Persistent aura without infarction is defined as an aura persisting for 1 week or more without evidence of infarction on neuroimaging. It is difficult to differentiate persistent visual aura without infarction from occipital lobe epilepsy. We report a Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy patient with prosopometamorphopsia and visual field defect improved by valproic acid. We also review ambiguity between visual aura in migraine and occipital lobe epilepsy.
중력 하중 차이에 의한 철근 콘크리트 플랫 플레이트 골조의 비선형 정적 해석

The purpose of this study was to analyze the seismic performance of reinforced concrete flat plate frame according to the gravity shear ratio. The gravity shear ratio is changed by gravity load. shear ratio. The failure modes which are composed of flexural failure and shear failure mode are related to gravity shear ratio. So 3 and 7 story frames are designed by different gravity loads. In study, the slab-column connections are used to analysis model in prior study. The analytical model is able to represent punching shear failure and fracture mechanism. Reinforced flat plate Frames are conducted to nonlinear pushover analysis. The analytical results are influenced by the gravity shear ratio.

This study proposed an analytical model for predicting nonlinear behavior of reinforced concrete(RC) flat plate frames. In general, RC flat plate frames have been used as a gravity load resisting system (GLRS) in building. This system should be constructed with lateral force resisting system (LFRS) such as shear walls and moment frames. When lateral loads such as earthquakes occur, LFRS undergo displacement by which connected gravity systems experience lateral displacement. Thus, flat plate system designed as GLRS should be predict unbalanced moments and punching failure due to lateral deformation. Proposed model in this study can predict various failure modes and punching failure of slab-column connections. For verifying the proposed model, the test results of slab-column connection specimens and flat plate specimens having two continuous spans with the difference gravity shear ratio(Vg/Vc) were compared with the results of analysis. The proposed model was predictable the failure modes and punching failures.
하부 철근 유무에 따른 포스트 텐션 플랫 플레이트 골조의 내진성능 평가

본 연구에서는 슬래브 하부철근이 포스트 텐션(PT) 플랫 플레이트 골조의 내진성능에 미치는 영향을 평가하였다. 이를 위하여 슬래브-기둥 접합부에 슬래브 하부철근이 있는 경우와 없는 경우의3층, 9층 골조를 중력하중만 고려하여 설계하였다. 본 연구에서는 대상 건물을 비선형 정적 푸쉬오버 해석하여 기둥을 관통하는 슬래브 하부철근 유무에 따른 전체 구조시스템 거동을 평가하였다. 본 연구에서 사용한 접합부 모델은 뚫림 전단과 파괴메커니즘을 예측할 수 있도록 본 연구자들에 의하여 기존 연구에서 제안된 것이다. 본 연구 결과에 따르면 슬래브 하부철근은 PT 플랫 플레이트 골조의 내진성능에 큰 영향을 미치는 것으로 나타났다. 특히 슬래브 하부 철근이 있는 PT 플랫 플레이트 골조는 슬래브 하부철근이 없는 골조에 비하여 최대 강도와 변형 능력이 크게 향상되었다. This study evaluates the seismic performance of post-tensioned flat plate structures with or without slab bottom reinforcement. For this purpose, 3 and 9 story frames were designed only considering gravity loads. This study conducts a nonlinear static pushover analysis. This study was an analytical model that is able to represent punching shear failure and fracture mechanism. The analytical results showed that the seismic performance of a post-tension flat plate is strongly influenced by the existence of slab bottom reinforcement through column. By placing slab bottom reinforcement in a PT flat plate frame, lateral strength and max drift capacity are significantly increased.

Background: New-onset refractory status epilepticus (NORSE) refers to the newly established concept of a disease characterized by refractory status epilepticus without an identifiable etiology. Recent reports have indicated the importance of immunotherapy for NORSE. Case Report: A 37-year-old man with no past history of epilepsy was admitted for a presenting complaint of confusion. He was treated with acyclovir and anti-epileptic drug (AED) for presumed herpes encephalitis. However, he developed generalized tonic-clonic seizures on day 6 of admission that worsened despite treatment with multiple AEDs. NORSE was considered to be a probable diagnosis and immunotherapy with methylprednisolone and immunoglobulin was scheduled. However, persistent seizure activity was observed on the electroencephalogram after the completion of initial immunotherapy. Subsequently, rituximab was administered for 4 weeks. He eventually regained consciousness and was able to resume social activity. Conclusion: Our patient exhibited a favorable outcome with early-intensive immunotherapy and subsequent rituximab treatment for NORSE.

Genetic prion diseases account for about 10-15% of all cases of human prion disease and are caused by mutations in the prion protein gene. Gerstmann-Sträussler-Scheinker (GSS) disease is a rare genetic prion disease, which is characterized by slowly progressive cerebellar ataxia and the occurrence of cognitive decline in the later stage. P102L is the most common mutation in GSS. We report a patient with a P102L mutation that initially manifested as rapidly progressive dementia without cerebellar symptoms.
