This study examined perceived discomforts encountered in residential facilities such as individual houses and apartments, and suggested a procedure for improving the problems in the living residence. The participants consisted of two groups of people: normal and underprivileged people; the underprivileged group was classified into 3 sub-groups: elderly people (over 65 years old), handicapped people and pregnant women. To identify inconvenient places and design factors in the residential housing, a total of 200 subjects (50 for each group) participated in the survey using the 7-point Likert scale. As a result, all the user groups mainly answered a high degree of discomfort due to narrow parking places, doorsills, and height differences between floors. The elderly, the handicapped, and the pregnant women replied that it is uncomfortable to use ramps and utility rooms. In a follow-up study, one-to-one in-depth interviews were conducted with some of the participants to analyze the detailed causes of inconvenient places and design factors. This study also suggested the procedure for solving the problems in various design factors from perspective of universal design, by considering prior studies which focused on a certain user group like the elderly. This procedure was suggested to consider as many user groups as possible at the same time. The results can be used to understand the discomforts of various user groups on residential housing. The procedure can also be used to develop universal design guidelines for the living residence.

Although the height of a ramp is an important design element, it has not been considered in prior studies. Therefore, in this study, the ramp slope and height are considered as independent variables. To analyze the effects of the slope and height, five levels of slope (1:6, 1:8, 1:10, 1:12 and 1:14) and three levels of height (15㎝, 30㎝ and 45㎝) are considered. For the dependent variables, the total time, velocity and perceived discomfort were considered as usability measures, pulse rate changes and EMG signals of four related muscles (extensor carpi radialis, triceps brachii, anterior deltoid and posterior deltoid) were considered as physiology measures. As a result, differences among usability and physiological characteristic for the five slopes increased as the height increased. Additionally, slope effects were minor when the height was low (15㎝). Almost domestic/international regulations and guidelines related to ramp recommended 1:12 slope for the ramp design, however, there was no significant difference between 1:10 and 1:12 according to result of this study. In addition, slope effects were minor at a low height; thus, a slope of 1:8 can be recommended if the installation space for a gentler ramp is not sufficient.
IgA nephropathy in a laboratory worker that progressed to end-stage renal disease

Background: IgA nephropathy (IgAN) is the most common form of glomerulonephritis, a principal cause of end-stage renal disease (ESRD) worldwide. The mechanisms of onset and progression of IgAN have not been fully revealed, and epidemiologic studies have yielded diverging opinions as to the role of occupational exposure to organic solvents in the initiation or worsening of IgAN. As the authors encountered a laboratory worker with IgAN that progressed to ESRD, we present a case report of IgAN progression due to dichloromethane exposure along with a review of literature. Case presentation: A 41-year-old male laboratory worker began to experience gross painless hematuria after two years of occupational exposure to toluene. Although clinical follow-up was initiated under the impression of IgAN based on clinical findings, the patient continued to work for four more years in the same laboratory, during which he was in charge of laboratory analysis with direct exposure to a high concentration of dichloromethane without proper protective equipment. During that time, his renal function rapidly worsened and finally progressed to ESRD 10 years after the first clinical symptoms. The result of exposure assessment through reenactment of his work exceeded the occupational exposure limit for dichloromethane to a considerable degree. Conclusions: The causal association between occupational solvent exposure and IgAN is still unclear; therefore, this case report could be used as a basis to support the relevance of occupational solvent exposure to IgAN and/or its progression. Early intervention as well as close monitoring of laboratory workers exposed to various organic solvents is important to prevent or delay the progression of glomerulonephritis to ESRD in the occupational setting.

This study investigated perceived discomforts and those reasons of the young and middle-aged, the aged, the disabled and the pregnant in various daily activities, and elicited universal design factors based on human capacities (physical, sensory, cognitive and affective). A total of eighty people (twenty subjects for each user group) participated in one-to-one interviews about thirty six items of daily activities. A degree of discomfort of the daily activities was measured using Borg's CR10 scale. As a result, the disabled and the pregnant complained about almost all of the daily activities, and their degrees of discomforts were significantly higher than those of the youth and middle-aged and the aged. However, the youth and middle-aged and the aged only complained about some specific items. The major reasons of discomforts were related to physical and affective capabilities in general, where the detailed reasons of each user group were comparatively different. Finally, the universal design factors were elicited for each daily activity item based on the results of each user group. This study can be used to understand the discomforts and characteristics of the aged, the disabled and the pregnant in the various daily activities. The universal design factors elicited can also be applied to design related products and facilities universally.

The information about time spent in microenvironments plays a critical role for an exposure assessment of the person concerned, considering the personal exposure relies on the characterization of activity patterns of the population at risk and human activities impact the timing, location, and level of personal pollutant exposure. The purposes of this paper were to present indoor time activity patterns of Korean from a population-based study and to determine individual factors of time spent in microenvironments. The population based study collected time activity pattern of about 32,000 Korean for two consecutive days. The mean times spent at home, other indoors, outdoors, and transportation in related to the whole sample of 19.025 people are 14.23hrs(59.3%), 6.80hrs(28.3%), 1.26hrs(5.2%), and 1.75hrs(7.3%) in weekday, respectively. Database provides information on how the proportion of persons in different locations changes by time of day in weekday. Here, we see that over 90% of respondents were in a residence from about 11 PM to 5 AM, and the largest proportions of respondents in offices, factories, schools and public buildings occur between 8 AM and 5 PM.
