However, dermoscopic findings are not fully elucidated in the course of β-blocker treatment for IH. Objectives: To investigate changes of dermoscopic findings during the course of β-blocker treatment for IH. Methods: Superficial or mixed-type IH patients treated with topical timolol solution or oral propranolol were enrolled in Pusan National University Hospitals (Busan and Yangsan) from November 2011 to January 2016. Clinical photograph and dermoscopic findings were recorded and assessed at the baseline, 1, 2, 3 and 6 months. Results: Total 115 patients (male: 38 and female: 77 / superficial type: 48 and mixed type: 67 / timolol: 77 and propranolol: 38) were included. Diffuse light erythema tended to be diminished but milky-red area increased gradually during the course of treatment. The patients treated with oral propranolol showed more prominent changes on each dermoscopic findings than those treated with topical timolol (p=0.023 and p=0.018, respectively). But, other findings like red-globular vessels, red-dot vessels, red-circulated vessels, red-linear vessels did not show significant changes. Conclusion: Dissolution of diffuse light erythema and increase of milky-red area were significant dermoscopic findings in accordance with clinical improvement after β -blocker therapy for IH.

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

        보조호르몬요법을 받는 여성 유방암 환자의 모발 변화와 치료 실태에 대한 기초 연구

        Background: Hormone therapy, which includes tamoxifen and aromatase inhibitors, is the most common adjuvant therapy used for breast cancer. However, only a few studies have reported endocrine therapy induced alopecia. Objective: We investigated the effects of long-term adjuvant hormone therapy on hair in patients with breast cancer, in addition to patients’ concerns and current treatment for hair loss. Methods: Patients completed a questionnaire that included information on self-perceived hair changes after each adjuvant therapy session, distress, and current treatment for hair loss. Using a folliscope, we measured hair density and thickness in each patient and in healthy controls. Results: The study included 93 patients with breast cancer (mean age 51.9±9.8 years). The density and hair thickness were 106.36±21.85 hairs/cm2 and 0.07±0.01 mm in the patient group and 147.86±30.67 hairs/cm2 and 0.07±0.01 mm in the control group (n=98, mean age 52.10±8.40 years), respectively. The mean hair density was significantly lower in the patient group than in the control group; however, no statistically significant intergroup difference was observed in hair thickness. Among 76 patients who perceived hair changes after adjuvant therapy, 71.1% (n=54) were distressed with regard to hair changes. However, only 7.8% of the patients, including two who were treated by dermatologists, currently received treatment for hair changes. Conclusion: Dermatologists should be familiar with hair changes in patients with breast cancer and provide appropriate education to encourage patients to consult dermatologists for hair loss and thinning after breast cancer treatment.         Expression of cellular retinoic acid-binding protein-I (CRABP-I) in the cerebrospinal fluid of adult onset moyamoya disease and its association with clinical presentation and postoperative haemodynamic change

        <P><B>Objective</B></P><P>The elevation of cellular retinoic acid-binding protein-I (CRABP-I) has been suggested as a candidate in the pathogenesis of paediatric moyamoya disease (MMD). However, few studies have addressed CRABP-I in adult onset MMD. The aim of this study was to examine the expression of CRABP-I in the cerebrospinal fluid (CSF) of adult onset MMD, and to evaluate its association with clinical presentation and postoperative haemodynamic change.</P><P><B>Methods</B></P><P>This study examined the CSF from 103 patients: bilateral MMD, n=58 (56.3%); unilateral MMD, n=19 (18.4%); atherosclerotic cerebrovascular disease (ACVD), n=21 (20.4%); and control group, n=5 (4.9%). The intensity of CRABP-I was confirmed by western blotting and expressed as the median (25th–75th percentile). The differences in CRABP-I expression according to disease entity (unilateral MMD vs bilateral MMD vs ACVD), initial presenting symptoms (haemorrhage vs ischaemia) and postoperative haemodynamic change (vascular reserve in single photon emission CT and basal collateral vessels in digital subtraction angiography) were analysed.</P><P><B>Results</B></P><P>CRABP-I intensities in bilateral MMD (1.45(0.86–2.52)) were significantly higher than in unilateral MMD (0.91(0.78–1.20)) (p=0.044) or ACVD (0.85(0.66–1.11)) (p=0.004). No significant differences were noted based on the initial presenting symptoms (p=0.687). CRABP-I was not associated with improvement in vascular reserve (p=0.327), but with decrease in basal collateral vessels (p=0.023) postoperatively.</P><P><B>Conclusions</B></P><P>Higher CRABP-I in the CSF can be associated with typical bilateral MMD pathogenesis in adults. Additionally, postoperative basal collateral change may be related to the degree of CRABP-I expression.</P>

      • Factors associated with inflammatory change of epidermal cyst

        Background: Epidermal cysts (EC) are the most common tumors found on the skin and accompanying inflammation is also frequent. However, the predictors for inflammatory change have not been investigated yet. Objectives: This study was performed to investigate the difference between inflamed and uninflamed epidermal cysts, and to assess associated factors for inflammatory change. Methods: We analyzed the biopsy-proven 1723 epidermal cysts (515 inflamed and 1208 uninflamed) at two tertiary hospitals from 2011 to 2021. Results: The proportion of inflamed epidermal cysts (IEC) and uninflamed epidermal cyst (UIEC) were 29.9 % and 70.1 %, respectively. Inflammatory change was less frequently observed on the face (25.5 %) than the other sites (32.1 %) (p=0.0046). The anatomic location with the highest rate of inflammatory change was back (40.8 %, p=0.0004). Mean duration of inflammatory change from UIEC to IEC on the face was shorter than the other sites (12.5 months vs 17.9 months, respectively, p= 0.0473). On ultrasonography, long axis of IEC (20.7mm) was generally longer than that of UIEC (15.7mm) (p=0.0218). Moreover, Inflammatory change was more frequent on the EC with punctum compared to those without punctum (35.3 % vs 30.3 %, P=0.0471). Conclusion: Based on the location, size and presence of punctum, decision on the optimal treatment strategies can be established when encountering patients with EC.

      • Biomechanical analysis of lumbar decompression surgery in relation to degenerative changes in the lumbar spine – Validated finite element analysis

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>There are no studies about the biomechanical analysis of lumbar decompression surgery in relation to degenerative changes of the lumbar spine. Therefore, the purpose of this study was to compare, by using finite element (FE) analysis, the biomechanical changes of the lumbar spine in terms of annulus stress and nucleus pressure after two different kinds of lumbar decompression surgery in relation to disc degenerative changes.</P> <P><B>Methods</B></P> <P>The validated intact and degenerated FE models (L2-5) were used in this study. In these two models, two different decompression surgical scenarios at L3-4, including conventional laminectomy (ConLa) and the spinous process osteotomy (SpinO), were simulated. Therefore, a total of six models were simulated. Under preloading, 7.5 Nm moments of flexion, extension, lateral bending, and torsion were imposed. In each model, the maximal von Mises stress on the annulus fibrosus and nucleus pressure at the index segment (L3-4) and adjacent segments (L2-3 and L4-5) were analyzed.</P> <P><B>Results</B></P> <P>The ConLa model and disc degeneration model demonstrated a larger annulus stress at the decompression level (L3-4) under all four moments than were seen in the SpinO model and healthy disc model, respectively. Therefore, the ConLa model with moderate disc degeneration showed the highest annulus stress at the decompression level (L3-4). However, the percent change of annulus stress at L3-4 from the intact model to the matched decompression model was less in the moderate disc degeneration model than in the healthy disc model.</P> <P><B>Conclusions</B></P> <P>Although the ConLa model with moderate disc degeneration showed the highest annulus stress, the degenerative models would be less influenced by the decompression technique.</P>

      • Treatment with spironolactone in Becker`s nevus associated with ipsilateral breast hypoplasia

        Becker nevus syndrome is the association of becker``s nevus with breast hypoplasia and other ipsilateral bone or muscle changes. It is considered to be a hormone-dependent disorder caused by a disturbance in the activity of the androgen receptor. The receptor appears to be increased in Becker``s nevus, which may influence to the development of associated lesions. Herein, we report a case of Becker nevus syndrome improved after the treatment with spironolactone. A 13-year-old girl presented with well-defined brownish patch over left breast for 8 years. In addition, she noticed hypoplasia of her ipsilateral breast (12 x 10 cm) since 10 years age. Histopathologic examinations revealed acanthosis, elongation of rete ridges in epidermis and hyperpigmentation of the basal epidermal layer. The diagnosis has been confirmed to Becker``s nevus associated with ipsilateral breast hypoplasia. The Laboratory test (CBC, electrolytes, and LH/FSH) done and showed normal range. Her hypoplastic breast was treated with spironolactone with 50mg single dosage twice daily. She experienced mild menstrual irregularity and it was improved within 1 month. The left breast had enlarged abruptly to 13 x 10.5 cm after the treatment of 19 weeks.

      • Kikuchi-Fujimoto Disease Coexistent with Papillary Thyroid Carcinoma in a Single Lymph Node

        Cervical lymphadenopathy can be developed from various causes such as viral infection, bacterial infection, Kikuchi-Fujimoto disease, tuberculosis, malignancy, and reactive changes. In patients who have malignancy, metastatic lymphadenopathy is possible but it is rare that other concomitant diseases are in the same lymph node. We experienced a case of coexistence of Kikuchi necrotizing lymphadenitis and papillary thyroid carcinoma in a single cervical lymph node. A 38-year-old man who was previously diagnosed with papillary thyroid cancer with cervical lymph nodes metastasis presented with cervical lymphadenopathy. The lymph node biopsy showed tuberculous lymphadenitis. After finishing anti-tuberculosis medication, recurrent lymphadenopathy had developed and a surgical biopsy was performed. At that time, the diagnosis was Kikuchi necrotizing lymphadenitis combined with metastatic papillary carcinoma in a single lymph node.
