시스틴증은 상염색체 열성으로 발생하는 매우 드문 용해소체 축적병으로서, 용해소체 막을 통한 시스틴 운반의 장애로 발생한다. 용해소체 내 시스틴의 축적과 결정화에 의해 전신의 여러 조직에 파괴가 진행되어 성장 장애, Fanconi 증후군, 신부전, 눈의 이상, 내분비 질환 등이 나타난다. 치료제인 cysteamine의 투여가 예후를 향상시킬 수 있으므로 조기 진단과 치료가 중요하다. 저자들은 성장 장애를 주소로 내원한 12세 남아에서 slit-lamp 검사와 혈액 백혈구 시스틴 농도 측정을 통하여 시스틴증을 확진하고 cysteamine 치료를 시행하면서 추적 관찰 중이다. Cystinosis, an autosomal recessively inherited lysosomal storage disease, results from impaired transport of the amino acid cystine out of cellular lysosomes. The consequent accumulation and crystallization of cystine destroys tissues, causing growth retardation, Fanconi syndrome, renal failure, eye problems, and endocrinopathies. The gene for cystinosis, CTNS, was mapped to chromosome 17p13. The diagnosis of cystinosis was made by measuring the leukocyte cystine content. The presence of typical corneal crystals on slit-lamp examination is also diagnostic. Since treatment with cysteamine has proved extremely effective, early diagnosis and treatment are critical aspects. We experienced a typical case of cystinosis in a 12-year-old boy with growth retardation.

In this study, five cases of post-stroke vision disorders are presented. The patients had no distinctive change in vision disorders before Korean medical treatment, which included herbal medicine, acupuncture, and electroacupuncture. After 11 to 28 days of Korean medical treatment, the patients experienced the first improvement of vision disorders after stroke. Also, patients who received Korean medical treatment after 30 days from onset, the duration in which the most improvement of post-stroke vision disorders occurs, progressed their vision disorders by the start of Korean medical treatment. Korean medical treatment might be effective in treating vision disorders after stroke.

A case of a 75-year-old Korean female with gait disturbance due to Normal pressure hydrocephalus (NPH) is presented. She was treated with acupuncture, electroacupuncture and herbal medicine - 柴苓湯(Shirhyung-Tang, Chai-ling-tang, Sairei-to) We used iNPH grading scale, and specified further the grade of gait disturbance category. After Korean medical treatment, there was notable improvement in gait disturbance on our specified scale. Cognitive impairment, tremor and rigidity were improved on each scale alongside. Korean medical treatment may be effective in treating NPH patients.

Case series of 3 post stroke bed-ridden patients with constipation is presented. They were treated with dietary herbal medicine - 六君子湯(Yukgunja-tang, Liujunzi-tang, Rikkunshi-to) After Yukgunja-tang treatment, there was notable improvement in constipation. Yukgunja-tang treatment may be effective in treating post stroke bed-ridden patients with constipation.

In this study, a 54-year-old female woman diagnosed as Multiple System Atrophy (MSA) presented diplopia with other symptoms - gait disturbance, ataxia, sleep apnea, dysuria. She had been cared with Korean medical treatment - Herbal medicine, acupuncture, bee venom acupuncture, electroacupuncture, cupping, moxa. Notable improvement was observed in diplopia expressing time and Unified Multiple System Atrophy Rating Scale (UMSARS). For MSA patient with no typical treatment indispensable, Korean medical treatment may be effective.

Extraction treatment has been used for a long time to treat crowding or lip protrusion patients and still extraction decision is the most difficult and important decision during diagnosis and treatment planning. If the amount of crowidng is severe, premolar extraction is often considered. Because of their location, premolar extractions would seem to allow for the most straightforward relief of crowding and the improvement of soft tissue profile. But patients and their parents often prefer nonextraction approach if possible and such a preference gives us serious question about the boundary of nonextraction treatment. Because Orthodontic Mini-Implant (OMI) become popular these days, distalization of posterior teeth can be obtained easily without patient’s compliance. For this reason, many orthodontists are trying to treat crowding patient with nonextraction than before. But sometime, unexpected side effects are observed including unesthetic profile, impaction of second molar and long treatment time. All the tools for space gaining – extraction, arch expansion, molar distalization and interproximal enamel reduction - have their limitations and indications. Possible side effects and limitations should be carefully considered during the treatment planning. Although Korean patients usually require extraction more often than US or European patients, more knowledge about the tools for space gaining would help us to decrease the rate of extraction and the problems during treatment of crowding patients.

현재 국내에서는 파리기후협약을 위해 2030년까지 전체 발전량의 20%를 재생에너지로 공급하는 목표를 세웠으며, 신재생에너지 중 1.6%를 해양에너지로 공급하고자 하는 계획을 가지고 있다. 조류발전은 풍력발전보다 에너지 밀도가 높고 출력을 정확하게 예측할 수 있는 장점이 있다. 그러나 조류발전의 경우 풍력발전 대비 냉각방식이 제한적이며, 적절한 냉각이 이루어지지 않을 경우 발전기 성능 저하 및 화재의 위험이 존재한다. 따라서 본 연구에서는 열유체 유동해석을 통해 조류발전기의 냉각시스템에 대한 냉각성능을 분석하였다. 시뮬레이션을 위하여 발전기의 발열량은 안전율 등을 고려하여 발전량의 10%인 130kW로 가정하였다. 발전기 고정자의 냉각에는 고정자 내부에 냉각관을 삽입하는 것으로 설계하였으며, 냉각관의 냉각에는 청수를 사용하며, 청수의 냉각에는 해수를 사용하는 것으로 고려하였다. 시뮬레이션 결과 End Winding의 온도가 86.8℃로 분석되어 목표 유지온도보다 낮은 것을 확인하였으나, 유량의 분배가 원활하지 않아 유체의 압력분포가 일정하지 않음을 알 수 있었다. Currently, in Korea, for the Paris climate agreement, the goal of supplying 20% of the total power generation from renewable energy by 2030 is set, and 1.6% of the renewable energy is to be supplied as marine energy. The tidal power generation using marine energy has a higher energy density than wind power generation and the output is easier to predict. However, in the case of tidal power generation, since the cooling method is limited compared to wind power generation, if proper cooling is not performed, there is a risk of deterioration of generator performance and fire. Therefore, in this study, the cooling performance of the cooling system of the tidal generator was analyzed through thermal fluid flow analysis. The assumptions for the simulation analysis are as follows. The heat generation of the generator was assumed to be 130kW, which is 10% of the power generation in consideration of the safety factor. To cool the generator stator, a cooling tube is inserted inside the stator. Freshwater was used to cooling the cooling tube, and seawater was used to cool the freshwater. As a result of the simulation, the temperature of the end winding was observed to be 86.8℃, and it was confirmed that it was lower than the target maintenance temperature.
