        상악동을 침범하는 역위성 유두종의 증례보고

        The present study reports a case of inverted papilloma of the nasal cavity and infiltrating into the maxillary sinus. Inverted papilloma is an uncommon and locally aggressive benign tumor of the sinonasal region. The patient, 51-year-old male, presented with unilateral nasal obstruction and periodic swelling on the palate without pain. Enhanced CT scan revealed a heterogeneously enhancing solid mass in the nasal cavity and infiltrating into the right maxillary sinus, as well as an incidental, secondarily infected residual cyst m the periapical area of the right maxillary canine. The sinonasal mass was revealed as an inverted papilloma on histopathologic examination.

        디지털 측방두부규격방사선사진과 MDCT의 3차원 재구성 영상을 이용한 합성측방두부규격방사선사진의 계측치 비교 및 머리 위치가 미치는 효과

        Purpose : To investigate the reproducibilities and compare the measurements in digital and MDCT-synthesized cephalometric radiograph, and to investigate the effect of head position on the measurement during imaging with MDCT. Materials and Methods : Twenty-two dry skulls (combined with mandible) were used in this study. Conventional digital cephalometric radiograph was taken in standard position, and MDCT was taken in standard position and two rotated position (10°left rotation and 10°right tilting). MDCT data were imported in OnDemand?? and lateral cephalometric radiograph were synthesized from 3D virtual models. Two types of rotated MDCT data were synthesized with default mode and with corrected mode using both ear rods. For all six images, sixteen angular and eleven linear measurements were made in V-Ceph?? three times. Reproducibility of measurements was assessed using repeated measures ANOVA and ICC. Linear and angular measurements were compared between digital and five MDCTsynthesized images by Student t-test. Results : All measurements in six types of cephalometric radiograph were not statistically different under ICC examination. Measurements were not different between digital and MDCT-synthesized images (P> .05). Measurements in MDCT-synthesized image in 10°left rotation or 10°right tilting position showed possibility of difference from digital image in some measurements, and possibility of improvement via realignment of head position using both ear rods. Conclusion : MDCT-synthesized cephalometric radiograph can substitute conventional cephalometric radiograph. The error on head position during imaging with MDCT have possibility that can produce measurement errors with MDCT-synthesized image, and these position error can be corrected by realignment of the head position using both ear rods.         CT 영상을 이용한 악골 골수염의 경조직 변화에 관한 연구

        Purpose : To assess the clinical findings and hard tissue changes of osteomyelitis of the jaws using computed tomographic (CT) image analysis. Materials and Methods : We reviewed and interpreted the CT images of 163 patients (64 males and 99 females, age range from 10 to 87 years) who visited the Seoul National University Dental Hospital from April 23, 2006 to December 31, 2008 and were diagnosed as osteomyelitis of the jaws through clinical, radiologic, and histopathologic examination. Each CT findings was investigated for frequency, correlation with age and gender. Results : Of the 163 patients, 31 (19.0%) were affected on the maxilla, 135 (82.8%) were affected on the mandible, and 3 (1.8%) were affected on the both jaws simultaneously. The mean age of the patients who were affected on the maxilla was 61.0 years and that of the patients who were affected on the mandible was 56.2 years. On the maxilla, the most frequent site of disease was the posterior area (83.9%) and on the mandible, mandibular body (83.0%), followed by angle (48.1%), ramus (38.5%), condyle (13.3%), incisal area (9.6%), and coronoid process (3.0%). Among the 31 maxillary osteomyelitis, defect in the trabecular bone was observed in 28 (90.3%), osteosclerosis 20 (64.5%), defect in the cortical bone 27 (87.1%), sequestrum 17 (54.8%), and periosteal reaction 2 (6.5%). Among the 135 mandibular osteomyelitis, defect in the trabecular bone was observed in 100 (74.1%), osteosclerosis 104 (77.0%), defect in the cortical bone 116 (85.9%), sequestrum 36 (26.7%), and periosteal reaction 67 (49.6%). Conclusion : Of our cases, the maxillary osteomyelitis was visibly observed more frequently in females than males. The incidence is the highest in seventies (28.8%) and the lowest in teens (3.1%). The osteomyelitis of the jaws was observed more frequently in males than females before the age of 50, and observed more frequently in females after the age of 50. The most noticeable point was that the sequestrum was observed more often on maxillary osteomyelitis and the periosteal reaction was observed more often on mandibular osteomyelitis.       • Factors associated with prediction of residual lesion in conization

        We investigated factors affecting margin involvement, follow-up cytology and high risk human papillomavirus (HPV) testing within 6 months after conization, following surgical treatment after conization as predictors of incomplete excision in conization. One thousand four hundred forty six patients who underwent conization due to diagnose and treat cervical intraepithelial neoplasia (CIN) in Gangnam Serverance and Severance Hospital from January 2008 to December 2011 were included in this retrospective study. We analyzed their clinical and pathologic characteristics. Aging (odd ratio [OR], 1.039; 95% confidence interval [CI], 1.022-1.056), high grade group of preconization cytology(OR, 1.800; 95% CI, 1.333-2.431), preconization high risk HPV testing positivity (OR, 5.215; 95% CI, 1.824-5.215) were statistically significant factors that affect conization margin positivity. The width of specimen in conization was predictive factor for follow-up cytology within 6 months after conization (OR, 0.579; 95% CI, 0.370-0.906). Low grade group of preconization cytology (OR, 0.619; 95% CI, 0.426-0.900), preconizaton HPV testing positivity (OR, 4.726; 95% CI, 2.172-10.28) were significantly different in follow-up high risk HPV testing within 6 months after conization. Aging (OR, 1.066; 95% CI, 1.044-1.090), high grade group of preconization cytology (OR, 2.293; 95% CI, 1.575-3.338), preconization high risk HPV testing (OR, 4.560; 95% CI, 2.158-9.639), local anesthesia (OR, 1.885; 95% CI, 1.136-3.128) were relative to the following surgical treatment after conization. It is important to predict residual lesion after conization that affected by age, parity, preconization cytology and high risk HPV testing, anesthesia. These findings could be used to determine appropriate follow-up protocols.

      • Clinical manifestations and survival outcomes in patients with endometrial cancer according to concurrent endometriosis: a single center, pilot study

        Objective: Endometriosis may be a risk factor of endometrial cancer based on previous studies. we intended to investigate clinical manifestations and survival outcomes related to endometriosis in patients with endometrial cancer. Methods: In this retrospective study, medical records of patients with endometrioid type endometrial cancer who were histologically confirmed in our hospital between Janunary 2010 and September 2016 were reviewed. Clinical manifestations and prognosis were compared in groups with or without endometriosis using Mann-Whitney test and Fishers exact test. Results: In patients with endometrioid type endometrial cancer, 10.4% (5/48) and 89.6% (43/48) were included into + endometriosis group and - endometriosis group, respectively. Stage distributions were as follows: in + Endometriosis group, stage 1A (n=4) and stage II (n=1); in - Endometriosis group, stage 1A (n=20), stage 1B (n=7), stage II (n=7), stage III (n=7), and stage IV (n=2). Grade distributions were as follows: in + Endometriosis group, grade 1 (n=3) and grade II (n=2); in - Endometriosis group, grade 1 (n=17), grade II (n=14), and grade III (n=12). Age at diagnosis (meanSE, 54.23.2 vs 51.62.4, p=0.761), BMI at diagnosis (meanSE, 27.83.2 vs 26.10.8, p=0.555), and rates of nulliparity (20% (1/5) vs 26.2% (11/42), p=1.000) and postmenopausal state (60% (3/5) vs 66.8% (24/43), p=1.000) were not different between + and - endometriosis groups. Moreover, recurrent rate (0% (0/5) vs 14.3% (6/42), p=1.000), and overall survival and disease specific survival rates (0% (5/5) vs 90.7% (39/43), p=1.000) showed no different between + and - endometriosis groups. Follow-up perioid was 30 months (range, 3-85) and 39 months (range, 2-133) in + and - endometriosis groups, respectively. Conclusion: In this pilot study, clinical manifestations and prognosis of endometrial cancer were not influenced by endometriosis. Large scale multicenter studies are needed to further clarify association between endometrial cancer and endometriosis.

        Group Locomotor Imagery Training-Combined Knowledge of Performance in Community-Dwelling Individuals With Chronic Stroke: A Pilot Study

        We evaluated whether group locomotor imagery training-combined knowledge of performance (KP) lead to improvements in gait function in community dwelling individuals with chronic stroke. Ten adults who had suffered a hemiparetic stroke at least 6 months earlier participated in group locomotor imagery training-combined KP for 5 weeks, twice per week, with 2 h intensive training. Dynamic gait index scores increased significantly after the group locomotor imagery training-combined KP. However, times for the timed up-and-go test did not improve significantly after the training. Group locomotor imagery training-combined KP may be a useful option for the relearning of gait performance for community dwelling individuals with chronic hemiparetic stroke.

      • 오갈피 열매의 주요성분 동정 및 분석법 검증에 관한 연구

        Background : Acanthopanax sessiliflorus (Rupr. et Maxim) Seem, belonging to the Araliaceae family, is widely distributed in Korea, China, and Japan. The plants belonging to Acanthopanax species are traditionally used in Korea as anti-rheumatoid arthritis, anti-inflammatory and anti-diabetic drugs and are recognized to have ginseng-like activities. A simple and sensitive high-performance liquid chromatographic (HPLC) method was developed and validated for independent analysis of major compounds and chlorogenic acid in A. sessiliflorus fruits. Chlorogenic acid was reported that prevent cancer and cardiovascular disease in vivo. Also, it has antioxidant effect in vitro test. In the previous experiment, chlorogenic acid were found in A. sessiliflorus fruits. This study was performed to identification of the major compounds and investigate the method validation for the determination of chlorogenic acid in A. sessiliflorus fruits. Methods and Results : Three major compounds were recorded on a Varian Unity Inova AS-400 FT-NMR spectrometer and analyzed by the new HPLC analysis method. HPLC analysis was carried out using an Waters e2695 and PDA detector. The new analyasis method was validated by the measurement of intra-day, inter-day precision, accuracy, limit of detection (LOD, S/N=3), and limit of quantification (LOQ, S/N=10) of chlorogenic acid. The results showed that the correlation coefficient (R2) for the calibration curves of chlorogenic acid was 0.997 in terms of linearity. The limit of detection (LOD) and limit of quantification (LOQ) were 0.565 ㎍/ml and 2.88 ㎍/ml, respectively. There was no interfering peak observed each other and HPLC system was suitable for analysis showing goodness of peak and high precision. Conclusion : This method is suitable to detect and quantify major compounds in A. sessiliflorus fruits. Furthermore, the result will be applied to establish chlorogenic acid as an standard compound for A. sessiliflorus fruits.

      • A case of apocrine hidrocystoma on back

        Apocrine hidrocystoma is benign cystic neoplasm of the secretory segment of apocrine sweat glands. Generally it occurs in middle-aged woman, as a solitary asymptomatic smooth skin colored papule. The usual location is on face, but occasionally on ear, scalp, chest and vulva. Clinically and histologically apocrine hidrocystoma resemble eccrine hidrocystoma. But unlike eccrine hidrocystoma, it does not relate to temperature. A 59-year-old male presented one asymptomatic bluish nodule on back. The papule tended to expand over time. Histologic examination revealed multilobular thin-walled cyst lined by columnar epithelium with decapitation secretion. Secretory epithelium was positive for gross cystic disease fluid protein 15 stain and contained periodic acid-schiff stain positive cytoplasmic granules. As a result, the patient was diagnosed with apocrine hidrocystoma. It is extremely rare for apocrine hidrocystoma developing on back. Herein, we report a case of apocrine hidrocystoma which developed in an unusual location.
