      • 신경증을 수반한 Paroxysmal Dystonic Seizure 2례

        신경증을 수반한 paroxysmal dystonic seizure 2례를 치험하였기에 문헌 고찰과 함께 이를 보고한다. The auther experienced a varient form of idiopathic reflex epilepsy ; paroxysmal dystonic seizure. The Two cases described in which motor seizure, consisting usually of focal or generalized paroxysmal dystonic writhing movements without loss of consciousness, were provoked by sudden movement. And they also suffered from panic disorder with agoraphobia, conversion disorder, avoident behavior and hypochondriacal ideation due to underlying paroxysmal dystonic seizure. This form of epileptic attack showed good response to anticonvulsant medication and the neurotic symptoms were relieved by supportive psychotherapy and behavioral therapy.

      • 설갑상선 : 1 예 보고 A Case Report

        갑상선 조직 전체가 설기저부에 발생한 이소성 설갑상선 증례를 경험하였기에 그 방사선학적 소견을 문헌고찰과 함께 보고하고자 한다. 증례는 59세 여자 환자로 40 일전부터 발생한 음성 변화를 주소로 내원하였다. 이학적 검사에서 설 기지부 중앙에 회백색의 격막에 둘러 쌓인 3 cm 가량의 종괴가 있었으며, 종괴는 내장으로 돌출하여 구인두를 막고 있었다. 경부 전산화 단층촬영 측면 정찰 촬영에서, 설 기저부에 후방으로 돌출하여 구인두를 막고 있는 경계가 분명한 연부조직 종괴가 보였으며, 전산화 단층촬영 동맥기와 정맥기에 주변의 혈관과 비슷한 강하고 균일한 조영증강을 보이는 장경 2.5 cm의 원형 종괴가 있었다. 또한 갑상선이 있어야 할 경부 전방 공간에서 정상 갑상선 조직은 전혀 보이지 않았다. 설 기저부의 종괴에서 생검을 시행하였으며 병리 검사에서 갑상선 조직으로 진단되어 설갑상선으로 확진되었다. The authors report a case of ectopic thyroid that whole thyroid tissue was replaced by lingual thyroid. A 59-year-old female presented with dysphonia duration of 40 days. On physical examination, there was a 3 cm sized well encapsulated whitish mass protruding to oral cavity in milline of tongue to base. Computerized tomography of neck revealed homogeneously enhancing well marginated 2.5 cm sized round mass in the cecum of tongue base on arterial and venous phase. And there was no normal thyroid gland in lower anterior neck.

      • 지역난방배관의 다중누수 감지기 개발에 관한 연구

        Local heating transportation pipe has sensor and return lines to detect water- leakage. There are impulse and resistance comparison measurementtypes for a water-leakage detection. The impulse type shows large detection error within a measurement range. Since the resistance comparison type can find a comparative accurate single water-leakage point in the measurement range of heating pipe. it has used to detect water-leakages these days. However if the multi water-leakages are happened in the measurement range of trange of transportation pipe. the resistance comparison type shows a detection error point by the parallel resistance between a detection sensor line and ground. But the detection error will be minimized by the divided transportation pipe loops. In this research, it suggests the design of remote controlled detector which can divide a large pipe loop and a possible single water-leakage measurement process in each divided loops.

      • 활동기준원가(ABC)를 활용한 은행 본부간접원가의 배분

        This study is focuses on allocation of overhead cost of bank head office and uses ABC as a means of the allocation of th cost. Originally, ABC was designed to improve inaccuracy of overhead cost allocation and has been studied in manufacturing industry. But ABC has enormous potential for improving many dimensions of operational performance for financial institutions, such as banks or securities companies. To know whether ABC can solve the problem of accurate allocation of overhead cost in process of a branch costing, I chose a bank for analysis and compared the method the company actually uses with ABC method. As a result, There is a wide difference between traditional method and ABC method. The application of ABC system will give advantage that the company can separate value-added and non value-added activities and provide timely information for strategic cost management and operational cost management. ABC can bring about significant changes in management behavior by focusing attention on expenditure patterns, operational processes, supproting activities, and responsibilities throughout the organization.

      • 기흉 후 발생한 뇌동맥 공기 색전증 : 1예 보고

        뇌혈관내 공기 색전증은 뇌혈관 조영술, 혈관내 도자술 시행시 발생하여 매우 심각한 합병증을 유발할 수 있다. 그러나 기흉의 합병증으로 발생한 경우는 보고된 적이 없다. 저자들은 기흉에 의하여 발생한 뇌혈관내 공기 색전증 1예를 경험하였기에 보고한다. Cerebral arterial air embolism is a potentially fatal event that may occur during the cerebral angiography and intravascular catheterization. The CT findings of cerebral arterial air embolism following pneumothorax have not been reported. We report a case of cerebral arterial air embolism following pneumothorax, and describe the CT findings.

        GABA와 Benzodiazepine 수용체 및 그 기능

        GABA is ubiquitously distributed throughout the CNS. It is probably the major central inhibitory aminoacid neurotransmitter which hyperpolarizes the postsynapic neurons and inhibits the release of neurotransmitters. Generally inhibition of GABA activity causes excitation leading to anxiety and convulsions, whereas activation of its activity causes antiexpressive. anticonvulsive activity and sedation. The GABA receptors have been divided into two distinct groups named GABA(A) and GABA(B) receptors by their pharmacological and physiological properties. GABA(A) receptors are coupled with Bz binding site and that in conjunction with a CI­ channel they form a supramolecular receptor complex which mediates rapid increasing of CI­ influx into postsynaptic neurons. thus contributing to the prompt inhibition of cellular excitability. On the contrary, the GABA(B) receptor does not contain an integral ion channel and is responsible for slow responses through receptor-G protein-effector complexes. GABA(B) receptors appear to be localized on presynaptic nerve terminals and stimulation of presynaptic GABA(B) receptors reduced Ca²+ influx, resulting in decreased release of neurotransmitters. Stimulation of presynaptic GABA(B) receptors increases K efflux, resulting in inhibition of cell firing. A division of function among the two types of GABA receptors appears to exist : GABA(A) receptor complex mediates anxiety, anticonvulsive activity and feeding, GABA(B) receptors, on the other hand, are involved in depression and analgesia. In those cases where GABA(A) and GABA(B) receptors mediate similar functions (e.g., cardiovascular regulation), they do so by affecting different transmitter systems and cellular mechanisms. The putative involvement of GABA(A) and GABA(B) receptors in various behavioral and physiological effects is summarized in Table 3. There are two types of Bz receptors in brain which are central and peripheral type Bz receptors. Central Bz receptors are in many regions of the brain, coupled with the receptors for GABA and they mediate the acute actions of Bz in CNS. Although there is general acceptance that Bz evert their major actions via the GABA(A) receptor, more recent studies suggest that other systems may be involved. In addition, endogenous anxiogenic ligands that interact with Bz receptors in several ways have been discovered, which of these,β-carbolines and DBI may produce anxiety and convulsion in human, The recent investigation of endogenous ligands and specific receptor agonist or antagonist and inverse agonist provide important new conceptual tools for the studies of anxiety and depression mechanisms. It is expected that a better understanding of GABA and Bz receptors will eventually help to demonstrate the biology of anxiety and depression.

      • 폐혈관상이 유지된 선천성 낭성 선종양 기형

        선천성 낭성 선종양 기형은 태생 16주 이후 말단세기관지의 형성이 중단되어 미성숙기관지들과 중배엽성분들이 과도하게 증식하여 발생하는 드문 선천성 폐질환이다. CT소견은 낭종의 크기 숫자, 내용물 등에 따라 다양하나, 병변내에 정상적인 폐혈관이 보인 예는 전세계적으로 보고된 적이 없었다. 저자들은 역동적 흉부 CT상 균일한 저음영의 병변내에 정상 폐혈관상이 유지된 선천성 낭성 선종양 기형 1예를 경험하였기에 보고한다. Congenital cystic adenomatoid malformation(CCAM) is a rare pulmonary disease that is thought to be developed by abnormal proliferation of immature alveoli and other mesenchymal component from the abrupt halting of the terminal alveoli formation after 16 weeks of gestation. On CT scan, there are no consistencies regarding to size, numbers, and its contents but no cases were reported in past showing normal pulmonary vascularities. We reports a case of CCAM that showed normal pulmonary vascularities within the hypodense lesions on a dynamic chest CT
