particularly introduces Antonin Artaud"s concept of "corps sans organs"(1947), who had founded the theory of "theater of cruelty", as a theoretical frame in humanistic horizon, and seeks for a new value that satires conventional and normative architecture.

        Optimum Design for an Air Current Pulverizing Blade Using the Computational Fluid Dynamics

        In the air current pulverizing type grinding method, the blade wings fitted inside a casing are rotated at a high speed to generate a cornering air current, which facilitates the collision of materials with one another, leading to the pulverizing phenomenon. In contrast to mechanical grinding, grit pulverizing leads to fine grinding and less acid waste and degeneration of the material. Moreover, this approach prevents the loss of nutritional value, while allowing the milling grain to have an excellent texture. However, the existing air current pulverizing type machines consist of prefabricated blades, which cannot be rotated at a speed higher than 5,000 rpm. Consequently, the grinding process becomes time consuming with a low productivity. To overcome these problems, in this study, the shape and structure of the air current pulverizing type wings were optimized to allow rapid grinding at more than 8,000 rpm. Moreover, the optimal design for the ripening parts for the air current pulverizing type device was determined by performing a computational fluid dynamics analysis based on airflow analyses to produce machinery that can grinding materials to the order of micrometers.

        반응표면분석법으로 최적화한 초음파와 저온 블랜칭의 병용처리 조건이 신선편이 양상추의 갈변과 품질에 미치는 영향

        Enzymatic action and microbial growth degrade the quality of fresh-cut lettuce. Browning, a bad smell, and softening during storage are the major forms of quality deterioration. Health-oriented consumers tend to avoid foods treated with chemicals to maintain their freshness. This study was conducted to evaluate the change in the quality of fresh-cut lettuce with combined low-temperature blanching (LB) and ultrasonication (US). The optimum condition was selected using the response surface methodology (RSM), through a regression analysis with the following independent variables; the ultrasonication time (X1), blanching temperature (X2), blanching time (X3), and dependent variable; `E value (y). It was found that the condition with the lowest ΔE value occurred with combined 90s US and 45℃ 90s LB (US+LB). The combined treatment group (US+LB) was stored at 10℃ for 9 days with the control group and each single-treatment group, with low-temperature blanching and ultrasonication. Overall, the US+LB group had a significantly high L* value, which indicates significantly low a*, b*, ΔE, browning index, PPO, and POD activity values, and a low total bacteria count (p < 0.05). The US+LB group also had the highest sensory score (except for aroma and texture; p > 0.05).

        Phytoncide 처리가 신선편이 양상추의 저장 중 이화학적, 미생물학적 및 관능적 특성에 미치는 영향

        본 연구는 솔잎 유래의 phytoncide essential oil을 증류수에 희석한 것(1%, V/V, DP)과 식용 에탄올에 희석한 것(1%, V/V, EP), 증류수(DW) 및 식용 에탄올(EE)을 침지액으로 이용하였다. pH는 DW 처리구에서 가장 높고 EP 처리구에서 가장 낮았으며, 증류수보다는 에탄올 포함 처리구에서, 대조구보다는 phytoncide 성분이 포함된 처리구에서 낮은 pH인 것으로 나타났다. 적정산도는 EP 처리구가 가장 높고, DW 처리구가 가장 낮은 것으로 나타나 pH의 결과와 잘 일치하는 경향이었다. 총 가용성 고형분 함량은 1.60~2.23의 범위로, 에탄올이 포함된 EE 및 EP 처리구에서 유의적으로 높은 함량으로 나타났으며, 포장필름 내 기체조성 분석 결과, EP 처리구가 modified atmosphere packaging (MAP)의 원리인 저산소 및 고이산화탄소 조건인 것으로 나타났다. 관능검사 결과, 외관, 색 및 갈변 정도에서 EP 처리구가 유의적으로 높은 점수를 얻었으며, DW 처리구가 가장 낮은 점수를 얻어 갈변 저해에 뛰어난 효과가 있음을 알 수 있었다. 총균수는 저장 중 꾸준히 증가하는 경향이었으며, EP처리구가 가장 완만한 증가율을 보여 항갈변뿐만 아니라 항균에도 효과가 있음을 확인할 수 있었다. 이러한 결과들로 보다 EP 처리구는 갈변 저해 및 향균에도 효과가 뛰어나 품질 유지에 긍정적인 영향을 미치므로, 고품질 신선편이 양상추의 가공 및 유통에 있어 효과적인 방법으로 이용될 수 있을 것이라 기대된다. The ciTed of phytoncide solution treatment on the browning and quality of fresh-cut iceberg lettuce during storage was studied. The treatments were applied as four solutions adjusted at l0℃: distilled water (DW) as the control; edible ethanol (FE); 1% (v/v) phytoncide essential oil hum line needle diluted with distilled water (1W); and 1% (v/v) phytoncide essential oil diluted with edible ethanol (EP). Fresh-cut lettuce was dipped in each solution for 60 sec, was rinsed with distilled water; was packaged with an OPP film bag, and was then stored at 4℃ for l2 days. The EP group had a significantly high level of total soluble solids, titratable acidity, and carbon dioxide, and low total bacteria counts, pH, and oxygen. The sensory score of color in the EP groups recorded a high value, hut the FE and EP groups recorded low scores in arorna and taste during the storage period. Alcohol and phytoncide were vaporized by opening the package for two hours and the score of the arorna and taste of EP showed no differences from those of the other groups. Based on the above results, it was deterrnined that the phytoncide essential oil diluted in edible alcohol with 1% solution inhibited the bruwning of and nicrubial growth in fresh-cut lettuce, and will be a useful natural compound in maintaining the quality of fresh-cut produce.
