        국내 5-11세 소아의 코로나19 백신 접종에 대한 델파이 연구

        Purpose: During the coronavirus disease 2019 (COVID-19) pandemic, we conducted a Delphi survey that included the experts from the field of COVID-19 immunization in children aged 5-11 years. The aim was to organize collective expert opinions on COVID-19 vaccination in young children in the Republic of Korea, and so thus assist the vaccination policy. Methods: The panels included pediatric infectious disease specialists, preventive medicine experts, infectious disease physicians, and COVID-19 vaccine experts consulting the Ministry of Health and Welfare. The Delphi survey was conducted online using a questionnaire from February 14 to February 27, 2022. Results: The Delphi panels agreed that children were vulnerable to COVID-19, and the severity of illness was modest. Furthermore the panels reported that children with chronic illness were more susceptible to a worsening clinical course. There were generally positive opinions on the effectiveness of COVID-19 vaccination in children aged 5-11 years, and experts gathered a slightly positive opinion that the adverse events of pediatric COVID-19 were not numerous. The benefits of COVID-19 vaccination were evaluated at a level similar to the potential risks in children. Currently, the only approved mRNA platform vaccine in children seemed to be sustainable; however, the recombinant protein platform COVID-19 vaccines were evaluated as better options. Conclusions: Due to the surge of the Omicron variant and an increase in pediatric cases, the COVID-19 vaccination in young children may have to be considered. Panels had neutral opinions regarding the COVID-19 vaccination in children aged 5-11 years. Thus monitoring of the epidemiology and the data about the safety of COVID-19 vaccination should be continued.

        In Korea, children aged 5 to 11 have been vaccinated against coronavirus disease 2019 (COVID-19) from March 2022. Due to this age group not being at a high risk of developing severe COVID-19 symptoms, each major country has different, circumstance dependent, immunization policies for children. In this review, the COVID-19 vaccination policies for 5-11 years old children in major countries were reviewed and considerations were presented for Korea, where children 5-11 years old are starting to get vaccinated against COVID-19.

        코로나바이러스감염증-19 유행과 교육기관 등교 정상화

        코로나바이러스감염증-19 (coronavirus disease 2019, COVID-19)의 대유행 동안 이에 대응하기 위해 학교 폐쇄가 이루어졌고, 이에 대해 경제학, 사회학, 수학 모델링, 역학 및 공중 보건을 포함한 다양한 학문 분야에서 연구가 진행되고 있다. 인플루엔자 대유행의 모형으로부터 파생된 학교 폐쇄의 COVID-19에 대한 수리 모형은 상충되는 결과들을 보였다. 초기 연구에서는 학교와 지역사회 전반에 걸친 전파를 모델링하여 개학의 위험을 평가했으나, 이 위험이 성인 또는 소아의 전파 중 어떤 측면에 인한 것인지 여부는 여전히 불분명하다. 경험적인 결과들을 종합했을 때, 학교 폐쇄가 COVID-19 유행의 통제에 명확한 효과를 보여주지 못하였으며 이는 지역사회에 따라 COVID-19 유행의 규모가 다르고, 학교 폐쇄 정책이 다양하며, 여러 가지 대응 전략이 함께 사용되었기 때문일 수 있다. 학교 폐쇄의 이익은 불분명하고 정량화하기 어려운 반면, 학교 폐쇄에 따른 교육 격차 및 사회 경제적 부담 등 미래 세대에 치러야 할 잠재적인 사회적 비용이 매우 커질 것이다. 본 종설에서는 COVID-19 대유행에 대응하기 위한 학교 폐쇄의 영향에 대해 정리하고 소아청소년의 건강 영향에 대한 고찰을 통해 교육기관 등교 정상화에 대한 제언을 하고자 하였다. School closures during the coronavirus disease 2019 (COVID-19) pandemic have been outlined in studies from different disciplines, including economics, sociology, mathematical modeling, epidemiology, and public health. In this review, we discuss the implications of school closures in the context of the current COVID-19 pandemic. Modeling studies of the effects of school closures, largely derived from the pandemic influenza model, on severe acute respiratory syndrome coronavirus 2 produced conflicting results. Earlier studies assessed the risk of school reopening by modeling transmission across schools and communities; however, it remains unclear whether the risk is due to increased transmission in adults or children. The empirical findings of the impact of school closures on COVID-19 outbreaks suggest no clear effect, likely because of heterogeneity in community infection pressure, differences in school closure strategies, or the use of multiple interventions. The benefits of school closings are unclear and not readily quantifiable; however, they must be weighed against the potential high social costs, which can also negatively affect the health of this generation.

        중증 열대열 말라리아에 합병한 흑수열 소아 1예

        흑수열은 급성 혈관 내 용혈, 발열과 검붉은색 소변 등의 증상을 보이는 증후군으로, 말라리아 및 불규칙적인 키닌의 투약과 관련 있는 것으로 알려져 있다. 국내에서는 삼일열 말라리아만 발생하고 있으나, 최근 말라리아 유행지역을 여행한 해외여행객들로부터 열대열 말라리아의 해외 유입 보고가 증가하고 있다. 따라서 의료진들은 열대열 말라리아와 그 합병증에 대해 알 필요가 있다. 본 증례는 쇼크를 동반한 중증 열대열 말라리아 이후에 흑수열이 합병한 14세 여자에 대해 보고하는 바이다. Blackwater fever is a serious clinical syndrome manifested by acute intravascular hemolysis, fever, and the passage of black or red urine, which is classically associated with falciparum malaria and irregular administration of quinine. In Korea, Plasmodium vivax is the only endemic malaria circulating; a number of imported cases of falciparum malaria have been reported in patients following return from international travel to a malaria endemic area. Therefore, it is important for health care professionals including pediatricians to be aware of the falciparum malaria and its clinical courses. Herein, we report a case of a 14-year-old girl with severe falciparum malaria that was complicated by blackwater fever.

        2017-2018, 2018-2019 절기 제주 지역 소아 인플루엔자의 역학적 특성

        목적: 제주 지역에서 2017-2018 절기와 2018-2019 절기에 소아 연령에서 인플루엔자의 역학에 대해 알아보고자 하였다. 이에 더하여 국내 인플루엔자 표본감시체계와 비교하여 그 대표성을 확인해보고자 하였다. 방법: 2017-2018, 2018-2019 각각의 절기에 제주대학교 병원에 방문하여 인플루엔자 검사를 시행 받은 13세 미만의 소아를 대상으로 하였다. 의무기록을 통하여 대상 환자들의 인구학적 자료, 인플루엔자 검사 결과를 후향적으로 분석하였다. 결과: 총 5,219명의 인플루엔자 의사환자가 연구 대상자로 포함되었다. 전체적으로 연구 대상자의 평균 나이는 2.85±2.79세였고, 두 절기 모두 인플루엔자 의사환자는 1세 연령 그룹이 가장 많았다. 인플루엔자 감염이 확인된 소아는 총 902 (17.3%)명이었다. 2017-2018 절기에는 인플루엔자 A형의 양성률은 10.4% (236/2,279), 인플루엔자 B형의 양성률은 9.1% (208/2,279)이었다. 2018-2019 절기에는 인플루엔자 A형의 양성률은 10.3% (303/2,940), 인플루엔자 B형의 양성률은 5.3% (155/2,940)이었다. 인플루엔자 환자의 평균 연령은 2017-2018 절기에는 4.09세, 2018-2019 절기에는 5.05세로 통계적으로 유의한 차이를 보였다 (P<0.05). 인플루엔자의 주별 분포는 국내 임상감시시스템의 인플루엔자 의사환자와 유사한 형태로 나타났다. 결론: 제주 지역에서 2017-2018 절기와 2018-2019 절기 사이에 인플루엔자 유행 양상과 연령 분포의 뚜렷한 차이를 보였다. 국내 다른 지역과의 역학적 특성과 비교하여 제주 지역 고유의 인플루엔자 역학에 대한 지속적인 연구가 필요하다. Purpose: In Korea, seasonal influenza is an important respiratory illness afflicting children every year. We aimed to investigate the childhood epidemiology in Jeju during the 2017-2018 and 2018-2019 seasons. Methods: Children aged <13 years, who were tested for influenza at the Jeju National University Hospital during the 2017-2018 and 2018-2019 influenza seasons, were included. Demographics and the influenza test results were retrospectively reviewed from their medical records. Results: This study included 5,219 cases of influenza-like illness (ILI) (2017-2018: n=2,279; 2018-2019: n=2,940). The mean age of the eligible children was 2.85±2.79 years, and the most common age among ILI patients in each season was 1 year group. There were 902 (17.3%, 902/5,219) confirmed influenza cases during the 2 seasons. The rate of influenza confirmed by rapid influenza diagnostic test or polymerase chain reaction among ILI patients in the 2017-2018 and 2018-2019 seasons was 10.4% (236/2,279) and 10.3% (303/2,940) for influenza A, and 9.1% (208/2,279) and 5.3% (155/2,940) for influenza B, respectively. The mean age of influenza-confirmed cases was 4.09 years and 5.05 years in the 2017-2018 and 2018-2019 seasons, respectively (P<0.05). Weekly distribution of influenza was similar to that of ILI in the clinical sentinel surveillance system in both seasons. Conclusion: The difference in the influenza epidemic trend and age-group distribution between the 2017-2018 and 2018-2019 seasons was distinct in Jeju. Steady epidemiological studies on influenza in Jeju are needed for comparison with other regions of Korea.

        5-11세 소아에서 코로나19 백신의 효능 및 안전성에 대한 체계적 문헌고찰

        Purpose: To evaluate the efficacy and safety of coronavirus disease 2019 (COVID-19) vaccines in children aged 5-11 years, a rapid systematic review was conducted on published clinical trials of COVID-19 vaccines and studies that analyzed real-world data on adverse events after COVID-19 vaccination. Methods: A systematic search was conducted on medical literature in international (Ovid-MEDLINE) and pre-published literature databases (medRxiv), followed by handsearching up to January 4, 2022. We used terms including COVID-19, severe acute respiratory syndrome coronavirus 2, and vaccines, and the certainty of evidence was graded using the GRADE approach. Results: A total of 1,675 studies were identified, of which five were finally selected. Among the five studies, four consisted of data from clinical trials of each of the four types of COVID-19 vaccines (BNT162b2, mRNA-1273, CoronaVac, and BBIBP-CorV). The remaining study consisted of real-world data on the safety of the BNT162b2 vaccine in children aged 5-11 years. This systematic review identified that COVID-19 vaccines in recipients aged 5-11 years produced a favorable immune response, and were vaccines were effective against COVID-19. The safety findings for the BNT162b2 vaccine in children and early adolescents aged 5-11 years were similar to those data noted in the clinical trial. Conclusions: There is limited data on COVID-19 vaccines in children aged 5-11 years. Consequently continuous and comprehensive monitoring is necessary for the evaluation of the safety and effectiveness of the COVID-19 vaccines.
