        CAVI를 이용한 급성기 중풍환자의 Arteriosclerosis와 한방변증의 관련성 연구

        Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.

        뇌졸중 환자의 복부비만에 따른 제특성 연구

        Objectives : This study was aimed to assess characteristics in acute stroke patients according to abdominal obesity. Method : 1,506 subjects were included from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Songpa Oriental Medical Center, Kyungwon University Incheon Oriental Medical Center, and D ongguk University Ilsan Oriental Medical Center from April 2007 to August 2009. Results : 1. Considering the demographic variables of the patients, the gender, older age, diabetes, hyperlipidemia and multiple infarction were significantly higher in the abdominal obesity group than in the control group. 2. Sasang constitution and oriental medical diagnosis showed significant difference in the abdominal obesity group. By oriental medical diagnosis, the D amp ness-Phlegm group was significantly higher in abdominal obesity group than in the control group. According to this analysis, we observed the general disposition of various characteristic distributions according to abdominal obesity in acute stoke patients These results can be utilized in the future as a basis material.

        완전이식 인공심장을 위한 제어시스템의 개발에 관한 연구

        For use in patients with severe forms of heart disease for which no surgical repair is possible, development of artificial hearts has many importance in point of economics, medical and industrial applications. To provide a sufficient cardiac output to the physiological demands of circulatory systems is the objective of control systems for an electromechanical artificial heart, which is based on the stable controller design for the motor in the artificial heart. In this paper, an implantable microcontroller-based brushless DC motor control system with the implantability, reliability, and stability is introduced. The developed control system for the artificial heart has the following advantages: (1) It is possible to be implanted in a body by realizing the fundamental functions such as a motor speed detection, proportional-intergral control, timer, and PWM generation through a software programming. (2) Thus, the power consumed in the controller is reduced. (3) The reliability and stability are improved through the reduction of electronic parts and line connetions at the controller. The performance of the artificial hearts and control system developed was evaluated through a series of mock circulatory experiments and a reliability test for one and half years. A sheep with the artificial heart and control system was survived for three days.

        In this paper, a new biotelemetry system for a transcutaneous data communication between an implanted artificial heart with a control system and an external human-interfaced management system has been developed. A radio telemetry using radio frequency is a commonly used method in the conventional telemetry systems. But, it is not suitable for the medical applications because of not only an interference due to a radio broadcasting but also a harmfulness to the human body. In this paper, therefore, a new biotelemetry system applied to an artificial heart has been developed with the results of the recent research for an optical telemetry system based on the infrared light transmission with good skin permeability. The performance of the biotelemetry system developed has been assessed through mock circulatory experiments, and the clinical applicability has been also confirmed with the successful results in the animal experiments. 본 연구에서는 체내에 이식되는 인공심장 및 제어시스템과 체외의 환자관리시스템간의 정보교환을 위한 무선 통신시스템(Biotelemetry System)을 개발하였다. 일반적으로 무선 telemetry라고 하면 전파 telemetry를 의미할 정도로 전파를 이용한 방식이 보편화되어 있으나, 늘어나는 전파무선기기에 의한 전파장해 및 간섭현상과 무엇보다 인체에 유해한 단점으로 의료용 장비에의 응용에는 부적합하다, 따라서 본 논문에서는 최근 빛을 이용한 무선telemetry에 대한 연구결과를 응용하여 피부투과성이 좋은 근적외광(Infra Red Light)을 이용한 광통신 방식으로 인공심장용 무선정보 전달장치를 구현하였다. 개발된 시스템의 성능은 인공 심장용 모의순환실험을 통해 평가되었으며 아울러 동물실험에서의 성능평가로 임상에의 응용가능성을 확인하였다.

        뇌졸중 환자의 단일 및 다발성 병변군의 특성비교연구

        Objectives : This study aimed to evaluate the characteristics of patients with single infarction and multiple infarctions. Method : We studied inpatients who were admitted from 2005/10/1 to 2007/3/30 at the KyungHee University Oriental Medical Center (KOMC) Department of Cardiovascular & Neurology (stroke center). We sorted small vessel occlusion patients and evaluated general characteristics of the patients along with the characteristics of single and multiple infarction patients. Result : We evaluated 262 inpatients, and did not find any significant difference in age, hypertension, diabetes, hyperlipidemia, diet, exercise, homocysteine, or Sasang constitution between single infarction and multiple infarction. However, there were more significant associations with patients' smoking and stress with multiple infarctions than single infarction. Conclusion : From this study we could roughly grasp the characteristics of Small Vessel Occlusion patients and evaluated the characteristics of patients with multiple infarction. However, due to the special circumstance of the KOMC inpatients it is difficult to generalize our results; further multiple center research with a larger study group is needed.

      • 딥러닝의 SU-net을 이용한 배 화상병 예찰 연구

        화상병(火傷病, Fire blight)은 배와 사과 등 장미과 식물에 생기는 세균성 병해의 일종으로 병원균인 Erwinia amylovora에 의해 발생한다. 국내에서는 2015년 5월 경기도 안성과 충남 천안에서 최초로 화상병이 발생하였으며, 2020년 현재까지 새로운 지역으로 전염되고 있다. 강한 전염성으로 인하여 감염시 2일 이내에 확진여부가 판정되고, 판정 직후 발병된 과수를 중심으로 100 m 반경의 과수를 매몰하기 때문에 화상병발병과수의 충분한 영상취득이 매우 어려운 실정이다. 따라서 지속적인 모니터링을 통해 확산·전염에 의한 2차 피해를 줄일 수 있는 화상병 예찰 시스템에 관한 연구가 필요하다. 본 연구에서는 실제 화상병 발병 과수에서 회전익 드론에 A6300 RGB 카메라를 장착하여 고도 10~12m 상공에서 영상을 취득하였다. 취득된 영상의 공간분해능은 6,000 pixel × 4,000 pixel 이었다. 화상병 검출을 위한 SU-net 구조는 Image input layer, Convolution layer, Batch normal layer, ReLU layer, Max Pooling Layer, Transpose Convolution layer 및 Sigmoid Layer로 구성되어 있다. 특히 화상병 영상 검출의 성능을 향상시키기 위하여 Batch normal layer를 적용하였고, depthwize_conv 결과에 pointwise filter(가중치, 스칼라)를 곱한 후 (1x1 convolution) 다른 필터의 연산 결과와 Sum하는 방법으로 사과 화상병 영상을 분석하였다. 기존 U-net 학습결과는 화상병 감염 과수 영상 가운데 64장이 화상병 감염으로 인식하였고, 16장은 비감염으로 인식하여 예측율은 80.0%로 나타났다. SU-net 학습결과는 화상병 감염 과수 영상 80장 중 70장이 화상병 감염으로 인식하였고, 10장은 비감염으로 인식하여 예측율은 87.5%로 나타났다. 또한 비감염과수 영상 20장의 테스트 결과 19장이 비감염으로 인식하였고, 1장은 감염으로 인식되어 예측율이 95.0%로 나타났으며 개선된 SU-net 학습 알고리즘이 기존의 U-net 학습 알고리즘보다 화상병 예측이 7.5% 증가한 것으로 나타나 성능이 향상된 것으로 판단된다. 따라서 향후 현장에서 드론을 이용한 화상병 예찰에 효과적으로 사용될 수 있을 것으로 판단된다.

      • 항생제로 유발된 Pseudomembranous Colitis환자의 이중탕(理中湯) 치료 1례

        Objective : Stroke patients tend to take antibiotics due to infection resulting from complication. One of many side effects from using antibiotics is diarrhea, which infact causes anaerobic organism dysfunction resulting Pseudo-Membranous Colitis (P.M.C). To treat P.M.C, antibiotics and antibacterial agents are usually used. But they may cause other side effects, therefore extreme caution must be taken. Methods : We treated an antibiotic induced P.M.C patient who diagnosed by sigmoid scopy with only Yijoong-tang (Lizhong-tang) without antibiotics and observed patients stool aspects. Results and Conclusion : Patiensts symptom was improved after 7days, and we can see the improvement by follow-up sigmoid scopy. This case may give us a possibility of that oriental medical treatments as complementary and alternative medicine to standard antibiotic treatment.

      • 교통수단의 발전에 따른 아파트 단지 계획요소 변화 연구

        The purpose of this study is to analyze the process of physical environment of apartment complexes in the metropolitan area in response to development of transportations in the mid-to-late 20th century. The analysis sites are Jamsil Apartment Complex(1974), Dunchon Apartment Complex(1979), Mok-dong New Town(1983), Sanggye New Town(1985), Bundang Newtown(1990) and Ilsan Newtown(1990), which were planned at the turning point of development of transportation and actively reflect the neighborhood unit theory. The analysis was conducted in three stages. Analyzing the status of transportation when the plan was established, deriving the planning elements, and classifying the planning elements into walking, bus, car, and subway which is the representative transportation. The conclusions from the analysis are as follows. First, completing the legal system and expanding the related infrastructure are essential. Second, in contrast to the increase in traffic volume, the pedestrian environment has been continuously improved. Third, despite the emergence of new mode of transportation, there was a tendency to activate the linkage between transportation.
