Guidelines for prescribing opioids for chronic non-cancer pain in Korea

As the treatment of chronic non-cancer pain gradually increases, clinicians have more opportunities to encounter opioid prescription. However, guidelines for prescribing opioids for chronic non-cancer pain have never been published in Korea. The present guidelines were prepared by reviewing various research data. In cases in which the data were insufficient, recommendations were presented following discussion among experts affiliated with the Opioids Research Group in the Korean Pain Society. The present guidelines may need to be continuously revised and amended as more clinical evidence is acquired. (Korean J Pain 2017; 30: 18-33)
하수구 막힘 방지를 위한 모발 절단 분쇄 장치 개발 및 성능평가

가정 하수구의 주요 오물은 난분해성 모발로서 이것이 하수의 악취를 막기 위한 유트랩 장치에 다량으로 쌓이게 된다. 이러한 오물은 물의 흐름을 막는 현상을 일으키기 때문에 대부분의 가정에서 이를 해결하기 위해 상업적으로 제조된 화학적 분해 용액을 하수구에 과량 투입하여 하수구의 막힘 현상을 해결하고 있다. 본 연구에서는 난분해성 케로틴 단백질이 주성분인 모발을 절단 분쇄하는 장치를 개발하여 실험하였다. 실험을 위하여 모발을 효과적으로 절단할 수 있는 절단 분쇄장치를 설계하였으며 그 절단 성능을 실험적인 방법을 통해 규명하였다. 모발 절단 분쇄 장치의 성능을 평가하기 위하여 섬유다발을 부분 절단한 후 무게를 측정하는 방법을 사용하여 절단 모발의 평균길이를 구하였다. 이 방법은 비교적 간단한 무게 측정 방법을 통해 섬유상의 평균 길이를 구하는 것인데, 이를 모발에 적용하여 평균 모발 길이를 크지 않은 오차 범위에서 구할 수 있었다. 또한, 평균절단수를 정의하여 본 연구를 위해 고안된 모발 절단 분쇄 장치의 성능을 평가하였다. 평균절단수 분석 방법을 적용하여 주어진 실험 조건에서 최적의 구조와 최적의 작동시간을 구하는 기준을 제시할 수 있었다. 이러한 실험 방법과 결과는 섬유형태 물질의 절단 분쇄가 필요한 다른 장치에서도 설계와 분석에 사용할 수 있는 하나의 좋은 예시가 될 것이다. 본 연구에서 고안된 절단 분쇄 장치가 실험을 통하여 효과적으로 모발을 절단 분쇄하는 것을 확인할 수있었다. 이러한 장치를 일반 가정에 설치하여 보급함으로써 최근 심각해지고 있는 하수구에 의한 수질환경 오염 방지에도 효과적인 도움이 될 것이다. The recalcitrant hairs which are accumulated in the U-trap equipped to prevent the stench from the drainage produce the phenomenon of blocking the flow of water through the home drainage pathway. The chemical dissolution solutions commercially manufactured are excessively poured into U-trap to solve this blocking problem. In this study, the specially designed instrument was developed for the experiments to cut and crush the hairs of which main component are the recalcitrant keratin proteins. The performance of the instrument for cutting and crushing hairs was investigated using experimental methods. The cutting and weighing method was used in order to obtain the average length of a large amount of hairs cut by instrument. This method is relatively simple to measure the weight of cutting hairs to obtain the average length of the fibers, the values of average length showed not greater tolerances. Also the average cutting number was defined to evaluate the performance of the cutting and crushing instruments designed for this study. We were able to apply these evaluation methods to provide the criteria to obtain the optimal structure of instrument and proper operation time in the given experimental conditions. These experimental methods and results will provide the good example to design and to analyze the various device that can be used for cutting the fibers. The cutting and crushing instrument developed in this study showed the effective abilities to cutting the hairs. The general using the cuttting and crushing instrument at every home would be helpful to prevent water environmental pollution.

<P>Estrogen-related receptor gamma (ERR gamma) is a major positive regulator of hepatic gluconeogenesis. Its transcriptional activity is suppressed by phosphorylation signaled by insulin in the fed state, but whether posttranslational modification alters its gluconeogenic activity in the fasted state is not known. Metabolically active hepatocytes direct a small amount of glucose into the hexosamine biosynthetic pathway, leading to protein O-GlcNAcylation. In this study, we demonstrate that ERR gamma is O-GlcNAcylated by O-GlcNAc transferase in the fasted state. This stabilizes the protein by inhibiting proteasome-mediated protein degradation, increasing ERR gamma recruitment to gluconeogenic gene promoters. Mass spectrometry identifies two serine residues (S317, S319) present in the ERR gamma ligand-binding domain that are O-GlcNAcylated. Mutation of these residues destabilizes ERR gamma protein and blocks the ability of ERR gamma to induce gluconeogenesis in vivo. The impact of this pathway on gluconeogenesis in vivo was confirmed by the observation that decreasing the amount of O-GlcNAcylated ERR gamma by overexpressing the deglycosylating enzyme O-GlcNAcase decreases ERR gamma-dependent glucose production in fasted mice. We conclude that O-GlcNAcylation of ERR gamma serves as a major signal to promote hepatic gluconeogenesis.</P>

New malting barley variety Doosan # 29 is uniform and early maturing, high tillering, good Quality and high yielding. This variety was developed from the cross Sacheon # 1×Zikukei No. 1537 made in 1979 at Sacheon Malting Barly Experiment Station, Doosan Farm’s Co, Ltd. This variety was determined to recommend to the all malting barley production areas in southern part of Korea from 1988.
늑막액에서 Cholesterol 측정의 진단적 의의에 관한 연구

N/A The cholesterol of pleural fluids was measured in 30 patients various diseases with pleural effusion to investigate the utility of differential diagnosis of exudates from transudates. Six cases were pleural transudates and 24 cases were pleural exudates as by classified predefined criteria. The results were as follows: 1) Mean protein level in group 1 (transudates) was l.9±0.8 g/dl and 4.8±0.9 g/dl in group 2 (exudates). Mean pleural fluid to serum ratio in group 1 was 0.30±0.11 and 0.80±0.48 in group 2. 2) Mean LDH level was 40X29 IU in group 1 and 325+271 IU in group 2. Mean pleural fluid to serum ratio was 0.20±0.11 in group 1 and 2.01±1.90 in group 2. 3) Mean cholesterol level was 37±14 mg/dl in group 1 and 97±42 mg/dl in group 2. Mean pleural fluid to serum ratio was 0.18±0.06 in group 1 and 0.71±0.32 in group 2. 4) A protein level of 3.0 g/dl, LDH level of 200 IU, cholesterol level of 60 mg/dl and their pleural fluid to serum ratios of 0.5, 0.6 and 0.3 respectively were used as dividing lines separating transudates from exudates, and 0% was misclassified in group 1. Misclassified effusions in group 2 for each single parameter were: protein 0%, protein ratio 4%, LDH 29%, LDH ratio 20%, cholesterol 20%, cholesterol ratio 0%, and Light criteria 4%. We found the use of cholesterol level to be superior to the measurement of LDH level and inferior to protein level. The ratio of pleural fluid to serum cholesterol markedly improved the result. On the basis of the above results, it is suggested that pleural fluid cholesterol is a useful index in differentiating exudative from transudative pleural effusions.

N/A Short-term, high dose corticosteroid therapy is often required for control of acute asthma episodes. To evaluate possible Hypothalamic-Pituitary-Adrenal (HPA)-axis suppression and recovery after such therapy, we studied 7 patients with adult acute asthma before and at 1, 3 and 7 days after completion of a 12 day course of corticosteroid. Corticosteroid was administered as a loading dose (methylprednisolone which was equivalent to 4 mg of hydrocortisone/kg body weight) and maintenance dose (equivalent to 3mg of hydrrcortisone/kg body weight/6 hours) for 3 days, followed by prednisolone (usual start dose was prednisolone 60 mg as a single daily morning dose). The dose was then reduced in half every 3 days. The function and reserve of the HPA-axis were evaluated with basal plasma cortisol, ACTH and short corticotropin stimulation tests. The results obtained were as follows: 1. Baseline plasma cortisol, ACTH, and cortisol responses to corticotropin before corticosteroid treatment were 10.3±5.6 ㎍/dl, 25.4±8.4 /㎍/ml, and 17.7±6.2 ㎍/dl, respectively. 2. One day after corticosteroid therapy, plasma cortisol and ACTH levels (3.9±2.4 ㎍/dl, 9.2±9.0 pg/ml) were significantly reduced compared to pretreatment levels (p<0.01. p<0.01), but the cortisol responses to corticotropin were preserved. 3. Three days after concluding the corticosteroid therapy, plasma cortisol and ACTH levels had returned to 68.9% and 71.7% of the pretreatment levels respectively, and were restored to near pretreatment baseline levels 7 days after treatment. These data suggest that a brief course of high dose corticosteroid treatment may limit the adrenal component of HPA responses for up to 7 days and patients may be at risk if they encounter major surgery or infection during this time.
