인간 자궁경부암조직에서 세포주기 조절단백 G1 Cyclins의 발현이 임상적 예후인자에 미치는 영향

Alterations in the expression of genes that control the cell cycle may be of critical importance in tumorigenesis and malignant transformation. The major regulatory events leading to cell proliferation occur in G1 phase of cell cycle, and the deregulated expression of G1 cyclins is related to oncogenesis. Cyclins D1 and E play important roles in the progression of cell through G1 phase of the cell cycle. Amplification and/or overexpression of the cyclin D1 gene and aberrant expression of cyclin E has been described in various forms of human cancer. However, the role of cyclins D1 and E in cervical cancer has been poorly defined. In this study, we examined the expression of cyclins D1 and E by Northern blot technique and the status of human papillomavirus(HPV) type 16 and 18 by polymerase chain reaction in 25 cases of cervical carcinoma to explore the relationship between cyclins D1 and E and cervical cancer. We found cyclin D1 expression showed down-regulated expression in cervical cancer but cyclin E expression was increased in cancer group. Other clinicopathological prognostic factors were not correlated with cyclins D1 and E expression. Further study based on larger numbers of cases with correlation of cyclins D1 and E status and survival data will be needed to elucidate the use of cyclin expressions as prognostic factor.

Tamoxifen 사용과 동반된 자궁내막폴립 및 자궁경부폴립 각 1 예

유방암의 치료에 있어서 주 치료는 수술, 항암치료 및 방사선치료등이 있으며 초기 및 진행된 유방암환자에서 주 치료후 보조적 치료수단으로 호르몬요법이 있으며 이 호르몬치료에 사용되는 제재로서는 tamoxifen이 현재 가장 보편적으로 사용되는 약이다. tamoxifen은 체내에서 항에스트로젠 효과를 보여 유방암치료에 도움을 주며 유방암의 재발방지에도 효과가 있다고 알려져 있다. 그러나 tamoxifen의 부작용 또한 여러 가지가 있으며 문헌에 보고된 것으로는 자궁내막폴립, 자궁내막증식증, 자궁내막암등이 있다고 알려져 있다. 본원에서도 역시 유방암으로 수술치료를 시행받은후 보조적 치료로서 호르몬제재인 tamoxifen을 사용하여 왔으며 이러한 환자들은 모두 일반외과와 협의진료하여 산부인과 외래로 추적관찰 해오고 있다. 최근에 본원 산부인과외래로 추적괸찰중인 tamoxifen사용 환자중 자궁내막폴립과 자궁경부폴립이 각1예씩 발견되었으며 이 환자들 모두 tamoxifen의 부작용으로 인해 폴립이 발생한 것으로 사료된다. 자궁내막폴립과 자궁경부폴립은 tamoxifen사용으로 인해 생길수는 있으나 그 발생빈도는 매우 드믄 것으로 보고되고 있다. 이에 본 저자들은 유방암수술후 보조적 치료로 tamoxifen을 사용한 환자들 중 본원 일반외과협의진료를 통해 외래추적관찰도중 우연히 발견된 자궁내막폴립 및 자궁경부폴립, 각1예를 문헌고찰과 함께 보고하는 바이다. Tamoxifen is a nonsteroidal triphenylethylene delivative that has been widely used in the treatment of breast cancer. tamoxifen is indicated for the adjuvant treatment of breast cancer in women following total mastectomy or segmental mastectomy, breast irradiation and chemotherapy. tamoxifen has been shown to have significant benificial effect in the treatment of breast cancer patient as hormonal therapy. However ,there is mounting evidence that tamoxifen may affect other hormon sensitive organs, including the uterus and ovaries. An increased risk of endometrial polyps, endometrial hyperplasia and adenocarcinoma has been reported in tamoxifen treated women. We have met cases of endometrial polyp and endocervical polyp which were associated with tamoxifen use, after modified radical mastectomy for infilterating ductal carcinoma of breast . So we report these cases with the brief review of literatures.
