Tenofovir disoproxil fumarate (TDF) is one of the most widely used treatment options for human immunodeficiency virus (HIV) and HBV infections. Despite its efficacy and safety, some cases of nephrotoxicity have been reported in the treatment of HIV patients. Even more recently, very few cases of Fanconi syndrome associated with tenofovir therapy in HBV monoinfection have been reported. Herein, we report a case of a 47-year-old male with an HBV monoinfection, who developed Fanconi syndrome and a secondary osteomalacia with multiple bone pain. After TDF withdrawal and supplementation of calcitriol, his renal function was reverted. Although the overall risk of TDF-associated nephrotoxicity is very low, both glomerular and tubular function should be monitored in patients undergoing TDF treatment. The Low-Lying Quadrupole Strength of 30Ne and 32Mg Is Revisited

The quadrupole strength functions in neutron-excess nuclei among N = 20 isotones are calculated by using the simple Bardeen-Cooper-Schrieffer plus quasi-particle random phase approximation method with a zero-range residual interaction. Adopting the neutron-pairing gap energy predicted by the recent Hartree-Fock-Bogoliubov calculation, we obtain reasonable results for the so-called anomalously strong B(E2) strength found in the lowest 2+ states of 30Ne and 32Mg. In fact, the results are quite similar to those obtained by a much more elaborated study which employed a residual interaction derived from the Skyrme Hamiltonian. Therefore, our outcome may indicate that the main mechanism for the strong collectivity of the low-lying quadrupole strength in 30Ne and 32Mg comes mostly from neutron-pairing correlations in the ground state and that a particular choice of the residual interaction is not so crucial in reproducing the collectivity.**";

이전에 합병증 없이 내시경 역행성 담췌관 조영술(ERCP)을 여러 번 시행받은 71세 남자 환자에게 내시경 삽입 중 식도점막 파열이 발생하였다. 점막열상은 내시경클립(endoclip)을 사용하여 성공적으로 봉합되었으나, 이 합병증으로 인해 식도암과 조기 위암이우연히 동시에 발견되었다. 십이지장경은 위장관, 특히 식도 관찰에 제약이 있기 때문에 임상적으로 중요한 병변을 놓칠 가능성이 있다. 따라서 점막 손상이나 천공을 예방하기 위해 내시경선단에 충분한 윤활제를 도포하는 것과 함께 ERCP 중 상부내시경 검사를 시행하는 것도 고려되어야 한다. 어느 환자군에서 ERCP 중 상부내시경 검사가 유용할지에 대해서는 향후 대규모 연구가 필요할 것이다. Esophageal mucosal tear occurred during scope insertion in a 71-year-old male patient who previously underwent endoscopic retrograde cholangiopancreatography (ERCP) several times without any complications. The mucosal tear was successfully sealed with endoclips using a forward-viewing scope. However, this mishap leads to the incidental discovery of both esophageal cancer and early gastric cancer. Duodenoscope has inherent limitation in observing the gastrointestinal tract, especially the esophagus, and may miss clinically significant lesions. Therefore, in addition to applying sufficient lubricant to the scope tip and considering the possibility of anatomical variation to prevent mucosal injury or perforation, performing upper endoscopy during ERCP should be considered in a certain patient population, albeit the utility of and the population benefiting from it remains to be proven by a large-scale study.
급성 하부위장관 출혈로 인하여 진단된 급성 화농성 충수염 1예

A 49-year-old man visited the emergency room of Korea University Ansan Hospital with hematochezia starting the day before the visit. Recently, he was on anti-platelet medication due to hypertension. The patient had no definite symptoms other than hematochezia. Digital rectal exam was positive and laboratory tests showed severe anemia. Sigmoidoscopy was initiated and almost no fecal material was observed in the intestinal tract, allowing insertion into the cecum. Active bleeding from the appendiceal opening was noted. On abdominal CT, contrast enhancement was observed at the tip of the appendix. Under suspicion of acute appendicitis, we consulted with a surgeon. The patient underwent appendectomy with partial cecal resection. Pathologic examination revealed a diagnosis of appendix bleeding due to acute suppurative appendicitis. The patient had no further bleeding after surgery and was discharged in a stable state. Careful observation by the endoscopist is necessary for accurate diagnosis of lower gastrointestinal hemorrhage. Appendiceal hemorrhage is very rarely reported, but it has various pathophysiologies. CT scan is useful when appendiceal hemorrhage is confirmed by endoscopic findings. Surgical treatment was needed in almost all cases reported worldwide. If bleeding from the appendix is confirmed, surgical treatment should be considered for both therapeutic and diagnostic purposes. (Korean J Gastroenterol 2019;73:45-49)

This paper is a study on the efficiency of the filtering method of signal processing and the metal detection method using deep learning for data obtained from multiple MI sensors. The MI sensor is a principle that detects changes in magnetic field and is a passive sensor that detects metal objects. However, when detecting a metal object, the amount of change in the magnetic field caused by the metal is small, so there is a limit to the detectable distance. In order to effectively detect and analyze this, a method using deep learning was applied. In addition, the performance of the deep learning model was compared and analyzed using the filtering method of signal processing. In this paper, the detection performance of CNN and RNN networks was compared and analyzed from the data extracted from the self-impedance sensor. The RNN model showed higher performance than the CNN model. However, in the shallow stage, the CNN model showed higher performance than the RNN model.

<P>Poly(dopamine)-treated graphene oxide/poly(vinyl alcohol) (“dG-O/PVA”) composite films were made and characterized. G-O was modified with poly(dopamine) in aqueous solution and then chemically reduced to yield poly(dopamine)-treated reduced G-O. A combination of hydrogen bonding, strong adhesion of poly(dopamine) at the interface of PVA and G-O sheets, and reinforcement by G-O resulted in increases in tensile modulus, ultimate tensile strength, and strain-to-failure by 39, 100, and 89%, respectively, at 0.5 wt % dG-O loading of the PVA. This paper is a study of metal detection methods using multiple MI sensors using deep learning. The MI sensor is a sensor that measures the impedance change of an atypical wire by the electrical conduction and transmittance of a detector. Simple principles, small power consumption, and compact manufacturing are possible. This is a field-available sensor that requires limited weight and power, such as a mobile environment. However, sensors are generally weak in strength due to the nature of sensors that detect changes in magnetic fields. This paper compares and analyzes the detection performance with the depth of each network for location detection via RNN on 16-channel raw data extracted from sensors in a self-impedance manner.
