        갓세르 신경절의 고주파 열응고술

        Destruction of the gasserian ganglion can be carried out by creating a radiofrequency lesion under biplanar fluoroscopic guidance, This procedure is reserved for patients who have failed various inter- ventions for intractable trigeminal neuralgia including retro-gasserian injection of glycerol and whose physical status otherwise precludes more invasive neuro-surgical treatments such as microvascular de- compression. Radiofrequency thermocoagulation of the gasserian ganglion provides a safe method of achieving long-standing relief from trigeminal neuralgia with low risk. This technique is currently emerging worldwide as the surgical treatment of choice for trigeminal neuralgia. Recently we performed a successful radiofrequency gasserian ganglionotomy, without any complication, under fluoroscopic gui- dance. The procedure was successful and complete pain relief was achieved for a patient who already had treatments of various interventions including microvascular decompression but never experienced pain relief.

      • The study about metal artifact reduction by using Tin-Filter in CT scan

        목적: 본 논문에서는 CT 검사 시 Tin-Filter 적용에 따라 metal artifact를 감소시킬 수 있는지에 초점을 맞추고 연구를 진행하였다. 대상 및 방법: CT장비는 SIEMENS사의 Somatom Force를 사용하였다. IM Nail을 이용하여 자체 제작한 팬텀과 AAPM 팬텀을 CTDIvol 10 mGy을 기준으로 80 kV부터 150 kV까지 10 kV단위로 스캔하여 Noise와 SNR을 구하였으며 Tin-Filter는 150 kV에 적용되었다. 정밀도를 측정하기 위한 척도로 RMSE 기법이 사용되었고, PH5 팬텀의 Liver nodule, Portal vein 두 부분을 이용하여 리커트 척도로 정성적 평가를 실시하였다. 결과 값들은 통계프로그램 SPSS를 사용하여 분산분석(ANOVA)을 진행하였다. 결과: 자체제작 팬텀 및 AAPM 팬텀을 이용한 Noise 및 SNR 평가에서 kV가 올라감에 따라 Noise가 감소하는 경향을 보였으며 특히 Tin-Filter를 적용했을 때 52.3~79.9%까지 대폭 감소하였다(P<0.01). RMSE값은 140 kV에서 최댓값인 34.27, Tin-Filter가 적용된 Tin150 kV에서 최솟값인 5.95의 값을 보였다. 리커트 척도를 이용한 정성적 평가에서도 kV 그룹 간에는 통계적으로 차이가 없어 영상의 시각적 평가에서는 차이가 없는 것으로 나타났다(p=0.792). 결론: Tin-Filter를 적용하여 검사를 진행하였을 때 선량의 증가 없이 영상의 전체적인 질은 유지하면서 metal artifact를 감소시킬 수 있는 것으로 사료된다. Purpose: In this study, we focused on whether we can reduce the metal artifact by using Tin-Filter in CT scan Somatom Force(SIEMENS,Germany) was used for the CT equipment. Materials & Method: Noise and SNR were obtained by scanning the self-produced phantom using IM Nail and AAPM phantom in units of 10 kV from 80 kV to 150 kV based on CTDIvol 10 (mGy). Tin-Filter was applied to 150 kV.RMSE was used to measure the precision, and qualitative assessment was performed by using the Likert scale on liver noduleand portal vein of PH5 phantom. Result values were analyzed by ANOVA Test using statistical program SPSS. Noise and SNR evaluation using self-produced phantom and AAPM phantom showed a tendency of noise reduction with increasing kV. Result: Especially, when Tin-Filter was applied, it decreased 52.3%~79.9% (P<0.01). The maximum value of RMSE was 34.27 at 140 kV, and the minimum value of RMSE was 5.95 at the Tin150 kV with the Tin-Filter. In the case of qualitative assessment, there was no statistically significant difference in image evaluation (p=0.792). Conclusion: As a result, it is considered that the metal artifact can be reduced without degrading the overall quality of the image and increasing the Dose, if the scan is performed using the Tin-filter.

        Macrolide계 항생물질 동시분석법 확립 및 모니터링

        축산물 중 macrolide계 항생물질 3종을 신속분석하기 위하여 효과적인 전처리법을 설정하고 고속액체크로마토그라피(HPLC)를 이용한 동시분석법을 제시하였다. 대상물질은 tilmicosin, tylosin, spiramycin이며, 확립된 분석법을 이용하여 모니터링을 실시하기 위하여 전국 6개 도시인 서울, 부산, 대전, 인천, 대구, 광주에서 수입원산지가 표시된 쇠고기와 돼지고기 및 그 가공품을 수거하여 분석하였다. 전처리법에 있어서 solid phase extraction(SPE)법에 비하여 액상추출법이 더 높은 회수율을 나타내었으며 전처리 단계도 간단하여 대상 항생물질을 분석하기에 적절하였다. 3종의 항생물질 분리를 위한 컬럼은 C18(250 ㎜×4.6 ㎜, 5 ㎛)을 사용하였으며, HPLC 이동상은 0.025M phosphate buffer(pH 2.5) 및 acetonitrile을 이용한 gradient 조건을 설정하였다. UV 검출파장은 spiramycin 경우 232 ㎚이고, tilmicosin과 tylosin은 288 ㎚을 이용하였다. 평균회수율은 83.0-90.2%이였으며, 검출한계는 각각 7(spiramycin), 12(tilmiconsin), 12(tylosin) ng/g으로 나타났다. 수입 축산물의 항생물질에 대한 안전성을 검토하기 위하여 국내 유통중인 수입축산물 및 그 가공품을 대상으로 하여 모니터링을 실시한 결과, 시료는 전국 6개 대도시에서 126건 구입하였으며 모든 시료에서 macrolide계 항생물질이 검출되지 않았다. In this study, a simple and rapid pre-treatment method based on liquid extraction was applied for the simultaneous determination of three macrolides (spiramycin, tylosin, and tilmicosin) residues. In these studies, the stock farm products was used as a matrix sample. When the liquid extraction method was compared with the solid phase extraction (SPE) method, the former showed higher recovery percentages and simpler steps than the latter. The macrolids were separated using a reverse-phase C18 (250 ㎜×4.6 ㎜, 5 ㎛) column and a gradient elution with mobile phases consisting of phosphate buffer (pH 2.5) and acetonitrile. Tylosin and tilmicosin were detected at 288 ㎚ and spiramycin was detected at 232 ㎚. The average recovery percentage ranged between 83.0-90.2% for samples spiked with the three macrolids at 50 and 100 ng/g The validation results showed that the limit of detection (7 (spiramycin), 12 (tilmiconsin), 12 (tylosin) ng/g)) was under the regulatory tolerances and the linearity from calibration curves was satisfactory for determining the multi-residue of three macrolids in farm products. Monitoring samples were collected at the main cities in Korea as Seoul, Busan, Deajeon, Incheon, Deagu, and Gwangju. Microlide antibiotics were not detected in most samples.

      • 건강 무증상인에서 상부 위장관 내시경 소견

        건강 신체검사를 위해 내원한 73례의 무증상 건강인에서 상부 위장관 내시경검사를 실시하여 23례에서 이상 소견을 관찰하였기에 문헌 고찰과 함께 보고한다. In a prospective study, 73 asymptomatic healthy persons, who were received routine physical examination, 18 to 60 years of age, were screened by esophagogastroduodenoscopy. Twenty-three (31.5%) showed abnormal endoscopic findings. Some volunteers had more than one of lesion or more than one grade of lesion in each anatomic location. In 6 (8.2%) of these subjects the esophagus was a site of lesion, while in 20 (27.4%) the stomach was involved, and in 15 (20.5%) the duodenum was involved. Endoscopic diagnosis of these asymptomatic subjects were 5.5% for erosive esophagitis, 17.8% for erosive gastritis, 1.4% for gastric ulcer, 12.3% for duodenitis and 2.7% for duodenal ulcer.

        Isocapnic Hypoxia로 유발된 호흡촉진 반응에 미치는 Morphine의 영향

        Background: The purpose of this study was to define morphine's effects on resting ventilation and the ventilatory response to hypoxia and hypercarbia. Methods : Six healthy nonsmoking young adult males were tested for the respiratory effects of intravenous morphine (0.15 mg/kg). Test began with baseline measurement of resting ventilation, isocapnic hypoxic ventilatory response (HVR), and normoxic hypercapnic ventilatory response (HCVR). After baseline measurement, morphine was administered and ventilatory responses were determined 20 and 40 min postinfusion. Results : Morphine significantly decreased resting ventilation, hypoxic ventilatory response, and hypercarbic ventilatory response. Resting hypoventilation manifested as a peak rise in PETCO2 from 38.0±1.4 to 42.8±1.0 mmHg ( SEM) at 20 min (p<0.05). Hypoxic ventilatory response, measured as the slope of the ventilatory response to hypoxia, decreased from a control of 20.7±3.8 to 14.5±7.2 at 20 min after morphine (p<0.05). Hypercapnic ventilatory response, measured as the slope of the ventilatory response to hypercarbia, also decreased from 34.9±7.5 to 11.1±4.9 (p<0.05) 20 min after morphine. Conclusion: These decreased responsiveness to the chemical stimuli to breathing may contribute to the ventilatory depression frequently seen after administration of morphine.
