        GeneXpert MTB/RIF Testing in the Management of Patients with Active Tuberculosis; A Real Life Experience from Saudi Arabia

        Background: GeneXpert MTB/RIF is a real-time PCR assay with established diagnostic performance in pulmonary and extra-pulmonaryforms of tuberculosis. The aim of this study was to assess the contribution of GeneXpert MTB/RIF assay to the management ofpatients with any form of active tuberculosis in a single large tertiary center in Saudi Arabia, with a special focus on the impact ontime to start of antituberculous therapy compared with Ziehl-Neelsen (ZN) smears and mycobacterial cultures. Materials and Methods: Clinical, radiological and laboratory records for all patients who were commenced on antituberculous therapybetween March 2011 and February 2013 were retrospectively reviewed. Results: A total of 140 patients were included, 38.6% of which had pulmonary tuberculosis. GeneXpert MTB/RIF was requested for only39.2% of patients and was the only reason for starting antituberculous therapy for only 12.1%. The median time to a positive GeneXpertMTB/RIF result was 0 days (IQR 3) compared with 0 day (IQR 1) for smear microscopy (P > 0.999) and 22 days (IQR 21) for mycobacterialcultures (P < 0.001). No patients discontinued antituberculous therapy because of a negative GeneXpert MTB/RIF result. Conclusions: In a setting wherein physicians are highly experienced in the diagnosis and treatment of tuberculosis, GeneXpertMTB/RIF was remarkably under-utilized and had only a limited impact on decisions related to starting or stopping antituberculoustherapy. Cost-effectiveness and clinical utility of routine testing of all smear-negative clinical samples submitted for tuberculosisinvestigations by GeneXpert MTB/RIF warrant further study.

        Theoretical investigation of electronic structure and optical response in relation to the transport properties of Ga1-xInxN (x = 0, 0.25, 0.50, 0.75)

        The state-of-the-art all-electron FLPAW method and the BoltzTrap software package based on semiclassical theory were adopted to explore the electronic structure and the optical and thermoelectric properties of Ga1-xInxN. Ga1-xInxN is predicted to be a direct band gap material for all values of x. Moreover, the band gap varies between 2.99 eV and 1.95 eV as x changes. Optical parameters such as the dielectric constant, absorption coefficient, reflectivity and refractive index are calculated and discussed in detail. The doping of In plays an important role in the modulation of the optical constants. The static dielectric constant ε (0) of Ga1-xInxN was calculated as 3.95, 3.99, 3.99 and 4.03 at x = 0.00, 0.25, 0.50 and 0.75, respectively. The static refractive index is 2.0 for pure Ga1-xInxN at x = 0.00. The thermal properties varied greatly as x fluctuated. The ternary alloy has large values for the Seebeck coefficient and figure of merit at high temperatures and is thus suitable for thermoelectric applications. Pure Ga1-xInxN at x = 0 exhibited ZT = 0.80 at room temperature, and at higher temperatures, the thermal conductivity decreased with increased In doping.

        Adaptive k-means clustering for Flying Ad-hoc Networks

        Flying ad-hoc networks (FANETs) is a vibrant research area nowadays. This type of network ranges from various military and civilian applications. FANET is formed by micro and macro UAVs. Among many other problems, there are two main issues in FANET. Limited energy and high mobility of FANET nodes effect the flight time and routing directly. Clustering is a remedy to handle these types of problems. In this paper, an efficient clustering technique is proposed to handle routing and energy problems. Transmission range of FANET nodes is dynamically tuned accordingly as per their operational requirement. By optimizing the transmission range packet loss ratio (PLR) is minimized and link quality is improved which leads towards reduced energy consumption. To elect optimal cluster heads (CHs) based on their fitness we use k-means. Selection of optimal CHs reduce the routing overhead and improves energy consumption. Our proposed scheme outclasses the existing state-of-the-art techniques, ACO based CACONET and PSO based CLPSO, in terms of energy consumption and cluster building time.

        The Top-100 most cited articles on dural arteriovenous fistula: A bibliometric analysis

        Objective Dural arteriovenous fistula (DAVF) is a rare pathological vascular lesion with variable clinical manifestations. Since 1968, several articles have been published to address spinal and cranial DAVFs. The aim of this study is to identify, analyse, and review the pertinent literature of the top-100 most cited articles on DAVFs published to date. Methods A title-specific, keyword-based search with no time restriction was performed in June 2020. The Scopus database was used to identify the top-100 most cited articles on DAVFs. The term “dural arteriovenous fistula” was used as a search keyword. The search results were arranged in descending order based on the total citation count. The top-100 articles were categorized into ten categories. Results Between 1968 and 2020, a total of 2298 articles were published on DAVFs. The top-100 most cited articles were published between 1983 and 2012. The total number of citations for the top-100 articles was 12393 (123 citations/article). Most articles (34%) were investigating the clinical aspect of DAVFs. The country contributing to the most impactful and highest volume of publications (46%) was the United States. The Mayo Clinic was the most active institute in contribution. Most articles (29%) were published by the Journal of Neurosurgery. Conclusions In the top-100 most cited articles on DAVF, most studies were published in neurosurgery/neuroradiology-dedicated journals. This bibliometric analysis identifies the publication trends and provides a comprehensive overview of the most influential articles addressing DAVFs.         Simulation and Experimental Validation of Gain-Control Parallel Hybrid Fiber Amplifier

        We demonstrate a simulation of a parallel hybrid fiber amplifier in the C+L-band with a gain controlling technique. A variable optical coupler is used to control the input signal power for both EDFA and RFA branches. The gain spectra of the C+L-band are flattened by optimizing the coupling ratio of the input signal power. In order to enhance the pump conversion efficiency, the EDFA branch was pumped by the residual Raman pump power. A gain bandwidth of 60 nm from 1530 nm to 1590 nm is obtained with large input signal power less than -5 dBm. The gain variation is about 1.06 dB at a small input signal power of -30 dBm, and it is reduced to 0.77 dB at the large input signal power of -5 dBm. The experimental results show close agreement with the simulation results.

        Smart City is gaining attention with the advancement of Information and Communication Technology (ICT). ICT provides the basis for smart city foundation; enables us to interconnect all the actors of a smart city by supporting the provision of seamless ubiquitous services and Internet of Things. On the other hand, Crowdsourcing has the ability to enable citizens to participate in social and economic development of the city and share their contribution and knowledge while increasing their socio-economic welfare. This paper proposed a hybrid model which is a compound of human computation, machine computation and citizen crowds. This proposed hybrid model uses knowledge-based crowdsourcing that captures collaborative and collective intelligence from the citizen crowds to form democratic knowledge space, which provision solutions in areas of civic innovations. This paper also proposed knowledge-based crowdsourcing framework which manages knowledge activities in the form of human computation tasks and eliminates the complexity of human computation task creation, execution, refinement, quality control and manage knowledge space. The knowledge activities in the form of human computation tasks provide support to existing crowdsourcing system to align their task execution order optimally.

        임상의사결정지원시스템 (Clinical Decision Support Systems: CDSS)은 일반적으로 지식기반(Knowledge Based: KB) CDSS와 Non-KB CDSS로 구분된다. KB CDSS는 의료전문가들의 경험을 기반으로 주로 룰 형태의 지식베이스를 구축하여 작동되며, Non-KB CDSS는 다양한 의료 빅데이터에 인공지능 기술을 활용하여 직관, 분석, 판단 및 지식을 제공한다. 본고에서는 KB CDSS와 Non-KB CDSS의 특성을 결합한 Hybrid CDSS 형태의 Intelligent Medical Platform (IMP)과 그 응용 서비스인 Silo를 소개한다. Silo란 IMP가 제공하는 특정 질환에 대한 지능형 의료 추천 서비스를 일컫는다. CDSS의 핵심은 신뢰도 있는 고품질의 지식베이스 구축 및 관리인데, 전문가 지식에 기반하는 기존의 KB CDSS의 지식베이스 구축은 지식공학자에 의존적이며 지식이 증가하는 경우 유지보수가 어렵다. 반면, 다양한 의료 빅 데이터로부터 주로 딥러닝을 사용하여 지식을 획득하는 Non-KB CDSS는 의사결정과정을 알 수 없어 투명성 부족으로 사용이 제한적이다. 따라서, 양쪽의 장점을 결합한 하이브리드형 CDSS의 일종인 제안하는 IMP 플랫폼은 고품질의 지식을 보장하고 유지보수가 용이하다. 신뢰도가 높은 고품질 지식을 보장하기 위해 의료 전문가로부터 도출된 룰 형태의 (화이트박스형) 지식을 기반으로 하고 정형, 비정형, 영상 등 다양한 형태의 의료 빅데이터로부터 머신러닝 (화이트박스형) 또는 딥러닝 알고리즘 (블랙박스형)을 사용하여 획득된 직관이나 지식을 병합한다. 한편, 진화하는 대용량 지식베이스 구축을 손쉽게 하기위하여 검증된 점진적 지식모델(Incremental Knowledge Model)인 화이트박스형 Ripple Down Rules (RDR)을 사용한다. 따라서, 최종적으로 구축되는 하이브리드형 지식베이스는 화이트박스형 RDR 지식 모델을 기반으로 하기때문에 의사결정의 투명성을 보장하고 방대한 지식베이스 구축이 용이하다. 동시에, IMP가 제공하는 엔지니어링 지원도구를 사용하여 의료인들은 지식공학자들의 도움을 최소화하면서 손쉽게 지식을 획득, 병합, 검증 및 유지보수를 할 수 있다. 본고에서는 저자가 개발하고 있는 IMP의 각 구성 컴포넌트를 소개하고, 이에 기반한 지능형 의료 서비스 사례로, 전문가 기반의 갑상선 암 치료 silo, 데이터 기반의 당뇨망막병증 진단 silo, 하이브리드 기반의 심부전 진단 silo를 소개한다.
