        Early Cardiac Dysfunction in Biopsy-proven Nonalcoholic Fatty Liver Disease

        Background/Aims: Nonalcoholic fatty liver disease (NAFLD) encompasses a range of diseases from nonalcoholic fatty liver (NAFL) to nonalcoholic steatohepatitis (NASH) and has been linked to cardiovascular disease and sub-clinical cardiac remodeling. This paper presents a retrospective study of biopsy-proven NAFL and NASH to examine the differences in subclinical cardiac remodeling. Methods: Patients were recruited from an institutional repository of patients with liver-biopsy-confirmed NAFLD. Patients with a transthoracic echocardiogram (TTE) within 12 months of the liver biopsy were included. The parameters of the diastolic dysfunction were reviewed for the differences between NAFL and NASH as well as between the stages and grades of NASH. Results: Thirty-three patients were included in the study, 17 with NAFL and 16 with NASH. The NASH patients were more likely to have lower platelets, higher AST, higher ALT, and higher rates of type 2 diabetes mellitus, coronary artery disease, and hypertension than the NAFL patients. The E/e’ ratio on transthoracic echocardiogram was significantly higher in NASH compared to NAFL, advanced-stage NASH compared to early stage, and high-grade NASH compared to low-grade. The E/e’ ratio was also significantly higher in NASH than NAFL in patients without diabetes mellitus. The presence of diastolic dysfunction trended toward significance. The other markers of diastolic dysfunction were similar. Logistic regression revealed a statistical association with E/e' and NASH. Conclusions: NASH patients had evidence of a higher E/e’ ratio than NAFL, and there was a trend towards a significant diastolic dysfunction. Patients with NASH compared to NAFL should be closely monitored for signs and symptoms of cardiac dysfunction. (Korean J Gastroenterol 2021;78:161-167)

        Calibration of Time-Interleaved Errors in Digital Real-Time Oscilloscopes

        <P>A channel mismatch calibration method is proposed for use in time-interleaved digital real-time oscilloscope (DRTO) applications. Linear equations are derived using Fourier transforms of the separated signals from each of the time-interleaved analog-to-digital converters (TIADCs). Thus the errors in the TIADCs can be easily calibrated by inversion of a matrix, as opposed to most previous work where additional filters are employed. The calibration accuracy of the proposed method is limited only by the noise produced after the TIADC circuitry, while other methods depend on the filter design. A transfer function measurement method is then proposed for application to commercially available DRTOs. Two-tone signals are measured using DRTOs from various suppliers to validate the proposed method. The occurrence of signals at spurious frequencies is considerably reduced, as demonstrated by the calibrated results.</P>

      • A New Origin and Function of Poomsae in Taekwondo Training

        The objective of this paper is to establish a new paradigm for the understanding of poomsae in Taekwondo training. Until now poomsae have been considered as a system of connected offensive and defensive drills for teaching the fundamentals of combat. Hence, they are understood as a means and preparatory stages for attaining fighting skills, and consequently, they are treated as a secondary activity to kyorugi (sparring). Contrary to this understanding of poomsae, we want to argue that poomsae training has its own end, independent from kyorugi training. To demonstrate this point, we will focus on the relation between poomsae and the Chinese gymnastic exercises known as daoyin-sul or daoyin exercises (i.e., Daoist exercises of guiding and pulling), which are performed in conjunction with breathing techniques and the stretching of the body to accumulate qi (vital energy) in order to provide a foundation for the potential to realize the pure self.

        Exciton binding energy and the optical absorption coefficient in a strained CdxZn1-xO/ZnO quantum dot

        Numerical calculations of the excitonic absorption spectra in a strained CdxZn1-xO/ZnO quantum dot are investigated for various Cd contents. We calculate the quantized energies of the exciton as a function of dot radius for various confinement potentials and thereby the interband emission energy is computed considering the internal electric field induced by the spontaneous and piezoelectric polarizations. The optical absorption as a function of photon energy for different dot radii is discussed. Decrease of exciton binding energy and the corresponding optical band gap with the Cd concentration imply that the confinement of carriers decreases with composition x. The main results show that the confined energies and the transition energies between the excited levels are significant for smaller dots. Non-linearity band gap with the increase in Cd content is observed for smaller dots in the strong confinement region and the magnitude of the absorption spectra increases for the transitions between the higher excited levels.

        Optical susceptibility of a confined magneto-LO-polaron in a strained GaAs<sub>0.9</sub>P<sub>0.1</sub>/GaAs<sub>0.6</sub>P<sub>0.4</sub> quantum dot

        Magneto-LO-polaron in a cylindrical GaAs<SUB>0.9</SUB>P<SUB>0.1</SUB>/GaAs<SUB>0.6</SUB>P<SUB>0.4</SUB> quantum dot is studied taking into consideration of geometrical confinement. The effect of phonon on the exciton binding energy is computed applying variational method within the single band effective mass approximation numerically. And the nonlinear optical properties with the incident photon energy are investigated in the presence of magnetic field strength and the phonon effect using compact density matrix method. Optical susceptibilities of generation of second and third harmonic as a function of incident energy for a confined exciton in the presence of magnetic field strength in a GaAsP quantum dot with and without the phonon effect are carried out. Our results show that the second and third harmonic generations have influence on the effects of geometrical confinement, the phonon and the magnetic field strength.

        Myoclonic status epilepticus after severe hyperthermia in a patient with coronavirus disease 2019

        Myoclonic status epilepticus (MSE) is a sign of severe neurologic injury in cardiac arrest patients. To our knowledge, MSE has not been described as a result of prolonged hyperpyrexia. A 56-yearold man with coronavirus disease 2019 presented with acute respiratory distress syndrome, septic/hypovolemic shock, and presumed community-acquired pneumonia. Five days after presentation, he developed a sustained fever of 42.1°C that did not respond to acetaminophen or ice water gastric lavage. After several hours, he was placed on surface cooling. Three hours after fever resolution, new multifocal myoclonus was noted in the patient’s arms and trunk. Electroencephalography showed midline spikes consistent with MSE, which resolved with 40 mg/kg of levetiracetam. This case demonstrates that severe hyperthermia can cause cortical injury significant enough to trigger MSE and should be treated emergently using the most aggressive measures available. Providers should have a low threshold for electroencephalography in intubated patients with a recent history of hyperpyrexia.

        Islanding Detection for Distributed Generation

        The paper discussed various techniques for islanding detection and described in detail the working principles of ROCOF and vector shift relays. A simulator was used to investigate why the stability of both types of relays maybe affected by network and load switching, short-circuit faults and in the case of a ROCOF relay the loss of bulk generation or the tripping of a transmission feeder. Finally, to enhance the reliability of Loss of Mains protection, a new islanding detection technique based on a GPS synchronized phasor measurement system was proposed.

        <P><B>Abstract</B></P><P>Magneto-exciton bound donor is investigated theoretically in a GaN/Ga<SUB>1−<I>x</I></SUB>Al<SUB><I>x</I></SUB>N quantum dot within the framework of single band effective mass approximation and compared with the experimental results. Binding energies on excitons bound hydrogenic donors and the diamagnetic susceptibility are obtained as a function of dot radius and magnetic field. The valence-band anisotropy is included in our theoretical model using different hole masses in different spatial directions. Zeeman effect is calculated through the energy dependent effective mass. The dependence of donor bound exciton diamagnetic shift is found. The interband optical transition of GaN/Ga<SUB>1−<I>x</I></SUB>Al<SUB><I>x</I></SUB>N dot is computed with various structural parameters in the influence of magnetic field. Our results show that (i) quantum size, the magnetic field and the interband optical transitions have a considerable influence on neutral donor exciton states, (ii) the diamagnetic susceptibility increases with the magnetic field and is not pronounced for smaller dot radii and (iii) the found diamagnetic shift is agreed with the other experimental investigator [Yoichi Yamada et al., 2000 [26]].</P>

        MINERVA: SMALL PLANETS FROM SMALL TELESCOPES

        The Kepler mission has shown that small planets are extremely common. It is likely that nearly every star in the sky hosts at least one rocky planet. We just need to look hard enough-but this requires vast amounts of telescope time. MINERVA (MINiature Exoplanet Radial Velocity Array) is a dedicated exoplanet observatory with the primary goal of discovering rocky, Earth-like planets orbiting in the habitable zone of bright, nearby stars. The MINERVA team is a collaboration among UNSW Australia, Harvard-Smithsonian Center for Astrophysics, Penn State University, University of Montana, and the California Institute of Technology. The four-telescope MINERVA array will be sited at the F.L. Whipple Observatory on Mt Hopkins in Arizona, USA. Full science operations will begin in mid-2015 with all four telescopes and a stabilised spectrograph capable of high-precision Doppler velocity measurements. We will observe ~100 of the nearest, brightest, Sun-like stars every night for at least five years. Detailed simulations of the target list and survey strategy lead us to expect $15{\pm}4$ new low-mass planets.
