Sixty-one patients with coronary artery stenosis underwent coronary artery bypass grafting from February 1986 to September 1993 at the department of Thoracic and Cardiovascular Surgery. There were 41 men and 20 women with a mean age of 54 years(range, 25 to 79 years). History of myocardial infarction was noted in 17 patients(28%). Twenty-eight patients(46%) had single-vessel disease, 24 patients(39%) had double-vessel disease and 9 patients(15%) had triple-vessel disease. The left main disease was in 4 patients(7%). Single aorta-coronary grafts were placed in 37 patients(60%), double or more grafts in 24 patients(40%). An average of 1.4 grafts were performed per patient. Saphenous vein grafts were employed in 49 patients(80%), and the internal mammary grafts, which were anastomosed to left anterior descending artery in 23 patients(38%). The mean bypass time was 135 minutes and cross-clamp time was 45 minutes. Early mortality was 16.4% (10 patients). Perioperative myocardial infarction was the most common complication, followed by atrial arrhythmia, wound infection, bleeding, pneumonia, pneumothorax. The 51 survivors were followed-up for 1 to 72 months, averaging 31 months. Ninety-five percent of the survivors were in New York Heart Association class I or Ⅱ postoperatively. There was one late death. Poor ejection fraction was significantly associated with hospital death(p<0.05).

Because of the anatomical form of posterior teeth, the amount of tooth reduction near the marginal ridge area in the typical, widely used intracoronal cavity will be inevitably maximal, while occlusal forces on the restoration in this area will be relatively minimal In an attempt to dicrease the amount of tooth reduction at the marginal ridge area, a modified cavity for inlay with a uniform depth of 15mm, just as in amalgam filling was designed in this study. To analize the stress development in the teeth in that cavity design, 2㎏/㎟ distributive forces were applied on the inlay restoratives and obtained results were compared with that in the typical cavity design which had a flat pulpal wall with a depth of 1.5m at the central fossa area. Analysis was done with two dimensional finite technique. The obtained results were as follows : 1. The patterns of stress distribution and displacement showed no apparent differences. 2. The stress values and the amount of displacement developed, both in inlay cast gold and in the teeth, in the cavity with a uniform depth were less than that in the cavity with a flat pulpal wall. 3. Greatest maximum principle values were concentrated around the interface between the base of inlay body and the teeth.

Objective: To evaluate the diagnostic performance of the modified Korean Thyroid Imaging Reporting and Data System (K-TIRADS), and compare it with the 2016 version of K-TIRADS using the Thyroid Imaging Network of Korea. Materials and Methods: Between June and September 2015, 5708 thyroid nodules (≥ 1.0 cm) from 5081 consecutive patients who had undergone thyroid ultrasonography at 26 institutions were retrospectively evaluated. We used a biopsy size threshold of 2 cm for K-TIRADS 3 and 1 cm for K-TIRADS 4 (modified K-TIRADS 1) or 1.5 cm for K-TIRADS 4 (modified K-TIRADS 3). The modified K-TIRADS 2 subcategorized the K-TIRADS 4 into 4A and 4B, and the cutoff sizes for the biopsies were defined as 1 cm for K-TIRADS 4B and 1.5 cm for K-TIRADS 4A. The diagnostic performance and the rate of unnecessary biopsies of the modified K-TIRADS for detecting malignancy were compared with those of the 2016 K-TIRAD, which were stratified by nodule size (with a threshold of 2 cm). Results: A total of 1111 malignant nodules and 4597 benign nodules were included. The sensitivity, specificity, and unnecessary biopsy rate of the benign nodules were 94.9%, 24.4%, and 60.9% for the 2016 K-TIRADS; 91.0%, 39.7%, and 48.6% for the modified K-TIRADS 1; 84.9%, 45.9%, and 43.5% for the modified K-TIRADS 2; and 76.1%, 50.2%, and 40.1% for the modified K-TIRADS 3. For small nodules (1–2 cm), the diagnostic sensitivity of the modified K-TIRADS decreased by 5.2–25.6% and the rate of unnecessary biopsies reduced by 19.2–32.8% compared with those of the 2016 K-TIRADS (p < 0.001). For large nodules (> 2 cm), the modified K-TIRADSs maintained a very high sensitivity for detecting malignancy (98%). Conclusion: The modified K-TIRADSs significantly reduced the rate of unnecessary biopsies for small (1–2 cm) nodules while maintaining a very high sensitivity for malignancy for large (> 2 cm) nodules.

N/A Background/Aims: p53 mutation was reported in many studies, but conventional PCR-SSCP protocol using radiolabeled PCR primers or nucleotides to generate a radioactive PCR products is cumbersome to handle. With the aim of a rapid and safe method for single-strand conformation polymorphism(SSCP) analysis of PCR products, we used Mini-SSCP method to detect the mutation of p53 gene of gastric cancer. Methods: In 27 pair of gastric cancer and normal tissues, PCR-SSCP was done with Mini-SSCP, and then Silver sequencing was done. Results: The p53 gene mutation of gastric cancer was found in 10 of 27(37%). Mutation of exon 4, 5 were 2, 5 cases and tnutation of exon 6, 7 mutation were 0, 3 cases respectively. Mutation rate was similar with other reports. P53 mutation was not correlated with stage, lymph node involvement and histological differentiation. Conclusions: Mini-SSCP method, modification of Cold-SSCP, greately increased our accessibility to detect polymorphisms with easiness and safeness. So, it is a useful method to detect genetic mutation. (Korean J Gastroenterol 1997;29:601-609)
