Electroceutical and Bioelectric Therapy: Its Advantages and Limitations

Given the long history, the field of electroceutical and bioelectric therapy has grown impressively, recognized as the main modality of mental health treatments along with psychotherapy and pharmacotherapy. Electroceutical and bioelectric therapy comprises electroconvulsive therapy (ECT), vagus nerve stimulation (VNS), repetitive transcranial magnetic stimulation (rTMS), deep brain stimulation (DBS), transcranial electrical stimulation (tES), and other brain stimulation techniques. Much empirical research has been published regarding the application guidelines, mechanism of action, and efficacy of respective brain stimulation techniques, but no comparative study that delineates the advantages and limitations of each therapy exists for a comprehensive understanding of each technique. This review provides a comparison of existing electroceutical and bioelectric techniques, primarily focusing on the therapeutic advantages and limitations of each therapy in the current electroceutical and bioelectric field.
P124 : The efficacy and safety of cryo fat reduction in the treatment of pseudogynecomastia

Background: Options for the treatment of pseudogynecomastia, a condition of enlarged male breast primarily due to the accumulation of adipose tissue, have been limited, including weight reduction and surgical liposuction. Cryo fat reduction provides noninvasive, selective, localized subcutaneous adipocyte destruction without epidermal or dermal injury by inducing adipocyte apoptosis. Objectives: This study was designed to evaluate the efficacy of cryo fat reduction in the treatment of pseudogynecomastia. Methods: In this prospective, 28-week trial, 12 male patients with psudogynecomastia were treated with cryo fat reduction twice bimonthly. Efficacy was determined by breast circumference measurement, ultrasonographic measurement of subcutaneous layer thickness, physician’s global photographic assessment, and patient’s satisfaction at baseline at week 4, week 8, week 16 and week 28. Safety was evaluated by questionnaire at each visit. Results: For the 11 subjects completing the trial, breast circumference and thickness of fat layer determined by ultasonography significantly improved at week 8, andgradually reduced until week 28. Physician’s photographic assessment and patient’s satisfaction showed significant improvement at each visit. No adverse event was observed except transient pain immediately after the treatment and bruise at the treated site. Conclusion: Cryo fat reduction is a safe and effective therapeutic option of the treatment of pseudogynecomastia.
Vagus Nerves Stimulation: Clinical Implication and Practical Issue as a Neuropsychiatric Treatment

Vagus nerve stimulation (VNS) has been approved as an adjunctive treatment for epilepsy and depression. As the progress of VNS treatment for these neuropsychiatric disorders continues, its applications have expanded to a wide range of conditions, including inflammatory diseases to cognitive dysfunctions. The branches of the vagal nerves directly or indirectly innervate the anatomical structures implicated in these neuropsychiatric conditions, which has led to promising results regarding the effectiveness of VNS. Previous studies investigating the effectiveness of VNS have mostly utilized invasive forms of stimulation. However, current preclinical and clinical research indicates that non-invasive forms of VNS, such as transcutaneous vagus nerve stimulation, hold the promise for treating various neuropsychiatric conditions. This review aims to delve into relevant clinical studies of VNS in various illness states, different methods of VNS, and the potential mechanisms underlying the therapeutic effects in these neuropsychiatric conditions.

Objective This study aimed to explore the relationship between childhood physical abuse and suicidal ideation considering the effects of genetic and environmental factors in patients with post-traumatic stress disorder (PTSD) by focusing on brain-derived neurotrophic factor (BDNF) polymorphism and social support, respectively.Methods One-hundred fourteen patients with PTSD and 94 healthy controls (HCs) were genotyped with respect to BDNF Val66Met polymorphism. All participants underwent psychological assessments. The hierarchical regression analysis and the simple slope analysis were conducted.Results As for patients with PTSD, the moderation effect of BDNF polymorphism was significant but not for social support. Specifically, the BDNF Val/Val genotype worked as a risk factor and strengthens the relationship between childhood physical abuse and suicidal ideation. As for the HCs, the significant moderation effect was found only in social support, but not for BDNF polymorphism. The relationship between childhood physical abuse and suicidal ideation was weakened for the HCs with high social support.Conclusion This study demonstrated a significant BDNF genetic vulnerability for suicide in patients with PTSD who experienced childhood physical abuse. Our results suggested that social support provided a mitigating effect on the relationship between childhood physical abuse and suicidal ideation only in the HCs.

Jet-M (Tav-Tech Ltd., Israel) is an instrument for skin resurfacing. When it sprays microdroplets of solution or shoots air on the skin, exfoliation and stretching of superficial layers can occur. So, it will increase percutaneous absorption of vitamins and other cosmeticagents. A cosmetic preparation containing β-glucan, rhodiolar extract, oligo-hyaluronic acid, copper-GHK, and tranexamic acid was used with jet-M in one patient. Anesthesia was not done and there was no pain during the treatment. A male aged 59 years was treated once a week for 12 weeks. In the clinical photos, wrinkles around the treated eye were greatly decreased. Skin biopsies were taken from treated and untreated areas. H&E and Masson’s trichrome staining showed collagen increase in the upper dermis. On the other hand, collagen IV was slightly increased. Fibrillin-1 and procollagen type 1 weregreatly increased and tropoelastin was also increased. There was no adverse effect during and after treatment.

<P><B>Abstract</B></P> <P>Intensive use of the thumbs for text entry on smartphones may contribute to discomfort, pain, or musculoskeletal disorders. This study investigated the effect of twenty-five button positions (5 rows × 5 columns) on a soft keyboard for two-thumb entry. Two experiments measured muscle activity, touch time, and discomfort as a function of the button positions. In Phase I, the muscle activities of two intrinsic (abductor pollicis brevis and first dorsal interossei) and two extrinsic (abductor pollicis longus and extensor digitorum communis) muscles associated with thumb motions were observed for ten college students (age: 24.2). In Phase II, touch time and discomfort were measured for 40 college students (age: 23.6). The results demonstrated that the %MVCs of the intrinsic muscles significantly increased when the thumbs flexed and abducted. Also, the button positions near the rest positions of the thumbs resulted in significantly shorter touch times (0.66 s) and lower discomfort ratings (0.70 pt) than their peripheral buttons (0.76 s; 2.29 pt).</P> <P><B>Highlights</B></P> <P> <UL> <LI> This study evaluated the effects of various button positions on a soft keyboard for two-thumb entry. </LI> <LI> %MVC of the intrinsic muscles varied significantly by the button position. </LI> <LI> Touch time and discomfort rating increased as a target button is away from the initial position of the thumb. </LI> <LI> The maps of %MVC, touch time, and discomfort were provided for various button positions. </LI> </UL> </P>
알츠가드: 경도인지장애 치료를 위한 대화형 디지털 테라퓨틱스

본 연구는 사회적 문제로 대두되는 있는 치매를 예방하기 위한 접근방법으로 알츠가드(Alzguard)를 제안하고자 한다. 알츠가드는 치매의 전단계인 경도인지장애를 치료하여 치매를 예방할 수 있도록 하는 카카오톡 플랫폼 기반 대화형 디지털 테라퓨틱스이다. 디지털 테라퓨틱스는 디지털 매체를 통해 질병 관리가 가능한 치료제를 말하는데, 비대면 관리와, 개인화된 치료가 가능한 접근방법이다. 이를 통해 경도인지장애 치료를 위한 요소인 훈련 몰입, 다각적 접근, 개인화된 훈련을 효과적으로 전달할 수 있다. 알츠가드는 이러한 요소를 중점으로 에이전트 ‘새미’와의 대화, 5 개 영역의 인지강화 훈련, 건강정보와 소셜 기능을 제공하며 이를 통해 경도인지장애를 치료하고 더 나아가 치매를 예방하고자 한다.
