        We have studied that dielectric and electrical properties of (Ba_(0.5),Sr_(0.5))TiO_(3) thin films deposited on Indium Tin Oxide-coated glass substrate by using rf-magnetron sputtering method in this paper. Substrates were heated at room, 300℃ , 400℃, and 500℃. Working pressure was changed 5mTorr, 10mTorr, 20mTorr, 30mTorr respectively substrates temperature, and Ar:O_(2), ratio was fixed 9:1. SEM analysis was conducted to investigate thickness of BST thin films. Dielectric constant, current-voltage(I-V), and transmittance were measured. We observed difference of that thickness, dielectric constant, current-voltage(I-V), and transmittance due to variable substrates temperature and working pressure. We also obtained best conditions at 400℃, 30mTorr. Dielectric constant was 209.1 at 1kHz, leakage current density was below 7.35X10^(-7)A/cm^(2) at 100V, and transmittance was over 91%.

        위선종과 조기위암의 내시경적 점막 절제술 후 추적관찰에 대한 연구

        목적: 내시경 점막 절제술은 위암의 선행병변으로 알려진 위선종과 조기위암의 치료로 보다 덜 침습적이고 안전한 방법으로 널리 주목받고 있다. 저자들은 EMR 시술 후 1년간의 추적관찰을 통해 위 선종과 조기 위암에 대한 EMR 치료의 효과와 유용성에 대하여 알아보고자 하였다. 방법: 2000년 6월부터 2004년 9월까지 위선종 또는 조기위암으로 진단된 환자 중 EMR을 시행하고 이후 6개월 이상 추적관찰이 가능했던 환자 54명, 54예를 대상으로 하여, 내시경 생검 조직과 EMR 후 조직 소견을 비교한 뒤, 이후 1년간 3개월 마다 내시경 및 조직 검사를 시행하여 국소 재발 여부를 추적관찰 하였다. 결과: 환자는 남자가 42명, 여자가 12명으로 평균 연령은 60세였고, 조직 소견에서는 위 선종이 42예, 조기위암이 12예였다. EMR 전후의 병리학적 진단에 차이가 있었던 경우는 총 9예로 그 중 2예는 위선종에서 조기위암으로 진단이 바뀐 경우였다. 완전 절제는 총 48예로 일괄 절제시 34예 중 33예(97%), 부분 절제시 20예 중 15예(75%)였고, 일괄 절제시 완전 절제율이 높았다. 재발은 총 4예(7.1%)로 평균 재발 기간은 7개월이었으며, 위선종에서 42예 중 3예(7.1%), 조기위암에서 12예 중 1예(8.3%)였고, 이를 절제 방법에 따라 나누면 일괄 절제 34예 중 1예(2.9%), 분할 절제 20예 중 3예(15%)였다. 결론: EMR은 위선종과 일부 조기위암의 치료법으로 안전하고 효과적인 방법으로 인식되고 있으나 조기위암의 림프절 전이 가능성과 다발성 병변의 가능성 면에서는 한계가 있다. 그러므로 이에 대한 시술 후 정기적인 추적검사는 필수적이다. 그러나 향후 지속적인 연구와 임상 경험이 축적된다면, 위선종 및 조기 위암에 대한 표준 치료로서 그 가치가 확립될 수 있으리라 기대한다. Background: Endoscopic mucosal resection (EMR) is now widely accepted as a useful treatment method for gastric adenoma and early gastric cancer (EGC) because of its minimal invasiveness and satisfactory post-procedure results. The purpose of this study is to define the follow-up results and usefulness of EMR. Methods: We analyzed 54 cases from June 2000 through September 2004. Endoscopy with histological examination was carried out every 3 months for 1 year after EMR. Results: The patients consisted of 42 men and 12 women, and the mean age was 60 years old. The histological results were 42 gastric adenoma and 12 EGC cases. There were 9 cases that had the histological diagnosis changes after EMR. Complete resections was performed for 48 cases and the en block resections were 33 of 34 cases (97%) and piecemeal resections were done in 15 of 20 cases (75%). Recurrence was seen in 4 cases (7.1%), and the mean recurrence period was 7 months. There were 3 gastric adenomas of 42 cases (7.1%), one case of EGC of 12 cases (8.3%), one en block resection of 34 cases (2.9%) and three piecemeal resections of 20 cases (15%). Conclusions: EMR is a safe and useful treatment method for gastric adenoma and EGC. However, EMR has some limitations that EGC may have lymph node metastases or multiple tumors. So, periodic follow-up is very important. As we acquire more clinical experience, EMR may be accepted as the standard treatment method for gastric adenoma and EGC.(Korean J Med 71:483-490, 2006)

        신소재 복합재료를 이용한 비굴착 지하매설관 보수 - 보강공법

        기존 굴착식 기술의 단점을 극복하기 위해 다양한 비굴착 보수-보강기술이 개발되고 시도되었으나, 이제까지 개발된 비굴착 기술들은 높은 공사비용과 시공의 불편함이라는 결점을 갖고 있다. 본 연구에서는 VARTM(Vacuum Assisted Resin Transfer Molding)과 유리섬유 고분자 복합재료를 이용해 지하매설관을 보수-보강하는 연구를 수행하였다. 개발된 공법은 기존 기술보다 적은 비용과 공사 시간을 필요로 하며 사용되는 장비 또한 간단하다. 신뢰성 있는 공정을 위해 유연한 금형의 역할을 하는 보강재에 수지를 주입한 후, 가압과 진공을 가하는 방법이 시도되었으며, 상용 유전장치인 LACOMCURE를 사용하여 RTM 공정 중 수지의 함침과 경화 상태를 추적하였다. 연구결과를 통해, 적절한 공정 변수 및 온라인 경화 모니터링 기법을 적용한 본 공법이 기존 방법에 비해 많은 장점을 가짐을 알 수 있었다. To overcome the disadvantages of conventional excavation technology, various trenchless (or excavation free, or no-dig) repair-reinforcement technologies have been developed and tried. But trenchless technologies so far developed have some drawbacks such as high cost and inconvenience of operation. In this study, a repairing-reinforcing process for underground pipes with glass fiber fabric polymer composites using VARTM (Vacuum Assisted Resin Transfer Molding) has been developed. The developed process requires shorter operation time and lower cost with smaller and simpler operating equipments than those of the conventional trenchless technologies. For the reliable operation of the developed method, a simple method to apply pressure and vacuum to the reinforcement was devised and flexible mold technology was tried. Also, resin filling and cure status during RTM process were monitored with a commercial dielectrometry cure monitoring system, LACOMCURE. From the investigation, it has been found that the developed repairing-reinforcing technology with appropriate process variables and on-line cure monitoring has many advantages over conventional methods.

        Excitatory Effect of $M_1$ Muscarinic Acetylcholine Receptor on Automaticity of Mouse Heart

        We have investigated the effects of relatively high concentration of carbachol (CCh), an agonist of muscarinic acetylcholine receptor (mAChR), on cardiac automaticity in mouse heart. Action potentials from automatically beating right atria of mice were measured with conventional microelectrodes. When atria were treated with $100{\mu}M$ CCh, atrial beating was immediately arrested and diastolic membrane potential (DMP) was depolarized. After exposure of the atria to CCh for $\~4 min$, action potentials were regenerated. The regenerated action potentials had lower frequency and shorter duration when compared with the control. When atria were pre-exposed to pirenzepine $(1{\mu}M)$, an $M_1$ mAChR antagonist, there was complete inhibition of CCh-induced depolarization of DMP and regeneration of action potentials. Pre-exposure to AFDX-116 (11 ({2-[(diethylamino)-methyl]-1-piperidyl}acetyl)-5, 11-dihydro-6H-pyridol[2,3-b][1,4] benzodiazepine-6-one base, $1{\mu}M$), an $M_2$ mAChR antagonist, failed to block CCh-induced arrest of the beating. However, prolonged exposure to CCh elicited gradual depolarization of DMP and slight acceleration in beating rate. Our data indicate that high concentration of CCh depolarizes membrane potential and recovers right atrial automaticity via $M_1$ mAChR, providing functional evidence for the role of $M_1$ mAChR in the atrial myocytes.

        Prescription Patterns of Oral Corticosteroids for Asthma Treatment and Related Asthma Phenotypes in University Hospitals in Korea

        Purpose: Oral corticosteroids (OCSs) are frequently prescribed for asthma management despite their adverse effects. An understanding of the pattern of OCS treatment is required to optimize asthma treatment and reduce OCS usage. This study evaluated the prescription patterns of OCSs in patients with asthma. Methods: This is a retrospective multicenter observational study. We enrolled adult (≥18 years) patients with asthma who had been followed up by asthma specialists in 13 university hospitals for ≥3 years. Lung function tests, the number of asthma exacerbations, and prescription data, including the days of supply and OCS dosage, were collected. The clinical characteristics of OCS-dependent and exacerbation-prone asthmatic patients were evaluated. Results: Of the 2,386 enrolled patients with asthma, 27.7% (n = 660) were OCS users (the median daily dose of OCS was 20 mg/day prednisolone equivalent to a median of 14 days/year). OCS users were more likely to be female, to be treated at higher asthma treatment steps, and to show poorer lung function and more frequent exacerbations in the previous year than non-OCS users. A total of 88.0% of OCS users were treated with OCS burst with a mean dose of 21.6 ± 10.2 mg per day prednisolone equivalent to 7.8 ± 3.2 days per event and 2.4 times per year. There were 2.1% (51/2,386) of patients with OCS-dependent asthma and 9.5% (227/2,386) with exacerbation-prone asthma. These asthma phenotypes were consistent over the 3 consecutive years in 47.1% of OCS-dependent asthmatic patients and 34.4% of exacerbation-prone asthmatic patients when assessed annually over the 3-year study period. Conclusions: We used real-world data from university hospitals in Korea to describe the OCS prescription patterns and relievers in asthma. Novel strategies are required to reduce the burden of OCS use in patients with asthma.

        The Mildly Elevated Serum Bilirubin Level is Negatively Associated with the Incidence of End Stage Renal Disease in Patients with IgA Nephropathy

        <P>Oxidative stress plays various roles in the development and progression of IgA nephropathy, while bilirubin is known as a potent antioxidant. We therefore hypothesized that serum bilirubin would be associated with renal prognosis in IgA nephropathy. The study subjects comprised 1,458 adult patients with primary IgA nephropathy in Korea. We grouped patients according to the following quartile levels of bilirubin: <0.4 mg/dL (Q1), 0.4-0.5 mg/dL (Q2), 0.6-0.7 mg/dL (Q3), and >0.8 mg/dL (Q4). The outcome data were obtained from the Korean Registry of end-stage renal disease (ESRD). Eighty patients (5.5%) contracted ESRD during a mean follow-up period of 44.9 months. The ESRD incidences were 10.7% in Q1, 8.2% in Q2, 2.8% in Q3, and 2.8% in Q4 (<I>p</I><0.001). The relative risk of ESRD compared to that in Q1 was 0.307 (95% confidence interval [CI], 0.126-0.751) in Q3 and 0.315 (95% CI, 0.130-0.765) in Q4. The differences of ESRD incidence were greater in subgroups of males and of patients aged 35 yr or more, with serum albumin 4.0 g/dL or more, with normotension, with eGFR 60 mL/min/1.73 m<SUP>2</SUP> or more, and with proteinuria less then 3+ by dipstick test. In conclusion, higher bilirubin level was negatively associated with ESRD incidence in IgA nephropathy.</P>
