Food allergy is an immune-mediated adverse reaction that occurs mainly by consumption of food. Some of the children with food allergies have mild symptoms like urticaria and pruritus, while others manifest fatal symptoms like anaphylaxis. Many cases develop in infants and children, and disappear spontaneously over time. The prevalence of food allergy is known to be up to 6% of children and 3%-4% of adults. Growing concern is that prevalence of food allergy seems to be increasing. The prevalence, common allergens and natural course of food allergy vary according to the race, age, eating habits, and unknown factors. In Korea, many researchers have reported regional data on food allergy for comparison with those of foreign countries. Awareness is increasing and food labelling is being established to help people avoid allergenic foods. In the future, monitoring of prevalence and common food allergens at regular intervals is needed to make appropriate policies for children with food allergy. There are still lack of data on pathogenesis, natural course and clinical trials of immunotherapy in Korean children. In addition, it is necessary to form a consensus on primary prevention of food allergy by early exposure to allergenic foods. Further regional studies should be performed to expand our understanding of food allergy and to lead to proper management for Korean children with food allergy. Atopic dermatitis (AD) has become a serious epidemic in Korean children. We aimed to investigate the association between vitamin C, E and other nutrients, and serum total IgE/specific IgE levels in children with AD. A total of 119 children (0-24 mo) diagnosed with AD were recruited for this cross-sectional study from a medical center in Seoul. A 24 h recall was used to assess dietary intakes. Serum total and six food-allergen specific IgE levels were measured by CAP-FEIA. Serum vitamin E was also measured but only in 25 out of the total 119 participants. Multiple linear regression analysis was performed to estimate the coefficients between serum IgE levels and dietary intake as well as serum vitamin E. Serum vitamin E levels showed a significantly inverse association with serum total IgE and all specific IgE levels (P < 0.05). Fat intake was inversely related with specific-IgEs for egg whites, milk, buck wheat, soy, and peanuts (P < 0.05). Positive associations were found between carbohydrate (CHO) intake and total IgE and specific IgEs to egg whites, milk, soy, and peanuts (P < 0.05). Vitamin C, E and n-3/n-6 fatty acids were not related with serum total IgE and specific IgE levels except for the association between buck wheat and vitamin E. In addition, there were no significant differences between males and females in dietary intake and serum IgE levels by student’s t-test. Although dietary vitamin E showed no association with serum IgE levels, serum vitamin E drew a significant inverse relationship with serum IgE levels. The evidence seems to suggest that vitamin E may possibly lower total and specific-IgEs in children with AD, and that it is important to maintain a relatively high serum vitamin E level in children with AD.
Genome Sequence Analysis of H5N1 Influenza A Virus Isolated from a Vietnamese in 2007

Highly pathogenic H5N1 avian influenza A virus (AIV) crossed the species barrier and caused a number of deaths in humans in Vietnam and 14 other countries. Since the last report of human H5N1 infection in November 2005, the first documented H5N1 human infection was reported in June 2007 in Vietnam and was followed by 7 more cases, including 5 fatalities. In this study, we isolated and analyzed the full length of the H5N1 genome from a sample from the first patient in 2007. Phylogenetic analysis of eight genomic segments of the H5N1 virus strain (A/Vietnam/HN/2007, VNH07) revealed that this strain appears to be of genotype V and contains the HA gene, which is classified into clade 2.3.4. The deduced amino acid sequence of the HA protein has a typical affinity sequence for α2,3 linkage (SAα2,3-Gal) receptors and typical multibasic cleavage sequences. Compared with other H5N1 isolates, VNH07 showed that the possible reassortments for the NA and NP segments occurred between A/goose/Guangxi/3017/2005-like isolates (2.3.2) and A/human/Zhejiang/16/2006-like isolates (2.3.4).

Background: Atopic dermatitis (AD) is a heterogeneous disease with different age of onset, disease course, clinical symptoms, severity, and risk of comorbidity. The characteristics of children with AD also vary by age or country. However, little is known about the clinical characteristics of AD in Korean school-aged children and adolescents. Furthermore, there are few studies on phenotypic differences according to onset age. This study aimed to explore the clinical characteristics and phenotypes according to onset age and severity of AD in children and adolescents in Korea. Methods: AD patients aged 6–18 years who presented to 18 hospitals nationwide were surveyed. The patients were examined for disease severity by pediatric allergy specialists, and data on history of other allergic diseases, familial allergy history, onset age, trigger factors, lesion sites, treatment history and quality of life were collected. The results of the patient’s allergy test were also analyzed. The patients were classified into infancy-onset (< 2 years of age), preschoolonset (2–5 years of age), and childhood-onset (≥ 6 years of age) groups. Study population was analyzed for clinical features according to onset-age groups and severity groups. Results: A total of 258 patients with a mean age of 10.62 ± 3.18 years were included in the study. Infancy-onset group accounted for about 60% of all patients and presented significantly more other allergic diseases, such as allergic rhinitis and asthma (P = 0.002 and P = 0.001, respectively). Food allergy symptoms and diagnoses were highly relevant to both earlier onset and more severe group. Inhalant allergen sensitization was significantly associated with both infancy-onset group and severe group (P = 0.012 and P = 0.024, respectively). A family history of food allergies was significantly associated with infancyonset group (P = 0.036). Severe group was significantly associated with a family history of AD, especially a paternal history of AD (P = 0.048 and P = 0.004, respectively). Facial (periorbital, ear, and cheek) lesions, periauricular fissures, hand/foot eczema, and xerosis were associated with infancy-onset group. The earlier the onset of AD, the poorer the quality of life (P = 0.038). Systemic immunosuppressants were used in only 9.6% of the patients in the severe group. Conclusion: This study analyzed the clinical features of AD in Korean children and adolescents through a multicenter nationwide study and demonstrated the phenotypic differences according to onset age and severity. Considering the findings that the early-onset group is more severe and accompanied by more systemic allergic diseases, early management should be emphasized in young children and infants.

최근 숲의 치유기능에 대해 임학과 의학 분야에서 큰 관심을 보이고 있다. 본 연구는 국립공원 치유 캠프가 아토피피부염 환자들의 증상완화에 미치는 영향을 평가하고자 수행되었다. 치유 캠프의 효과는 의사 진단에 의한 환자의 중증도와 환자 본인의 가려움 정도를 캠프 전·후 평가하여 검증하였다. 연구결과, 국립공원 치유캠프는 아토피피 부염 환자들의 증상 호전 및 심리적 안정에 효과적인 것으로 나타났다. The therapeutic effect of forest settings is an emerging issue in the field of forestry and medicine. The purpose of this study was to evaluate the therapeutic efficacy of the national park healing camp for patients with atopic dermatitis (AD). Using pre-test and post-test group design with objective and subjective SCORAD score, mean changes in patient with atopic dermatitis by completion of the healing camp was measured. The result of this study indicated that the national park healing camp showed that has potential as an improved health status and psychological relaxation for patients with AD.
