신선 동결 아킬레스 동종건을 이용한 후방십자인대 재건술 후 보행분석

목적: 아킬레스 동종건을 이용한 관절경적 후방십자인대 재건술을 시행한 환자군에 대해 임상적 결과를 평가하고 보행분석을 시행하고자 하였다. 대상 및 방법: 2004년 3월에서 2005년 3월 까지 후방십자인대 손상에 대해 신선 동결 아킬레스 동종건을 이용한 관절경적 재건술을 시행 받은 환자들 중, 저자들이 지시한 재활 프로그램에 순응한 12명의 환자를 대상으로 하였다. 모든 환자에서 수술후 추시 36개월에 임상적 결과 평가와 보행분석을 시행하였다. 임상적 결과는 관절 운동 범위, Lysholm knee score, Tegner Activity Score, IKDC score, 후방 전위 검사, Telos stress arthrometer를 이용한 후방 전위를 측정하여 평가하였고 보행검사는 3차원적 보행분석기를 이용하여 분석하였다. 결과: 모든 임상적 결과 평가 항목에 있어서 술 전과 비교하여 후방십자인대 재건술 후 통계학적으로 유의한 만족할 만한 결과를 보였다. 보행분석 결과 환측과 건측 슬관절에 굴곡각 및 내반각의 약간의 차이를 보였지만 통계적으로 유의성을 보이지 않았으며, 전반적으로 양쪽 슬관절의 유의한 보행의 차이는 관찰되지 않았다. 결론: 후방십자인대 손상 슬관절에서 관절경적 재건술을 시행한 환자 중 재활프로그램에 순응한 환자를 대상으로 평가한 결과 술 전과 비교하여 의미있게 증가된 임상적 결과와 건측과 비교하여 차이를 보이지 않는 보행분석 결과를 얻은바 후방십자인대 재건시 체계적이고 엄격한 재활치료가 중요할 것으로 사료된다. Purpose: The purpose of this study was to analyze the clinical result and the gait patterns for patients who had undergone the arthroscopic posterior cruciate ligament (PCL) reconstruction using Achilles allograft. Materials and Methods: Among the patient who had undergone the arthroscopic PCL reconstruction using fresh-frozen Achilles allograft between March 2004 to March 2005, we selected 12 patients who compliance to our rehabilitation program. Clinical result and gait analysis were carried out at 36 months postoperatively. There were measured by using range of motion (ROM), Lysholm knee score, Tegner activity score, IKDC score, posterior stress test, and posterior translation measured by using Telos stress arthrometer. Kinematic gait analysis was carried out using 3-dimensional gait analysis system. Results: The PCL reconstruction surgery yielded statistacally significant results in all of clinical evaluation. In gait analysis, the average knee flexion angle increased during stance phase and decreased during the swing phase, but was not statistically significant and there were no significant difference between both sides of their knees overall. Conclusion: Since there was no statistically significant difference between the injured and uninjured sides as a result of the gait evaluation of the patients who had arthroscopic reconstruction surgery on their PCL injured knee and conformed to the rehabilitation program, systemic and strict rehabilitation ought to be important in the reconstruction surgery.

In this paper, flow structures in several kinds of DEAS(Diesel Exhaust After-treatment System) were calculated by CFD analysis. When the exhaust gas passed through the bending portion of inlet pipe and diffuser, flow structure was highly distorted by centrifugal force and flow separation for the default system. changing the shape of diffuser reduced the flow resistance of system and installing the artificial impedance in diffuser improved the uniformity of velocity passing through the catalyzed substrate.

A effect of Diesel Fuel Additive on the Performance of Diesel Particulate Filter were investigated in DI injection NA diesel engine. Also, Fuel born catalysts(Type A and Type B FBC) used in this study were Fe based fuel additive in diesel fuel. FBC dosage was fixed a experimental condition(600ppm, 1,200ppm). As the result, Loading time of Type A FBC was shorter than Type B FBC. Also, It is shown that the reaction was more quickly occurred 450℃ condition than 420℃. This difference in reaction rate can be reduced to inject higher concentrations of the fuel additive.

The Experimental Study on the mechanism occurred of white smoke of Diesel Particulate Filter were investigated in DI injection TCI diesel engine. The diesel particulate filter mounted the diesel engine in this study were performed to leave the 10 hours in sub-zero temperatures to same of field conditions. As the result, it is shown that the white smoke was occurred around 50℃before the filter temperature and raised some liquid from the system about 120~130℃. Most of the ingredients in liquids have been formed by water.

경추외상 환자의 진찰에서 모든 부위에 대한 손상가능성을 염두에 두고 면밀한 검사가 필요하다. 의인성 손상을 최소화하기 위하여 환자는 적절하게 후송되어야 하고 응급실에서 철저히 검사를 시행해야 한다. 면밀하게 구성된 치료 원칙으로 치료를 시행함으로서 양호한 결과를 얻을 수 있다. 환자의 상태가 확인되면 치료는 골절의 양상과 신경학적 상태에 따라서 결정한다. 치료의 원칙은 빠른 정복에 의해 더 이상의 신경 손상을 막고 신경 회복을 최대화하며, 변형과 동반된 골절과 탈구를 고정시키는 것이다. 수술시 사용되는 전방이나 후방 접근법은 적절하게 선택된 환자에서 시행된다면 만족할만한 결과를 얻을 수 있다. 수술 후 일정기간 동안 적절한 외고정의 중요성은 아무리 강조해도 지나치지 않다. 정확한 진단과 면밀하고 합리적인 치료를 실시함으로서 치료 후의 합병증을 최소화하고 만족할 만한 기능적 결과를 얻을 수 있다. Historically, injuries of lower cervical spine were associated with a poor prognosis and result in some type of neurologic deficit. But, the incidence of cervical spine injury is increasing because of motor vehicle accidents, followed by falls and diving activities, etc. Proper initial in-field management and early identification and institution of definitive treatmem are crucial to obtaining a favorable prognosis. The Allen and Ferguson classification system divided into six pathogenesis or categories; Compression flexion, vertical compression, distraction flexion, compression extension, distraction extension, and lateral flexion injuries. If allows for a convenient mechanistic description of the biomechanical deficiencies present after cervical injury. Distinguishing between complete and partial spinal cord injuries (SCIs) is important for determining a patient's overall functional prognosis as well as an acute in-hospital treatment plan. Early mismanagement may result in preventable neurologic compromise. The current recommendation for selection of imaging studies put forth by the American College of Radiology are predicated on patient's symptom and an overall suspicion of injury. Plain radiograph can be extremely effective in diagnosing cervical trauma. It consists of three or five-films cervical spine series. A magnetic resonance imaging (MRI) scan is recommended for patients with a neurologic deficit but negative plain radiographic findings. The goals of treatment are to maximize neurologic recovery, to maximize the degree of neurologic and medical comorbidities, to obtain spinal stability, to improve patient functionally, and to help the patient to cope psychologically. In cases of SCIs, high dose each type of fracture have a treatment option which included closed reduction, external stabilization device (cervical and cervicothoracic orthoses, halo vest), and corpectomy and plating or fusion.
