        Adsorption of Reactive Blue 114 dye by using a new adsorbent: Pomelo peel

        This paper describes the removal of Reactive Blue 114 dye from aqueous solutions by using pomelo(Citrus grandis) peel. Pomelo peel can be described as a new, low cost, abundantly available adsorbent. The optimum adsorbent mass, dye concentration, contact time and pH were determined in this study. The parameters of Langmuir, Freundlich and Temkin adsorption isotherms were also obtained usingconcentrations of the dyes ranging from 1.0 to 200 mg/L. Maximum adsorption capacity was obtained as16 mg/g at pH 2 and 303 K solution temperature. The adsorption process was observed to be reachingequilibrium after about 90 min.

        Bilateral Brachial Artery Aneurysms with Distal Embolisms in a Patient with Prolonged Crutch Compression

        Chronic axillary crutch use may lead to axillary or brachial artery aneurysms and associated axillobrachial thromboembolic complications. Most of these aneurysms remain asymptomatic and undiagnosed until a complication occurs. Early diagnosis and appropriate surgical management of the aneurysms is required for a favorable outcome. We report a case of bilateral brachial artery aneurysms with left radial and ulnar artery embolisms in a patient who had been using crutches for 33 years due to a congenital skeletal anomaly.

        Reversible Thermochromic Polycaprolactone Nanofibers for Repetitive Usage

        Color change technology offers unique and challenging opportunities. Thermo-responsive color-changing nanofibers with reversibility have great potential as thermal sensors due to their increased sensitivity and fast response. Herein, polycaprolactone (PCL) nanofibers were produced by adding a leuco-based thermochromic dye with various concentrations (1%, 3%, and 5% wt corresponding to PCL1, PCL3, and PCL5, respectively). The color-changing properties with repetitive heating and cooling were studied, and the effect of dye concentration on the nanofiber properties was determined. The surface properties, dye presence, thermal and mechanical properties were analyzed by SEM-EDS, FTIR, DSC, and tensile tests. Finally, the color change properties were monitored by 1000 heating and cooling cycles between 20 and 40 °C. Thermochromic PCL nanofibers were successfully produced by electrospinning. However, some agglomerates were observed on the nanofibers with increasing dye concentration in SEM images. It was seen that the optimum dye concentration was 3% in terms of the electrospinnability. For PCL5, both presence of carbon, oxygen, nitrogen, and fluorine in EDS spectra, the shifted peaks at 2917 and 2849 cm-1, and the new peaks at 1558, 1517, 1330, 1274, 1213 and 883 cm-1 in FTIR spectra confirmed that dye had been successfully incorporated into the PCL structure. The dye addition caused a decrease in the crystallization degrees, which resulted in lower mechanical properties. PCL5 had the lowest modulus. Color measurements showed that 1% of dyes concentration was not sufficient for the thermochromic property, and the color change was still visually detectable for PCL3 and PCL5 even after 1000 heating and cooling cycles. Color change activation temperature (TA) was confirmed between 30 and 32 °C, and the stability of color change was confirmed for 1000 heating and cooling cycles. After 1000 heating and cooling cycles, the color change was still detectable for PCL3 and PCL5. Consequently, this study showed that reversible thermochromic PCL nanofibers could be promising materials for future sensor applications.

        Bleeding metastasis of renal cell cancer to anal canal treated with radiation

        Renal cell cancer (RCC) has the ability to metastasize to various organs, including the anal canal which is reported to be the rarest location. An 88-year-old male patient who had previously been treated for right RCC subsequently developed distant metastases to the prostate, lungs, and small bowel. Four years following nephrectomy, the patient presented with a bleeding anal mass which was excised and has been proven to be an anal canal metastasis of RCC. Seven months post excision, regrowth occurred. The patient underwent stereotactic ablative body radiotherapy resulting in satisfactory regression during the 2-month follow-up period, without episodes of bleeding. The treatment options for metastatic post-nephrectomy disease should be considered with a multidisciplinary approach in order to achieve satisfactory symptom relief.

      • Which One is More Effective, Filgrastim or Lenograstim, During Febrile Neutropenia Attack in Hospitalized Patients with Solid Tumors?

        Background: Chemotherapy-induced febrile neutropenia (FN) with solid tumors causes mortality and morbidity at a significant rate. The purpose of this study was to compare the effects of filgastrim and lenograstim started with the first dose of antibiotics in hospitalized patients diagnosed with FN. Materials and Methods: Between February 2009 and May 2012, 151 patients diagnosed with FN were evaluated, retrospectively. In those considered appropriate for hospitalization, convenient antibiotic therapy with granulocyte colony stimulating factors was started within first 30 minutes by completing necessary examinations in accordance with FEN guide recommendations. Results: In this study, 175 febrile neutropenia attacks in 151 patients were examined. Seventy three of the patients were male and 78 were female. The average age was 53.6 and 53.6, respectively. The most common solid tumor was breast carcinoma in 38 (25%). One hundred and five FN patients (58%) were those who received granulocyte colony stimulating factors as primary prophylaxis. Conclusions: While studies comparing both drugs generally involve treatments started for prophylaxis, this study compared the treatment given during the febrile neutropenia attack. Compared to lenograstim, filgastrim shortens the duration of hospitalization during febrile neutropenia attack by facilitating faster recovery with solid tumors.

        Collapsed L4 Vertebral Body Caused by Brucellosis

        Brucellosis is caused by gram-negative, aerobic, non-motile, facultative, intracellular coccobacilli belonging to the genus Brucella. A 50-year-old man working as an employee was admitted to neurosurgery clinic with severe low back, radicular right leg pain and hypoesthesia in right L4-5 dermatomes for 2 months. Brucella tube agglutination (Wright) test was positive in serum sample of the patient with a titer of 1/640. Brucella melitensis was isolated from blood culture. X-ray and MRI of the lomber spine showed massive collapse of L4 vertebral body. Neural tissue was decompressed and then posterior L3-5 short segment transpedicular screw fixation and stabilization was performed. Brucella melitensis was isolated from microbiologic culture of pathologic specimen. Antibiotic therapy was given as doxycycline 200 mg/day and rifampicin 600 mg/day for 6 months. Brucellosis is a systemic zoonotic infection and still an important public health problem in many geographical parts of the world. Vertebral body collapse caused by brucellosis occurs very rarely but represents a neurosurgical emergency because of its potential for causing rapidly progressive spinal cord compression and permanent paralysis. Neurosurgeons, emergency department personnel as well as infectious disease specialists should always keep a high index of suspicion and include brucellosis in the differential diagnosis of vertebral body collapse.

        Bilateral Thoracic Ganglion Cyst : A Rare Case Report

        Ganglion cysts usually arise from the tissues around the facet joints. It is usually associated with degenerative cahanges in facet joints. Bilateral thoracic ganglion cysts are very rare and there is no previous case that located in bilateral intervertebral foramen compressing the L1 nerve root associated with severe radiculopathy. We report a 53 years old woman who presented with bilateral groin pain and severe numbness. Magnetic resonance imaging revealed bilateral cystic mass in the intervertebral foramen between 12th thoracal and 1st lumbar vertebrae. The cystic lesions were removed after bilateral exposure of Th12-L1 foramens. The result of hystopathology confirmed the diagnosis as ganglion cyst. The ganglion cyst may compromise lumbar dorsal ganglion when it located in the intervertebral foramen. The surgeon should keep this rare entity in their mind for differential diagnosis.

        Medicinal leech therapy—an overall perspective

        Complementary medicine methods have a long history, but modern medicine has just recently focused on their possible modes of action. Medicinal leech therapy (MLT) or hirudotherapy, an old technique, has been studied by many researchers for possible effects on various diseases such as inflammatory diseases, osteoarthritis, and after different surgeries. Hirudo medicinalis has widest therapeutic usage among the leeches, but worldwide, many different species were tested and studied. Leeches secrete more than 20 identified bioactive substances such as antistasin, eglins, guamerin, hirudin, saratin, bdellins, complement, and carboxypeptidase inhibitors. They have analgesic, anti-inflammatory, platelet inhibitory, anticoagulant, and thrombin regulatory functions, as well as extracellular matrix degradative and antimicrobial effects, but with further studies, the spectrum of effects may widen. The technique is cheap, effective, easy to apply, and its modes of action have been elucidated for certain diseases. In conclusion, for treatment of some diseases, MLT is not an alternative, but is a complementary and/or integrative choice. MLT is a part of multidisciplinary treatments, and secretes various bioactive substances. These substances vary among species and different species should be evaluated for both treatment capability and their particular secreted molecules. There is huge potential for novel substances and these could be future therapeutics.
