The receptor activator of nuclear factor κB (RANK) and its ligand RANKL are key regulators of osteoclastogenesis and well-recognized targets in developing treatments for bone disorders associated with excessive bone resorption, such as osteoporosis. Our previous work on the structure of the RANK-RANKL complex revealed that Loop3 of RANK, specifically the non-canonical disulfide bond at the tip, performs a crucial role in specific recognition of RANKL. It also demonstrated that peptide mimics of Loop3 were capable of interfering with the function of RANKL in osteoclastogenesis. Here, we reported the structure-based design of a smaller peptide with enhanced inhibitory efficiency. The kinetic analysis and osteoclast differentiation assay showed that in addition to the sharp turn induced by the disulfide bond, two consecutive arginine residues were also important for binding to RANKL and inhibiting osteoclastogenesis. Docking and molecular dynamics simulations proposed the binding mode of the peptide to the RANKL trimer, showing that the arginine residues provide electrostatic interactions with RANKL and contribute to stabilizing the complex. These findings provided useful information for the rational design of therapeutics for bone diseases associated with RANK/RANKL function.

Purpose: The goal of oncoplastic breast surgery is to restore the appearance of the breast and improve patient satisfaction. Thus, the assessment of cosmetic results and patient-reported outcomes (PROs) using appropriately constructed and validated instruments is essential. The aim of the present study was to assess the long-term objective cosmetic results and corresponding PROs after oncoplastic breast surgery. Methods: Cosmetic results were assessed by the patients, a medical panel, and a computer program (BCCT.core). PROs were assessed using BREAST-Q, a questionnaire that measures the perception of patients having breast surgery. The cosmetic results and PROs were analyzed in patients who underwent quadrantectomy and partial breast reconstruction utilizing the latissimus dorsi flap. Results: The mean duration of the follow-up period was 91.6 months (range, 33.3–171.0 months), and mean age of the patients was 51 years old (range, 33–72 years). The mean tumor size was 2.1 cm (range, 0.9–5.5 cm). There was fair agreement between the medical panel and BCCT.core score (K = 0.32, P < 0.001), and a statistically significant correlation between the BCCT.core score and medical panel cosmetic results was identified (r = 0.606, P < 0.001). A better BCCT.core result was related to a higher PRO of each BREAST-Q domain—satisfaction with breasts (R<SUP>2</SUP> = 0.070, P = 0.039), satisfaction with outcome (R2 = 0.087, P = 0.021), psychosocial well-being (R<SUP>2</SUP> = 0.085, P = 0.023), sexual well-being (R<SUP>2</SUP> = 0.082, P = 0.029), and satisfaction with information (R<SUP>2</SUP> = 0.064, P = 0.049). Conclusion: Our long-term results of oncoplastic surgery achieved a high level of patient satisfaction with good cosmetic results. The medical panel and BCCT.core results correlated well with the PROs of the patients using valid, reliable, and procedure-specific measures.

GG-43 : A case of repeated spontaneous vulvar hematoma in pregnant woman

In adult female, most of vulvar hematoma is usually caused by traumatic injury and spontaneous vulvar hematoma is rare condition. In pregnancy, vulvar hematoma occur as rare complication during childbirth. Only a few cases of spontaneous vulvar hematoma during pregnancy have been reported in the literature, but to the best of my knowledge there is no report about repeated spontaneous vulvar hematoma in pregnancy. A 33 years old pregnant women admitted to the hospital for spontaneous left side vulvar hematoma at 30 weeks pregnant and treated with surgery to remove hematoma. But at 39 weeks pregnant, right side vulvar hematoma occurred spontaneously and delivered by cesarean section.

To evaluate the clinical features of fetuses with prenatally diagnosed parvovirus B19 infection and hydrops fetalis This is a retrospective review of all fetuses diagnosed at Asan Medical Center with fetal hydrops fetalis caused by anemia due to parvovirus B19 infection between March 1999 and March 2011. Fetal and neonatal conditions were identified from the maternal and neonatal electronic medical records and recorded images. In some cases, telephone interviews were conducted to determine the conditions of some newborns. Parvovirus B19 infection was diagnosed by polymerase chain reaction of the amniotic fluid or fetal cord blood in our laboratory. Fetal anemia was assessed by Doppler measurement of the middle cerebral artery peak systolic velocity (MCA-PSV) and confirmed by CBC of fetal blood obtained by cordocentesis. Intrauterine transfusions (IUT) were performed only if MCA-PSV was >1.5 MoM. Following IUT, we periodically monitored MCA-PSV to screen for anemia recurrence. Our study population included 10 cases with prenatally diagnosed parvovirus B19 infection that was associated with hydrops fetalis. The median gestational age at diagnosis was 21 weeks (range: 16.3-24.2 weeks). Among these, 5 cases received IUT. Four fetuses survived and were healthy at birth. One fetus died in utero after 2 fetal transfusions at 24.5 weeks of gestation. The remaining 5 cases managed without IUT. Two cases survived and were healthy at birth (MCA-PSV < 1.5 MoM). Three cases were dead in utero at 17.0-18.0 weeks of gestation after short follow-up period. One fetus died at the first abnormal ultrasound examination, and the others died at the second follow-up examination but before transfusion. All of the surviving newborns were doing well without complications when this study was prepared. The survival rate of parvovirus infection in association with hydrops fetalis was 60%. Intrauterine transfusion is useful for the management and treatment of fetal anemia due to parvovirus B19 infection.
