Effect of Low Temperature on Schwann-Like Cell Differentiation of Bone Marrow Mesenchymal Stem Cells

Mesenchymal stem cells (MSCs) are multipotent stromal cells that can differentiate into a variety of cell types. The potential of MSCs for differentiation and proliferation depends on the control of various processes. In this study, we have investigated the effects of low temperature on the Schwann-like cell differentiation of human bone marrow mesenchymal stem cells (BM-MSCs). The growth of BM-MSCs decreased at low temperature, but increased Schwann-like cell differentiation. The expression of cell surface antigen decreased CD73, CD105, and CD90, which are known markers of MSCs at 35°C. Also, the levels of matrix RNA [S100, glial fibrillary acidic protein (GFAP), p75, myelin basic protein, P0, and others] at 35°C in the differentiation culture were higher than those at 37°C in the differentiation culture, and S100, p75, and GFAP proteins were expressed more than at 37°C. GFAP and S100 were stained strongly at 35°C by immunohistochemical examination. These data demonstrate that culturing BM-MSCs at reduced temperature can improve differentiation into Schwann-like cells.

Objectives : We investigated the efficacy and safety of miniscalpel acupuncture (MA) treatment combined with non-steroidal anti-inflammatory drugs (NSAIDs) for chronic neck pain (CNP) in an assessor-blinded randomized controlled pilot trial to establish a basis for larger-scale randomized controlled studies on this subject. Methods : Participants (n=36) were recruited and randomly allocated to the MA group, NSAIDs and combined treatment group. The MA group received MA three times over three weeks. The NSAIDs group was administered orally with zaltoprofen 80mg t.i.d. over three weeks. The combined treatment group received MA and zaltoprofen in the same manner as MA and NSAIDs groups. The primary outcome was pain as assessed by a visual analogue scale (VAS) and the secondary outcomes were assessed using the Neck Disability index (NDI), EuroQol 5-dimension questionnaire (EQ-5D), and Patients’ Global Impression of Change scale (PGIC). Assessments were made at week 0 (baseline), 1, 2, 3 (primary end point) during treatment and at week 7 (4 weeks after the end of treatment). Results : 35 participants completed the study. No serious adverse event occurred and blood test results were within normal limits. The improvement of VAS and NDI was significantly greater in combined and MA group than that in NSAIDs group (p<0.017). The combined group showed better outcomes in EQ-5D at visit 2 and 5, in PGIC at visit 4 than the NSAIDs group (p<0.017). No significant differences were found between combined and MA group. Conclusions : Our results suggest that both combined and MA group can be more effective in improving pain control than NSAIDs group. A large-scale clinical study is warranted to further clarify these findings.
Miniscalpel acupuncture points of the cervical region: A review

Objectives : We conducted a literature search to identify trends with respect to miniscalpel acupuncture points (MAPs) of the cervical region with the aim of providing guidance regarding miniscalpel acupuncture (MA) in the cervical region. Methods : We searched 2 overseas sites (PubMed and Google Scholar) and 5 domestic sites (NDSL, RISS, KISS, OASIS and Korean Traditional Knowledge Portal) for clinical trials of MA of the cervical region. The search terms used were “acupotomy or miniscalpel acupuncture or miniscalpel” and “cervical.” Of 73 studies found, 11 were included in this review, 6 of which presented exact MAPs. Owing to the lack of data, we refer to 1 Korean and 4 Chinese articles to analyze the exact location and meaning of the MAPs used in the published studies and to propose other clinically applicable MAPs. Results : We identified 10 of the most commonly used MAPs in the published studies, including those located below the external occipital protuberance, 2-2.5 cm both side down of the external occipital protuberance, 4.5-5 cm both side down of the external occipital protuberance and C3-7 spinous process. Furthermore, we proposed 13 additional MAPs that can be used clinically. Conclusion : We hope that this study will form a basis for the development of MA and hope to improve the safety and reproducibility in future MA studies of the cervical region.
The Effects of Miniscalpel Acupuncture on Cervicogenic Headache: A report of three cases

Objectives : This study examined the effects of miniscalpel acupuncture (MA) in three patients with cervicogenic headache. Methods : Patients were treated with MA, which was performed once per week for three weeks. A headache score, visual analogue scale (VAS), and neck disability index (NDI) were used for the evaluation of treatment effects. Results : In cases 1, 2, and 3, the headache score decreased from 4 to 1, 3 to 0, and 3 to 1, respectively. The VAS score decreased from 8.5 to 1.9, 5 to 0.4, and 5.3 to 2 in cases 1, 2, and 3, respectively. The NDI score decreased from 28 to 4, 50 to 4, and 38 to 16 in cases 1, 2, and 3, respectively. Conclusion : MA appears to be effective for the treatment of cervicogenic headache. Further data should be collected and a comparative study using other treatment methods should be performed.
