      • Integration of Tobacco Control in Masters of Public Health Curricula of India

        Context: Tobacco is the single largest cause of preventable death among adults globally, as it is in India. Despite this alarming situation, there is very minimal inclusion of tobacco in formal education systems, including the medical discipline, in India. Aims: The present study analyzed the extent of integration of tobacco control related content in Masters of Public Health (MPH) curricula of various institutes in India. Materials and Methods: This cross-sectional study was conducted during January 2011 to May 2011 in all colleges of the country offering a MPH course. The colleges were enlisted using various internet search engines (Google Scholar, Pubmed, Medline), other published literature and snowball technique. A 50 items semi-structured questionnaire was designed, posted and e-mailed (followed by hard copy) to the Person-In-Charge of the MPH program. Statistical Analysis: Descriptive statistics were used to profile the tobacco control content in respective institutions. All data entry and analysis was conducted using SPSS (version 16) for windows. Results: The duration of the MPH course was two years in all institutes and had accreditation with some affiliated body. Tobacco related diseases were covered under 'non communicable diseases' section by every institute. However, a mere 41.4% of institute's had faculty who had received specialized training in tobacco control. More coverage was given to health risks and effects of smoking as compared to cessation interventions (5 A's), symptoms of withdrawal and pharmacological treatments. Only 25% of institutes were in process of introducing tobacco courses into their curricula. Lack of expertise and administrative barriers were cited as perceived major problems in inclusion of tobacco control in MPH curricula. Conclusions: It can be concluded that tobacco control is not receiving adequate attention in public health curricula in India. There is a need for coordinated efforts in the area of tobacco control so as to reduce morbidity and mortality from tobacco induced diseases.

        Nutrient recycling of source-separated human faeces using biochar immobilized indigenous psychrotrophic bacteria for sustaining the agroecosystems of north-western Himalaya

        The Himalayan composting toilets (CTs) offer a sustainable solution for converting human faeces (HF) into com post, supplementing the low-fertile land of the region. However, CTs face challenges such as delayed composting processes (6–8 months), increased heavy metal content, and foul odour. Therefore, the current study evaluated biochar-amended psychrotrophic bacteria for HF degradation under low-temperature conditions (10 ± 2 °C). Out of 153 psychrotrophic bacteria isolated from HF compost, 17 bacterial strains were selected based on highest and two or more hydrolytic activities. Furthermore, considering the isolation source, bacterial strains were exam ined for haemolytic activity, biofilm formation, cytotoxicity and seed germination assay. In total, 14 potential strains belonging to Pseudomonas, Microbacterium, Arthrobacter, Streptomyces, Glutamicibacter, Rhodococcus, Serratia, Exig uobacterium, and Jeotgalicoccus genera were considered safe for both human handling and plants. The composting process was conducted in modified plastic drums at 10 ± 2 °C for 90 days through two treatments: Treatment 1 (T1) involving HF, non-immobilized biochar and cocopeat, and Treatment 2 (T2) involving HF, consortium-immobilized biochar and cocopeat. The consortium-immobilized biochar (T2) degraded HF within 90 days with hemicellulose and cellulose degradation ratios of 73.9% and 62.4%, respectively (p ≤ 0.05). The compost maturation indices like C/N ratio (16.5 ± 1.85), total nitrogen (2.66 ± 0.07), total phosphate (0.4 ± 0.005), total potassium (1.8 ± 0.05) also improved in T2 treatment (p ≤ 0.05). Additionally, T2 was more effective in achieving safe levels of faecal coliforms (< 1000 MPN g−1) and reducing heavy metal content compared to T1. 16S rRNA amplicon-based analysis demonstrated an enhancement of bacterial community diversity in T2, with the presence of Rhodococcus, Pseudomonas, Arthro bacter, and Streptomyces at the end of the composting period promoting HF degradation. Furthermore, T2-fertilized soil showed a germination index (121 ± 0.4, p ≤ 0.05) and stimulated root, shoot and yield by 110%, 45.2%, and 288%, respectively, in pea (Pisum sativum var. AS-10) compared to T1 (49.6%, 19%, and 5.8%, respectively) (p ≤ 0.05). In con clusion, the developed biochar-based formulation proved effective in degrading HF at low temperatures, mitigating foul odours, reducing heavy metals, and enhancing the agronomic value of the final compost. This study presents a promising approach for the sustainable management of HF that can supplement the non-nutritive soil of high altitude regions.

        Bilateral Total Hip Arthroplasty in a Rare Case of Multicentric Reticulohistiocytosis

        Multicentric reticulohistiocytosis (MRH) is a rare systemic disease, which commonly manifests as muco-cutaneous papulonodules and inflammatory erosive polyarthropathy. In this research, we report the clinical manifestations and management of a rare case of MRH with destructive arthropathy of bilateral hip joints and arthritis mutilans presenting with characteristic deformities. Disabling hip arthropathy that occurs secondary to MRH can be successfully managed with bilateral total hip arthroplasty (THA). Osteopenia and acetabular bone defects must be anticipated during THA. This case is reported due to its rare occurrence and because little literature has been published regarding THA in such patients.

        Primary pyomyositis in North India: a clinical, microbiological, and outcome study

        Background/Aims: Pyomyositis is an infective condition with primary involvement of the skeletal muscles. There is sparse recent literature on patients with pyomyositis. Methods: This study was carried out at emergency services of a tertiary care center located in subtropical area of Indian subcontinent. Results: Sixty-two patients of primary pyomyositis formed the study cohort. Mean age of occurrence was 29.9 ± 14.8 years. There were 54 men. Twelve patients had underlying medical diseases. Muscle pain was seen in all 62 patients. Forty-eight patients (77.4%) had the fever. Most common site of involvement was thigh muscles (n = 29, 46.8%). Forty-nine patients (79%) presented in the suppurative stage of illness. Patients with comorbidities were older (age: median 36 years [interquartile range (IQR), 25 to 47] vs. 24 years [IQR, 16 to 35], p = 0.024), had higher culture positivity with gram-negative organisms (8/9 [88.89%] vs. 6/29 [20.69%], p = 0.001). Importantly, higher number of these patients received inappropriate antibiotics initially. Patients with positive pus culture result had higher complication rate (32/38 [84.21%] vs. 10/18 [55.56%], p = 0.044). Six patients (9.7%) had in-hospital mortality. Lower first-day serum albumin, initial inappropriate antibiotic therapy, and advanced form of the disease at presentation were associated with increased in-hospital mortality. Conclusions: Primary pyomyositis is not an uncommon disease entity. Patients with comorbidities were more likely to receive initial inappropriate antibiotic therapy. Patients with positive pus culture report had the higher rate of complications. Lower first-day serum albumin, initial inappropriate antibiotic therapy and advanced form of the disease at presentation were associated with increased in-hospital mortality.

        68Ga-DOTA-RGD2 Uptake in Plantar Fasciitis: New Insights into its Pathogenesis

        Plantar fasciitis is chronic degenerative tendinopathy of the plantar fascia. Although it has many treatment modalities, none has been very effective due to the largely unknown pathogenesis of this condition. 68Ga-DOTA-RGD2 has an upcoming role in assessing disease activity and treatment response in rheumatoid arthritis. In the present case, we demonstrate the potential role of angiogenesis imaging in understanding the pathogenesis of plantar fasciitis and provide new opportunities for novel imaging options.

        Determination of levels of nitric oxide in smoker and nonsmoker patients with chronic periodontitis

        Purpose: Cigarette smoking is a major risk factor in periodontal diseases. The pathogenesis of periodontal diseases may be affected by alterations of the inflammatory response by smoke. Nitric oxide (NO) is a gaseous, colorless, highly reactive, short-lived free radical with a pivotal role in the regulation of various physiological and pathological mechanisms in the body. It is important in host defense and homeostasis, on the one hand, whereas, on the other hand, it modulates the inflammatory response in periodontitis, leading to harmful effects. The aim of this study was to assess the levels of NO in both the serum and saliva of smokers and nonsmokers having chronic periodontitis and to compare them with periodontally healthy controls. Methods: Sixty subjects participated in the study and were divided into three groups: group I, healthy nonsmoking subjects; group II, nonsmoking patients with chronic periodontitis; group III, smoking patients with chronic periodontitis. Each group consisted of twenty subjects. The biochemical estimation of NO in the collected serum and in the saliva was performed using the Griess colorimetric reaction. Results: The results showed that the mean value of the salivary and serum NO was greater in group II than in group I, and also greater in group III than in group II. Conclusions: NO appears to play an important and rather complex role in the immuno-inflammatory process and in the remodeling and maintenance of osseous structures. It is therefore logical that modulation of this mediator has potential for the treatment of a number of inflammatory conditions including periodontal disease.
