      • 교합성 골수강내 금속정으로 치료한 대퇴골 골절의 치료

        대퇴골 간부 골절시 교합성 골수강내 금속정을 이용한 견고한 고정은 조기 관절 운동과 조기 체중부하가 가능한 보편적인 수술적 치료방법으로 그 사용이 증가하고 있다. 그리고 최근에는 골수강내 금속정이 대퇴골 간부 골절 뿐만 아니라 대퇴골의 근위부나 원위부에 치우친 골절이나 청소년기의 대퇴골 간부 골절의 치료에도 확대되고 있다. 본 논문은 대퇴골 골절 환자 54례를 대상으로 금속정 삽입술의 적응증의 확대와 그에 수반되는 문제점에 관하여 증례 분석 및 결과를 문헌 고찰과 함께 보고하고자 한다. The rigid internal fixation of femur shaft fracture with intramedullary nailing enables early joint motion and weight bearing. This procedure has been used with wide popularity because it is possible simultaneously to preserve the range of motion and to obtain the bone union. But in the proximal and distal femur fracture, the interlocking screws are too close to the fracture site, the rigid fixation cannot be achieved and when early weight bearing is permitted, failure of the metal is possible. Also, when femur fractures in the adolescence be treated with traction followed by casting, sometimes angular deformity and shortening may develope. These problems have solved to the use by intramedullary nailing. The authors reviewed and analyzed 54 cases of the fractures of femur treated by interlocking nails at Sang-Gye Paik Hospital from August 1989 to July 1992. The results were as follows ; 1.Among the 54 patients, male were 42 and female were 12. 2.The mean time of bone union was 19.3 weeks. 3.The authors usually used the closed locked nailing technique to fix the mid-shaft fracture of femur. In cases such as mid-shaft fracture combined wish proximal or distal femoral fracture, for the stability of fracture and the early mobilization, a reconstruction nail or modified nailing technique was used and an excellent bone union was obtained without any complication. 4.In a mid-shaft fractures of the femur in adolescents or young adult, the authors used interlocking nails of proper length or diameter by cutting-off the distal portion of the nail or by over-reaming of the too narrow medullary canal, and the outcome was favorable.

      • 암남공원의 식물상과 삼림식생에 관한 연구

        The objective of this study is to analyze the flora and forest vegetation of Amnam urban natural Park. 10 transects of quadrat of 10×10m were selected for the experiment. They were surveyed from January to October 1997. The obtained results were summerized as follows ; 1. The flora of plants distributed in Amnam Park was 88 families, 245 genus, 374 species. Dicotyledom in an angiosperm was 81.3% of total percentage was 73 families, 194 genus, 304 species, Monocotyledonous plant was 13.9% of total percentage was 7 families, 38 genus, 52 species, Gymnospermous plant was 2.1% of total percentage was 4 families, 5 genus, 8 species. Pteridophyta was 2.4% of total percentage was 3 families. 7 genus, 9 species, Shave grass was 0.3% of total percentage was 1 family, 1 genus, 1 species. 2. The representative wood plants of Amnam Park were Pinus thunbergii, Machilus thunbergii, Camellia japonica, Quercus spp., Celtis sinensis, Morus bombysis, Prunus sangentii, Sorbus alnifolia, Albizzia julibrissin, Juniperus rigida, Platycaya strobilacea, Lespedeza spp., Zanthoxylum piperitum, Zanthoxylum schinifolium, Rhus spp., Carpinus coreana, Mallotus japonicus. Ficus erecta, Pittosporum tobira, Eurya japonica. Elaeagnus macrophylla. 3. The area of plant community in Amnam Park was 84.29% in Pinus thunbergii community. 6.7% in Quercus dentata-Celtis sinensis community, 1.5% in Pittosporum tobira-Eurya japonica community, 1.4% in Quercus aliena-Prunus sargentii community, 1.29% in Pinus thunbergii-Pittosporum tobira community, 1.27% in Pinus thunbergii-Eurya japonica community.

        경골 결절의 견열 골절에 합병된 구획 증후군

        Avulsion of the tibial tubercle is an uncommon physeal injury. Complications from this fracture have rarely been reported and seldom affect the long-term outcome. Three adolescent boys who sustained avulsion fracture of the tibial tubercle, were complicated by compartment syndrome and treated at Inje University Sang-Gye Paik Hospital from September 1989 to February 1995. Injury to the soft tissue surrounding the tibial tubercle avulsion may be more extensive than is usually appreciated. The anatomy of the proximal tibia and the tibial tubercle with nearby branches of the anterior tibial recurrent artery suggest a predisposing factor for the development of compartment syndrome. So, compartment syndrome should be added to the list of possible complications of this type of fracture.

      • Speciation and capture performance of mercury by a hybrid filter in a coal-fired power plant

        <P><B>Abstract</B></P> <P>Mercury speciation and removal were investigated for a newly developed hybrid filter (HF) originally designed to remove fine particulates and retrofitted to the exhaust cleaning system in a coal-fired power plant. The test plant used a blend of anthracite and semi-anthracite coals for combustion. At the boiler outlet, Hg<SUP>0</SUP> was the dominant mercury species in the flue gas at 89% of total Hg. After passing through the electrostatic precipitator (ESP) and HF, Hg<SUP>2+</SUP> became the dominant species owing to oxidation of Hg at the lower temperature and interaction with other air pollutants in the flue gas. The Hg<SUP>2+</SUP> subsequently accumulated as Hg<SUB>p</SUB> by bonding with fly ash. Due to higher oxidation of Hg in the HF than in the ESP, more Hg<SUP>2+</SUP> could be removed by the flue gas desulfurization (FGD) system through dissolution of Hg<SUP>2+</SUP> when an HF was included. By installing hybrid filters alongside the existing devices, removal efficiencies of 86.7% and 99.3% for Hg and fine particulates, respectively, could be achieved, as opposed to 62.4% and 95.2% without the installation.         Puerarin ameliorates hepatic steatosis by activating the PPARα and AMPK signaling pathways in hepatocytes

        <P>Non-alcoholic fatty liver disease (NAFLD) is characterized by the hepatic manifestation of metabolic syndrome and is the leading cause of chronic liver disease. Steatohepatitis plays a critical role in the process resulting in liver fibrosis and cirrhosis. Puerarin is a herbal product widely used in Asia, and is believed to have therapeutic benefits for alleviating the symptoms of steatohepatitis. The present study was designed to investigate the effects and mechanisms of action of puerarin in reducing lipid accumulation in oleic acid (OA)-treated HepG2 cells. Hepatocytes were treated with OA with or without puerarin to observe lipid accumulation by Oil Red O staining. We also examined hepatic lipid contents (e.g., triacylglycerol and cholesterol) following treatment with puerarin. Western blot analysis and reverse trabscription-polymerase chain reaction (RT-PCR) were used to measure sterol regulatory element binding protein (SREBP)-1, fatty acid synthase (FAS), peroxisome proliferator-activated receptor alpha (PPAR alpha) and adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK) protein and mRNA expression, respectively. Our results revealed that puerarin suppressed OA-induced lipid accumulation, and reduced the triacylglycerol and cholesterol levels. Furthermore, puerarin decreased the expression levels of lipogenic enzymes, such as FAS and SREBPs, and increased the expression levels of PPARa, which are critical regulators of hepatic lipid metabolism through the AMPK signaling pathway. These results indicate that puerarin has the same ability to activate AMPK, and reduce SREBP-1 and FAS expression, thus inhibiting hepatic lipogenesis and increasing hepatic antioxidant activity. We found that puerarin exerted a regulatory effect on lipid accumulation by decreasing lipogenesis in hepatocytes. Therefore, puerarin extract may have therapeutic benefits in the treatment of fatty liver and lipid-related metabolic disorders.</P>

        Design of a Pep-1 peptide-modified liposomal nanocarrier system for intracellular drug delivery: Conformational characterization and cellular uptake evaluation

        <P>In order to facilitate the intracellular delivery of macromolecules, Pep-1 peptide-modified liposomal (Pep1-Lipo) nanocarriers were designed and examined for their <I>in vitro</I> cell translocation capability. Pep-1 peptides were coupled via thiol-maleimide linkage to small unilamellar vesicles composed of phosphatidylcholine, Tween 80, and <I>N</I>-[4-(p-maleimidophenyl)butyryl]-phosphatidylethanolamine (MPB-PE). The amount of Pep-1 peptide conjugated to the vesicle was effectively controlled by the amounts of maleimide groups on the vesicular surface, ranging from 70 to 700 molecules per vesicle. Systems were evaluated for cell uptake capacity by monitoring entrapped fluorescence-labeled bevacizumab, a model protein for poorly permeable macromolecule, using confocal microscopy. The novel carriers rapidly bound to the cell membrane and migrated into the cells within 1 ??h, exhibiting better translocation of macromolecules compared to that of conventional liposomes. Cellular uptake of Pep1-Lipo was proportional to the amount of Pep-1 peptide on the liposomal surface. In conclusion, we found that the Pep1-Lipo formulation was a promising nanocarrier system for intracellular delivery of macromolecules.</P>
