        When Thoughts Are in a Race: Area 10 and Bipolar Disorders

        Racing thought, when patients incessantly shift from one word or sentence to another while pending previous ones unfinished, is a symptom of (hypo)mania in bipolar disorders received less attention hitherto. Here, based on few evidence, we aim to unfold our hypothetical viewpoint that the frontopolar cortex that is believed to play a part in multitasking and management of competing goals might be dysfunctional in bipolar patients and may contribute in induction of flight of ideas. We then address new avenues for future research and try to encourage researchers to design more comprehensive studies to either accept or decline this proposed conjecture.

        Microtensile bond strength of resin cement primer containing nanoparticles of silver (NAg) and amorphous calcium phosphate (NACP) to human dentin

        PURPOSE. The purpose of the current study was to evaluate the effect of incorporating nanoparticles of silver (NAg) and amorphous calcium phosphate (NACP) into a self-etching primer of a resin cement on the microtensile bond strength of dentin, regarding the proven antibacterial feature of NAg and remineralizing effect of NACP. MATERIALS AND METHODS. Flat, mid-coronal dentin from 20 intact extracted human third molars were prepared for cementation using Panavia F2.0 cement. The teeth were randomly divided into the four test groups (n=5) according to the experimental cement primer composition: cement primer without change (control group), primer with 1% (wt) of NACP, primer with 1% (wt) of physical mixture of NACP+Nag, and primer with 1% (wt) of chemical mixture of NACP+Nag. The resin cement was used according to the manufacturer’s instructions. After storage in distilled water at 37℃ for 24 h, the bonded samples were sectioned longitudinally to produce 1.0 × 1.0 mm beams for micro-tensile bond strength testing in a universal testing machine. Failure modes at the dentin-resin interface were observed using a stereomicroscope. The data were analyzed by one-way ANOVA and Tukey’s post-hoc tests and the level of significance was set at 0.05. RESULTS. The lowest mean microtensile bond strength was obtained for the NACP group. Tukey’s test showed that the bond strength of the control group was significantly higher than those of the other experimental groups, except for group 4 (chemical mixture of NACP and NAg; P=.67). CONCLUSION. Novel chemical incorporation of NAg-NACP into the self-etching primer of resin cement does not compromise the dentin bond strength.

        A Study on the Antitumoral and Differentiation Effects of Peganum Harmala Derivatives in Combination with ATRA on Leukaemic Cells

        Plant derived agents may exert a new approach to the treatment of leukaemia. The present study was an evaluation of proliferation, cytotoxicity and differentiation of harmine and harmaline on HL60 cells, alone or in combination with ATRA and G-CSF. Counting of cells, viability, MTT assay, morphology, NBT reduction and flow cytometry analysis were performed using CD11b and CD 14 monoclonal antibodies. The data showed that harmine and harmaline reduced proliferation in dose and time dependent manner. Optimal antiproliferative concentration of these agents was chosen. However, both agents in higher doses were cytotoxic. Combination of ATRA, G-CSF and each agent alone, particularly harmaline in optimal dose, resulted in partially additive decrease in cell proliferation. Cells treated with both harmaline and ATRA demonstrated some morphological changes and NBT positivity, but the extent of changes observed following treatment with harmaline was less than ATRA. Flow cytometric analysis showed that ATRA induced a neutrophilic differentiation, while harmaline led to a predominantly monocytic differentiation. Combination of harmine and harmaline with ATRA and G-CSF did ret change the extent of differentiation, and the cells differentiated into the neutrophilic lineage. This shows that the direction of differentiation is dominantly determined by ATRA. These preliminary data implies a new approach in treatment of leukemia.

      • Incidence and Trends of Malignant and Benign Pancreatic Lesions in Yazd, Iran between 2001 and 2011

        Background: Despite recent valuable steps in initiating a cancer registry in Iran, data depicting prevalence, incidence, and clinical picture of pancreatic tumors in the country are exceedinglyly sparse. With the aim of filling this knowledge gap, we reviewed cases in the pathology archive of Shahid Sadoughi hospital (Yazd, Iran), between 2001 and 2011. Materials and Methods: Medical records of 177 patients are reported in the present study. In cases for which paraffin-embedded blocks were available, the specimens were evaluated by two independent pathologists blinded to the primary diagnosis. We extrapolated the frequency of malignant lesions in our study to the population of Yazd province, derived from national census data, to generate cancer incidence rates. Results: Final diagnosis of malignancy was made in 117 cases (66.1%), and the remainder (60 lesions, 33.9%) were classified as benign. Adenecarcinoma and neuroendocrine tumors were the two most common histological types of malignancy identified in 88 (75.2%) and 11 (9.4%) specimens, respectively. Crude annual incidence of pancreatic cancer was 0.55 per 100,000 person in 2001 and increased to 1.68 in 2011. Age standardized incidence rates in 2001 and 2011 were 0.75 and 2.68, respectively. A significant increasing trend in cancer incidence was observed during the 11 years of the study period (r=+0.856, p=0.009). Sex-stratified analysis, confirmed the observed trend in men (r=+0.728, p=0.034), but not women (r=+0.635, p=0.083). Conclusions: Over the past decade, incidence of pancreas malignancies has risen steadily in Yazd, Iran. Nevertheless, these figures are still substantially lower than those prevalent in developed nations.

        이케토 구리(II) 착물의 합성 및 송아지 Thymus DNA(CTDNA)와의 상호작용

        이케토형 리간드는 thiophene-2-aldehyde와 acetylacetone의 Knoevenagel 축합반응을 통해 합성하였으며, 이를 이용하여 Cu(II), Ni(II) 및Co(II) 염화물의 착물을 합성하였다. 모든 착물의 특성은 다양한 물리-화학적 방법으로 규명하였다. 몰전기전도도 결과로부터 이들 착물이 이온성을 가짐을 알았다. 전자 및 EPR 스펙트럼을 통해 구리(II) 이온이 사각평면 기하구조를 가짐을 알았다. 구리(II) 착물과 CTDNA(송아지 thymus DNA)의 상호작용을 흡수 스펙트럼과 순환 전압전류법으로 연구하였다. $k_{obs}$ 대 [DNA]의 도시는 선형을 보였는데, 이는 유사-1차반응을 의미한다. 순환 전압전류 그림으로부터 구리(II) 착물이 각각 -0.240 V와 -0.194 V의 $E_{1/2}$ 값을 갖는 일전자 Cu(II)/Cu(I) 산화-환원 쌍에 대해 준가역적임을 알았다. CTDNA를 첨가한 경우, $E_{1/2}$값이 각각168 mV와 18 mV 이동하였고 Ep 값도 감소하였다. CTDNA의 존재 하에 $E_{1/2}$이 이처럼 이동하는 것은 구리(II) 착물이 CTDNA에 강하게 결합됨을 의미한다. A diketo-type ligand was synthesized by the Knoevenagel condensation reaction of thiophene-2-aldehyde with acetylacetone, subsequently its transition metal complexes with Cu(II), Ni(II), and Co(II) chlorides were also prepared. All the complexes were characterized by various physico-chemical methods. The molar conductivity data reveals ionic nature for the complexes. The electronic spectrum and the EPR values suggest square planar geometry for the Cu(II) ion. Interaction of the Cu(II) complex with CTDNA (calf thymus DNA) was studied by absorption spectral method and cyclic voltammetry. The $k_{obs}$ values versus [DNA] gave a linear plot suggesting psuedo-first order reaction kinetics. The cyclic voltammogram of the Cu(II) complex reveals a quasi-reversible wave attributed to Cu(II)/Cu(I) redox couple for one electron transfer with $E_{1/2}$ values -0.240 V and -0.194 V. respectively. On addition of CTDNA, there is a shift in the $E_{1/2}$ values 168 mV and 18 mV respectively and decrease in Ep values. The shift in $E_{1/2}$ values in the presence of CTDNA suggests strong binding of Cu(II) complex to the CTDNA.

        Prediction of the thorax/pelvis orientations and L5-S1 disc loads during various static activities using neuro-fuzzy

        Spinal posture including thorax/pelvis orientations as well as loads on the intervertebral discs are crucial parameters in biomechanical models and ergonomics to evaluate the risk of low back injury. In vivo measurement of spinal posture toward estimation of spine loads requires the common motion capture techniques and laboratory instruments that are costly and time-consuming. Hence, a closed loop algorithm including an artificial neural network (ANN) and fuzzy logic is proposed here to predict the L5-S1 segment loads and thorax/pelvis orientations in various 3D reaching activities. Two parts namely a fuzzy logic strategy and an ANN from this algorithm; the former, developed based on the measured postures of 20 individuals, is to determine 3D orientations of the thorax/pelvis during the various activities while the latter, developed based on the predicted L5-S1 loads by a detailed musculoskeletal model of the spine, is to estimate compression/shear forces at the L5-S1 disc. The fuzzy logic rules are extracted based on Sugeno inference engine and the ANN is trained by LevenbergMarquardt algorithm. To evaluate the performance of the proposed strategy, the comparison between the predicted values, the target values and the presented values in the literature are reviewed. The comparison demonstrated that the proposed algorithm had a promising performance. The maximum relative error for all predictions was ~19 % and with respect to the target values while this error for the literature’s values was ~37 %. Also, the average improvement of the proposed strategy was ~17 % with respect to the presented strategy in the literature.

      • Sensitivity analysis based on complex variables in FEM for linear structures

        One of the efficient and useful tools to achieve the optimal design of structures is employing the sensitivity analysis in the finite element model. In the numerical optimization process, often the semi-analytical method is used for estimation of derivatives of the objective function with respect to design variables. Numerical methods for calculation of sensitivities are susceptible to the step size in design parameters perturbation and this is one of the great disadvantages of these methods. This article uses complex variables method to calculate the sensitivity analysis and combine it with discrete sensitivity analysis. Finally, it provides a new method to obtain the sensitivity analysis for linear structures. The use of complex variables method for sensitivity analysis has several advantages compared to other numerical methods. Implementing the finite element to calculate first derivatives of sensitivity using this method has no complexity and only requires the change in finite element meshing in the imaginary axis. This means that the real value of coordinates does not change. Second, this method has the lower dependency on the step size. In this research, the process of sensitivity analysis calculation using a finite element model based on complex variables is explained for linear problems, and some examples that have known analytical solution are solved. Results obtained by using the presented method in comparison with exact solution and also finite difference method indicate the excellent efficiency of the proposed method, and it can predict the sustainable and accurate results with the several different step sizes, despite low dependence on step size.

        White Blood Cell Count to Mean Platelet Volume Ratio Is a Prognostic Factor in Patients with Non-ST Elevation Acute Coronary Syndrome with or without Metabolic Syndrome

        Background and Objectives Leukocyte and platelet have been found to be associated with metabolic syndrome (MetS). We aimed to determine the usefulness of a novel marker named white blood cell count to mean platelet volume ratio (WMR) for predicting outcomes of non-ST elevation acute coronary syndrome (NSTE-ACS) with or without MetS. Subjects and Methods A total of 331 NSTE-ACS individuals (60±12.5 years, 57.4% male) were enrolled and followed for a median of 24 months. MetS was identified using the National Cholesterol Education Program Adult Treatment Panel III criteria. Results Patients were divided into two groups: high WMR (WMR≥720) and low WMR (WMR<720). Major adverse cardiovascular events (MACE) and MetS rates were significantly greater in the higher WMR group compared to those in the low WMR group (MACE: 14.3% vs. 25%, p=0.014; MetS: 50.9% vs. 75%, p<0.001). MetS was diagnosed in 62.2% of patients. MACE incidence in patients with or without MetS was comparable (p=0.737). Among MetS individuals, patients in the high WMR group had more MACE than the low WMR group (11.2% vs. 26.5%, p=0.007). However, MACE was comparable among non-MetS individuals (p=0.681). In multivariable Cox regression analysis, hazard ratios (HR) of MACE incidence for high-WMR in MetS individuals was 2.616 (95% confidence interval: 1.282–5.339, p=0.008). However, HR of MACE incidence for high WMR in non-MetS individuals was not significant. Conclusion Among NSTE-ACS patients without revascularization therapy, elevated admission WMR was associated with an increased risk of developing composite MACE in MetS individuals but not in non-MetS patients.
