비강내 누낭비강문합술시 시행한 양막충전술의 단기 임상 결과

`Purpose: To evaluate the short-term surgical results of Merocel? nasal packing wrapped with human amniotic membrane on the internal ostium during endonasal dacryocystorhinostomy (DCR). Methods: Sixty patients who underwent endonasal DCR to treat nasolacrimal duct obstruction from October 2005 to April 2006 were enrolled in this prospective study. The patients were divided into two groups: 30 patients treated with Merocel? packing wrapped with amniotic membrane were assigned to group 1, and the remaining 30 patients were assigned to group 2. During the follow-up period, the authors evaluated surgical wound healing, the incidence of granuloma, and the patency of the internal ostium. Results: Success rates at 6 months after surgery were higher in group 1 than in group 2, but these rates showed no statistically significant difference between the two groups (group 1: 93.3%, group 2: 86.7%, p=0.67). The time for mean wound healing in group 1 was significantly shorter than that in group 2 (group 1: 4.8±1.5 weeks, group 2: 7.3±1.5 weeks, p<0.001). The incidence of granuloma in group 1 was lower than that in group 2 at 2 months after surgery (group 1: 13% (4/30), group 2:33% (14/30), p=0.04). However, there was no difference of granuloma formation between the two groups at 6 months postoperatively (group 1: 33% (11/30), group 2: 46% (14/30)). Conclusions: The authors believe that the use of Merocel? packing wrapped with amniotic membrane after endonasal DCR might decrease postoperative inflammation, accelerate wound healing, and decrease granuloma formation in the early postoperative period.`

To investigate the effect of fatigue crack growth due to the surface residual stress, it is measured the residual stress distribution by x-ray diffraction at the crack tip each constant crack growth in the notch specimens, and quantitively assessed the effect of crack closure caused to the distribution of compressive stress at the crack tip from evaluating crack openning stress using the finite element analysis. It is concluded that the degree of the residual stress distribution at the crack tip is decreased with increasing the crack length. From the fact that it is similar to the crack openning stress ratio, it is found that the compressive residual stress distribution and size is related to the crack closure effect and surface residual stress field with propagating crack in the notch specimens depends on the stress intensity factor range at the crack tip.

Purpose: To evaluate the efficacy of internal limiting membrane (ILM) removal during vitrectomy in proliferative diabetic retinopathy (PDR) patients. Methods: Seventy-four eyes of 74 PDR patients who underwent vitrectomy were enrolled. All patients had been followed for at least one year. Thirty-three patients who underwent vitrectomy without ILM peeling from January 2004 to February 2005 were assigned to Group A and 41 patients who underwent vitrectomy with ILM peeling from March 2005 to October 2005 were assigned to Group B. Visual acuity (LogMAR), central macular thickness measured by optical coherent tomography (OCT), and presence or absence of macular edema were compared between the two groups at six months and one year after surgery. Results: Postoperative mean best corrective visual acuity (BCVA; LogMAR) were improved significantly in both groups at six months and one year after surgery. However, the overall amount of improvement in vision was not different between the two groups. The mean central forveal thickness in Group B was significantly thinner than in Group A at six months and one year after surgery. There was no difference between the two groups in macular edema incidence at that time. There were no serious complications in Group B by one year after surgery. Conclusions: There was no difference in visual acuity between the Group A and Group B However, the central forveal thickness measured by OCT was thinner in the ILM peeling group, and there were no serious complications in ILM peeling group. Routine ILM peeling during vitrectomy in PDR will help to maintain the structural stability of the macula postoperatively.

2000년대 국내 항공기 사고·준사고 발생유형 사례연구

Since year 2001 to the present time, the aircraft accidents and serious incidents in our country have surpassed 150 occurrences. The Boeing has published the statistical summary of commercial jet airplane accidents annually for the past 10 years on the basis of the occurrence categories defined by the CICTT(CAST/ICAO Common Taxonomy Team), and the number of occurrences is in order of loss of control(LOC-I), controlled flight into terrain(CFIT) and runway excursion (RE). Like the NTSB and the EASA, when fatal and non-fatal accidents are aggregated, though fatality rate is low, abnormal runway contact(ARC), system/component failure(SCF-PP/NP), ground handling(RAMP) rank high in the CICTT occurrence categories. With the less occurrence frequency, it is difficult to statistically analyze the aircraft accidents in our country, thus customarily the accidents and the serious incidents on aggregate are consolidated, and the statistical analysis is performed. This study categorizes the accidents and serious incidents to the domestic transportation aircraft in the past 10 years according to the CICTT occurrence categories, that is compared with foreign practices, and the implications have been discussed. From years 2001 through 2010, the accidents to the domestic transportation aircraft occurred in order of system failure(SCF-NP), ARC and power plant failure(SCF-PP), and when the accidents and the serious incidents are consolidated and analyzed, it is verified that a distribution appears similar to the European accident occurrence categories defined from 300 accident occurrence data.

청소년 어머니의 정서지능 기반 부모교육 참여경험 내러티브 탐구

목적 본 연구는 청소년 어머니의 정서지능 기반 부모교육에 참여한 실제적 참여경험과 그 경험에 대한 의미를 심층적으로 탐구하기 위한 목적으로 진행되었다. 방법 이를 위하여 2023년 9월부터 11월까지 정서지능 기반 부모교육을 실시하였고 청소년 자녀를 둔 부모참여자 1명을 선정하여 회 대면 면담을 통해 자료를 수집하였다3.연구 참여자의 삶의 맥락 속에서 생생한 경험과 그 의미를 있는 그대로 인정하고 해석할 수 있도록 Clandinin과 Connelly(2007)가 제안한 시간성, 상호성, 상황의 3차원적 내러티브 탐구공간에서 분석되었다. 결과 전사한 자료를 분석한 결과, 내러티브 3개의 경험 영역과 1개의 의미영역으로 15개의 하위범주가 도출되었다. 첫 번째 영역은 참여자가 부모교육에 참여한 동기로 전문가가 되기 위한 간절한 바램‘’허전한 마음으, ‘로부터 편안하고 싶은 열망 이다 두 번째 ’. 영역은 연구 참여자가 과거의 나와 자녀의 만남으로 ‘내처 질까 봐 두려운 나’, ‘어린 시절에 채우고 싶었던 간절한 부모 자리’, ‘부모역할에 최선을 다하다가 스파크가 터지는 순간’, ‘자식을 잘못 키웠다는 소리를 들을까 봐 끊임없이 단속하는 엄마’, ‘쉼 없이 열심히 살아오다가 감정이 휘몰아쳐서 와르르 무너지는 마음’, ‘밖에서는 괜찮은 척 개그맨 짓을 하는 나’, 세 번째 영역은 현재의 나와 자녀의 만남으로 나에게로 향하는 따뜻한 지지와 돌봄의 기쁨‘’깊은 내면의 두려움을 , 기꺼이 체험하는 용기‘’진정한 자기이해로 , ‘성장하는 나 로 자신의 삶을 이야기하고’,다시 이야기 하는 과정에서 자신의 정체성을 새롭게 재구성하였다. 결론 이를 통해 나타난 정서지능 기반 부모교육의 참여 경험 의미는 ‘깊어지고 넓어지는 부모로서의 나’, ‘따뜻하게 살피는 눈으로 나와 타인에 대한 비난을 멈춤’, ‘그토록 바라던 푸근하고 편안한 나의 호호아줌마’, ‘움츠린 어깨를 펴고 당당하게 비상하는 나’ 로 자신과의 진정한 ‘Meet Me’의 시작으로 성장의 시간을 맞이했다. 자녀와의 관계 속에서 일방적으로 통제하고 변화를 요구했던 자신를 인식하고 이해하는 과정에서 내면적 성장으로 연결하였다. 정서지능 부모교육의 참여 경험 의미는 자신과의 진정한 만남으로 자녀와의 관계는 물론 자신의 성장 변화의 변곡점이 되었다. Objectives This study was conducted to explore in-depth the actual participation experience of a mother with adolescents participating in emotional intelligence-based parent education and the meaning of the experiences. Methods To this end, emotional intelligence-based parent education was conducted from September to November 2023, and data were collected through face-to-face interviews three times by selecting one parent participant with adolescents. It was analyzed in the three-dimensional narrative inquiry’s space proposed by Clandinin & Connelly(2000) to interpret vivid experiences and their meanings in the context of the participant's life stories naturally and spontaneously. Results There were 3 experience domains and 1 semantic domains and 15 lower categories. The first domain were ‘desperate desire to become an expert and be comfortable from a empty heart’ as the motivation. The sec-ond domain were ‘I'm afraid I'm going to be hurt’, ‘I'm desperate to fill my childhood parent position’, ‘the moment when a spark bursts while doing my best to play the role of a parent’, ‘mother constantly cracking down on me for fear of being told that I raised my child wrong’, ‘I'm living a hard life without a break and collapsing because of my emotions,’ and ‘I pretend to be okay outside and I'm acting like a comedian.’ The third domain were ‘the joy of warm support and care toward me’, ‘the courage to experience deep inner fears’, and ‘the person who grows into true self-understanding’. Conclusions In the process of recognizing and understanding oneself, who unilaterally controlled and demanded change in relationships with children, it led to inner growth. The meaning of the participation experience in emo-tional intelligence parental education became an inflection point not only for the relationship with the children but also for the change in one's own growth through true meeting with oneself.
