Severe hypercalcemia due to persistent hyperparathyroidism after kidney transplantation causes serious medical complications. Medical or surgical treatment is recommended to decrease serum calcium level, but percutaneous ethanol injection therapy (PEIT) is an also useful method for patients who are refractory to medical treatment and under poor condition for parathyroidectomy. Here, we report a case of successful PEIT of refractory hypercalcemia in a 62-year old patient after kidney transplantation. This patient underwent PEIT because of poor general condition. Just after PEIT, serum calcium level was normalized and biointact PTH level was markedly decreased without serious complications. He has been followed up for 6 months without recur so far. PEIT may be recommended for treatment of sustained hypercalcemia in kidney-transplant patients who are refractory to medical treatment or in poor condition for surgical treatment.
쇄골하동맥과 원위부 내경동맥의 혈전을 동반한 흉곽출구 증후군 1예

본 증례는 첫째 늑골 변형으로 쇄골하동맥 혈전 및 상지색전과 함께 뇌경색이 동반된 동맥성 흉곽출구 증후군이 발생한 환자다. 레이노 현상을 보이면서 상지의 색전증이나 뇌경색을 동반하는 환자에서 동맥성 흉곽출구 증후군의 가능성을 염두에 두고 자세한 병력, 이학적 검사 및 방사선학적 검사 등을 시행하는 것이 필요하다고 생각한다. Thoracic outlet syndrome (TOS) manifests as a spectrum of symptoms that are produced by compression of neurovascular bundles as they pass through the scalene muscles, clavicle, and the first rib. When a vascular structure like subclavian artery is involved, TOS may result in serious complications, such as thromboembolism at distal extremities or, more rarely, the brain. Here, we present a case of 25-year-old male presenting with Raynaud`s phenomenon, finger tip necrosis, and left-sided hemiplegia secondary to thromboembolism associated with arterial TOS. Radiologic studies, including simple X-ray, neck computed tomography (CT) angiography, thoracic aortography, and cerebral angiography, revealed anomalous articulation of the right first rib to the second rib, right subclavian artery stenosis with poststenotic dilatation and intramural thrombus, and thromoboembolism of the right distal internal carotid artery (ICA). The patient successfully underwent resection of the anterior scalene muscle and embolectomy. (Korean J Med 75:602-606, 2008)

Background: The purpose of this study was to demonstrate the norepinephrine-induced nociceptive effects by monitoring catecholamines and its metabolites in human body. Methods: To exam the antisympathetic effect from the healthy volunteer(male:15, female:15) by monitoring changes of epinephrine, norepinephrine, dopamine, metanephrine, normetanephrine, and others of urine, a comparative study with warm water immersion ($40.8{\pm}0.3^{\circ}C$) and infrared (250W) was carried out. Results: The urinalysis showed that the concentration of epinephrine and norepinephrine were significantly decreased by both warm water immersion-and infrared-stimulated group of urine in 24 hours. Conclusion: Therefore, these results suggest that the diminished responsiveness on the epinephrine and norepinephrine to warm water immersion and infrared in volunteer may be, in part, related by the increased of antisympathetic effects.
뇌허혈 유발 후 전침자극이 전뇌세포사 중 Bax와 Caspase-3 발현 변화에 미치는 영향 연구

Objectives: The study aims to look into how the application of EA(Electro-acupuncture) in the initial step of ischemic brain injury affects the changes in Bax and Caspase-3 expression among forebrain apoptosis. Methods: It caused GI by using common carotid artery occlusion and conducted the test by dividing into the no applied EA group after 6 hours and 12 hours, the LI 4 EA group that applied EA to LI 4, and GV 20 EA group that was applied to GV 20. After that, the following results were obtained by comparing Bax and Caspase-3 expression, which were apoptosis-related factors, with its application time through immunohistochemical staining after extracting brain of each group. Results: According to each group`s Bax expression by the application time, the order of expression increase after 6 hours is shown to be the no applied EA group, LI 4 group, and GV 20 group. The stimulation after 12 hours was most lowly expressed in the GV 20 group, and the no applied EA group and LI 4 group showed similar level. Conclusions: EA stimulation in the initial step after ischemia seems to affect positively Bax and Caspase-3 expression that are index of forebrain apoptosis.
대퇴네갈래근에 테이핑요법 적용이 근력과 근피로에 미치는 영향

Background: This study examines the changes in muscular strength and muscle fatigue substances when taping treatment is applied in squat exercise and treadmill exercise, in which quadriceps femoris muscles are used frequently so as to clarify the effects of taping as an aid of exercise ability improvement. Methods: 20 healthy male adults in their 20's participated in the study, in which tapes were put on their quadriceps femoris muscles, and various variables as the indicator of muscular strength and muscle fatigue substances were measured by means of the measuring tool before and after exercises. Results: the following conclusion was drawn up: 1. In exercise using muscular strength of the right knee extension, the reduction of muscular strength after exercise of the taping group showed significant statistical difference from that of the non-taping group. 2. In exercise using muscular strength of the lower limbs, the reduction of blood Glaucos after exercise of the taping group showed significant statistical difference from that of the non-taping group. 3. In exercise using muscular strength of the lower limbs, the value of lactic acid after exercise of the taping group showed significant statistical difference from that of the non-taping group. Conclusion: Therefore, the results of this study demonstrate that the application of taping treatment in sports activity or daily life where quadriceps femoris muscles are used a lot can have positive effects on the improvement of muscular strength and relief of muscle fatigue.

부비동에서 기원한 소세포암은 매우 드문 질환으로 부비동염으로 오인될 수 있어 주의가 필요하다. 조직검사와 함께 전신적 검사를 실시하여 원격전이 유무를 감별해야 하고 공격적이고 빠른 진행을 고려할 때 전신항암치료 및 방사선치료를 우선적으로 고려해야 한다. 저자들은 59세 남자에서 원격 전이를 동반한 상악동에서 기원한 소세포암을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Primary small cell carcinoma of the paranasal sinuses is extremely rare. It is aggressively locally invasive, with high recurrence and metastatic rates. A combination of systemic chemotherapy and locoregional treatment, such as radiotherapy, is currently recommended based on the treatment of small cell carcinoma of lung. We report a case of small cell carcinoma originating from the maxillary sinus with distant metastasis at the time of diagnosis. The patient had a good initial therapeutic response to etoposide-cisplatin chemotherapy, but the cancer progressed after the sixth cycle of chemotherapy and the prognosis was poor. 본 연구에서는 무인 농업기계 개발의 기초 연구로써 2D 기계시각 및 딥러닝을 이용하여 농작업 시 작업 영역의 경계를 검출을 목적으로 하며 트랙터의 경운작업 시 정면영상 내 경작 전, 후 영역 검출 기술 개발 및 성능을 평가하였다. 딥러닝 기반 경계 검출을 위해 트랙터 경운 작업의 정면영상을 수집하였으며, 입력된 영상의 구역별 분류 기반 영역 군집화를 통해 경계를 검출하였다. 구역별 영상의 분류는 4개의 합성곱신경망과 1개의 완전연결망으로 구성된 인경신경망 모델을 이용하였으며, 분류기에서 출력되는 경작 후 부류의 확률값을 원 영상에서의 위치를 기준으로 결합하여 2차원의 확률맵이 생성하였다. 분할된 영역은 측면과 상단에서 각각의 경계를 가지고 있으며, 측면이 경작 전 영역과 맞닿아 있는 영역의 경계로, 허프변환을 이용하여 이를 대표직선으로 표현하였다. 성능평가 결과 학습모델의 분류 성능은 F1-score가 약 0.91, 경계 검출 성능은 경작 전, 후 영역의 위치에 따라 횡방향 오차 평균 11~12.6 cm, 각도 오차 평균 8.0~9.8°로 나타났다. 본 연구결과는 기존에 수행된 농업분야 자율주행 기술의 성능범위 수준을 보여주었으며, 저가의 2D 영상 시스템 및 딥러닝을 이용한 자율주행 농업기계 기술이라는 측면에서 기존 연구에 비해 높은 실용화 가능성 및 넓은 적용 범위의 장점이 있다고 판단된다.
