        노인의 운동실천 제약요인 및 운동행동변화단계별 차이

        이 연구는 노인의 운동실천 제약요인을 탐색하고 범이론적 모형(TTM)에 기초하여 운동행동변화단계에 따른 제약요인의 차이를 알아보고자 하였다. 이를 위해 운동을 실천하고 있거나 실천하지 않는 60제 이 상 103명의 노인을 대상으로 개방형 설문을 실시하였으며, 이 결과를 토대로 개발된 운동실천 제약요인 척도를 활용하여 309명의 자료를 수집하고 제약요인의 구조 및 운동행동변화단계에 따른 차이를 검증하였다. 연구결과, 노인들의 운동실천을 제약하는 6개요인(부상우려, 운동지식 부족, 건강상실과 질병, 운동동기 부족, 시간기회 부족, 경제여건)이 발견되었으며, 운동행동변화단계에 따른 제약요인의 차이를 분석한 결과에서는 성과 연령 및 운동행동변화단계에 따른 상호작용효과 및 주 효과가 동시에 나타나 복잡한 양상을 보여주었다. 성과 연령을 함께 고려할 때, 운동행동변화단계를 가장 잘 판별하는 제약요인은 건강상실과 질병요인이었다. 이러한 결과는 노인의 운동실천 제약요인의 타당성과 중재전략 개발 의 방향성 및 후속연구의 과제와 관련하여 논의하였다. This study, based on hierarchial model of leisure constraints and trans-theoretical model(TTM), aimed to investigate the conceptual structure of exercise constraints in the aged and the difference according to the stages of change of exercise. The data were collected via a open-ended questionnaire(103 age of over 60) and self-administrated residents questionnaire survey(309 aged people) in incheon city in Korea. content analysis, descriptive and inferential analyses including factor analysis, three-way MANOVA through the stepdown F-test were used to analyze the data. The results of the study showed that the constraint dimension of exercise in the aged were 6 multi-dimensional factors, which were lack of knowledge and information to exercise, bed health and illness, motivation loss, lack of time and chance, low economic condition. Furthermore, as the results of analysis on the differences according to the stages of change of exercise, 6 constraints of exercise offered a significant contribution towards the prediction of the stages of change. The bed health and illness dimension was the best predictor for distinguishing among the stages of change considered the gender x age x stages of change effects. These results were discussed relate to cross validity of constraints and strategy of intervention for regular exercise participation.

        Functional Characterization of Mammary Gland of Holstein Cows under Humid Tropical Summer Climates

        Physiological parameters were measured on six primiparous, non-pregnant Holstein cows prior to peak lactation over a 3-month summer season in southwestern Taiwan. The objectives were to characterize heat stress-induced change in functionality of mammary gland under natural climates of tropical summer and to establish physiological indices applicable to this environment in referring to this change. Environmental and physiological readings, milk and blood samples were taken at 15:00 h biweekly for totally five time points during the study. Climate readings showed that the afternoon humidex value reached the highest (53.5) around mid summer. Rectal temperature of cows taken simultaneously varied between $38.26^{\circ}C$ and $40.02^{\circ}C$ in parallel to humidex. Milk production declined drastically from 29.2 to 22.2 kg/d the first month entering summer but leveled up at end of the summer season suggesting effects exerted by heat stress rather than stages of lactation. Lactose content decreased linearly (p<0.05) with times in summer, from 4.69 to 4.38%. On the other hand, activity of N-acetylglucosaminidase (NAGase) in milk increased linearly to over two folds (p<0.05) during the same intervals. Elevations of fractional constituent of BSA in whey protein and serum cortisol level were also noticed in the course. Measurement of arteriovenous concentration (A-V) difference across the mammary gland demonstrated net uptake of glucose and net release of urea throughout the study period. The amount of urea released from mammary gland increased (p<0.05) progressively from 1.54 to 7.76 mg/dl during summer. It is concluded that gradual regression of mammary gland occurred along the humid tropical summer season. This regression is likely initiated through elevation of body temperature, which is irreversible above certain point. The increased release of urea from mammary gland during heat stress suggests its potential role as an early indicator of suboptimal mammary function.

        Britanin Suppresses IgE/Ag-Induced Mast Cell Activation by Inhibiting the Syk Pathway

        The aim of this study was to determine whether britanin, isolated from the flowers of Inula japonica (Inulae Flos), modulates the generation of allergic inflammatory mediators in activated mast cells. To understand the biological activity of britanin, the authors investigated its effects on the generation of prostaglandin D2 (PGD2), leukotriene C4 (LTC4), and degranulation in IgE/Ag-induced bone marrow-derived mast cells (BMMCs). Britanin dose dependently inhibited degranulation and the generations of PGD2 and LTC4 in BMMCs. Biochemical analyses of IgE/Ag-mediated signaling pathways demonstrated that britanin suppressed the phosphorylation of Syk kinase and multiple downstream signaling processes, including phospholipase Cγ1 (PLCγ1)-mediated calcium influx, the activation of mitogen-activated protein kinases (MAPKs; extracellular signal-regulated kinase 1/2, c Jun NH2-terminal kinase and p38), and the nuclear factor-κB (NF-κB) pathway. Taken together, the findings of this study suggest britanin suppresses degranulation and eicosanoid generation by inhibiting the Syk-dependent pathway and britanin might be useful for the treatment of allergic inflammatory diseases.

        Dextromethorphan Protect the Valproic Acid Induced Downregulation of Neutrophils in Patients with Bipolar Disorder

        Objective: Valproic acid (VPA) is an anticonvulsant and commonly long term used as a mood stabilizer for patients with mood disorders. However its chronic effects on the hematological changes were noticed and need to be further evaluated. In this study, we evaluated, in Taiwanese Han Chinese patients with bipolar disorders (BD), the chronic effects of VPA or VPA plus dextromethorphan (DM) on the hematological molecules (white blood cell [WBCs], red blood cells [RBCs], hemoglobin, hematocrit, and platelets). Methods: In a 12-week, randomized, double-blind study, we randomly assigned BD patients to one of three groups: VPA plus either placebo (VPA+P, n = 57) or DM (30 mg/day, VPA+DM30, n = 56) or 60 mg/day (VPA+DM60, n = 53). The Young Mania Rating Scale and Hamilton Depression Rating Scale were used to evaluate symptom severity, and the hematological molecules were checked. Results: Paired t test showed that the WBC, neutrophils, platelets and RBCs were significantly lowered after 12 weeks of VPA+P or VPA+DM30 treatment. VPA+DM60 represented the protective effects in the WBCs, neutrophils, and RBCs but not in the platelets. We further calculated the changes of each hematological molecules after 12 weeks treatment. We found that combination use of DM60 significantly improved the decline in neutrophils induced by the long-term VPA treatment. Conclusion: Hematological molecule levels were lower after long-term treatment with VPA. VPA+DM60, which yielded the protective effect in hematological change, especially in the neutrophil counts. Thus, DM might be adjunct therapy for maintaining hematological molecules in VPA treatment.

        Harm Avoidance is Correlated with the Reward System in Adult Patients with Attention Deficit Hyperactivity Disorder: A Functional Magnetic Resonance Imaging Study

        Objective: Hypoactivity in the reward system among patients with attention deficit hyperactivity disorder (ADHD) is a well-known phenomenon. Whether the activity in the reward pathway is related to harm avoidance, such as in sensitivity to punishment, is unclear. Evidence regarding the potential difference between ADHD patients and controls in terms of this association is scarce. Methods: Event-related functional magnetic resonance imaging was conducted on subjects performing the Iowa gambling test. Fourteen adults with ADHD and 14 controls were enrolled in the study. Results: Harm avoidance was found to be positively correlated with the activities of the bilateral orbitofrontal cortex and right insula in individuals with ADHD. A group difference was also confirmed. Conclusion: Understanding the roles of harm avoidance and brain activation during risk tasks is important.

        The Evaluation of Otilonium Bromide Treatment in Asian Patients With Irritable Bowel Syndrome

        Background/Aims Antispasmodics including otilonium bromide (OB) are recommended to treat irritable bowel syndrome (IBS). However, reports about OB experience in Asia is sparse. The purpose of present study was to provide the efficacy of OB in treating Asian IBS patients. Methods Overall, 117 IBS patients meeting Rome II criteria were enrolled in an 8-week, double-blind, active-controlled and single center trial. Randomized participants received either OB 40 mg or mebeverine 100 mg 3 doses daily. The primary endpoints were to evaluate the net changes of abdominal pain/discomfort frequency score (APDFS) and safety profile, while the secondary endpoints were to assess the changes in abdominal pain/discomfort intensity, flatulence, abdominal bloating, satisfied stool frequency etc. Results Finally, 49 OB and 52 mebeverine subjects were eligible for efficacy analysis. Compared to baselines in per protocol populations, the reduced APDFSs in OB and mebeverine were 0.55 ± 1.20 (P = 0.011) and 0.37 ± 1.11 (P = 0.042), respectively, to show similarly reduced scores. The most reported side effects included dry mouth, nausea and dizziness. Besides, the improved APDFSs at 4th week visit, final alleviations in abdominal pain intensity, flatulence, abdominal bloating and satisfied stool frequency with global assessments filled by both patients and investigators were significantly achieved by both treatments, and OB was not inferior to mebeverine in treating these parameters. Conclusions In Orientals, OB is as effective as mebeverine for alleviating IBS symptoms in terms of abdominal pain, flatulence, abdominal bloating etc. However, obvious side effects are also observed. A large-scaled trial and post-marketing surveillance are recommended to confirm its efficacy and safety. (J Neurogastroenterol Motil 2011;17:402-410)

      • Expression and Clinical Significance of Myeloid Derived Suppressor Cells in Chronic Hepatitis B Patients

        We here document discovery of expression profile of myeloid derived suppressor cells (MDSCs) in chronic hepatitis B (CHB) patients and changes in the course of disease. The study population was composed of 75 outpatient HBV cases and 15 healthy control cases. Peripheral blood samples were collected for separation of mononuclear cells. Levels of MDSCs labeled with Lin-DR-CD11b+CD33+ obtained from peripheral blood mononuclear cells (PBMC), were revealed to have significant differences between the CHB and other groups. They were 0.414% for health control cases and 0.226% for CHB cases (Z=-2.356, p=0.0189). It also observed that the group of HBeAg positive cases had significant difference in MDSCs/PBMC median ($X^2=11.877$, p=0.003), compared with group of HBeAg negative cases and the healthy control group. It suggested considerable MDSCs might be involved in HBeAg immune tolerance. In addition, negative correlations between MDSCs/PBMC and parameters of ALT, AST and TBil, while positive correlation between MDSCs/PBMC and ALB parameter were found. Multiple comparisons between the four phases and health control phase again, there was a statistically sifnificant difference ($X^2=17.198$, p=0.002). Taken together, these findings may provide a new immunotherapy strategy for reduced the expression levels of MDSCs in CHB patients, through induction of an autoimmune response to virus removal.

      • Trajectory for Robotic Manipulators with Torque Minimization by Using Hermit Interpolation Method

        In this paper, we present a new method to get the trajectory of robotic manipulators with torque minimized. For the robotic manipulators for our research, just the position and velocity of the start and the final point are given. With given conditions and arbitrary chosen mutative points, we get the Hermite interpolation method used. Beside the start point and the final point, we will search for middle arbitrary chosen mutative points. Then the Hermite interpolation method is used to make sure the positions and velocities of the links satisfy the given conditions. With the change of position and velocity on the arbitrary chosen mutative points, with which taken into performance Index, the trajectory for torque minimization can be got.
