목적: 경추부 및 요추부 신경근증과 혼동을 일으킬 수 있는 대상 포진의 임상 양상에 대하여 기술하고자 한다. 대상 및 방법: 대상 포진과 관련된 상지 또는 하지의 통증을 주소로 내원하였던 7명의 환자를 대상으로 의무 기록 및 검사 결과를 후향적으로 검토하였다. 결과: 면역기능저하 환자로서 포진후 신경통 또는 대상 포진 척수염의 후유증이 의심되었던 1예를 제외한 모든 환자가 갑자기 발생한 심한 초기 통증을 호소하였으며, 전구 증상 발생 후 평균 11일만에 전형적인 피부 병변이 나타났다. 3예에서 초진 당시 피부 병변이 나타나지 않은 상태였고, 요추 또는 경추의 자기 공명 영상을 시행한 5예 중 3예에서 환자의 증상에 합당한 소견을 보여 진단에 혼동을 초래할 가능성이 있었다. 대부분 항바이러스제를 포함한 보존적인 치료로 증상이 호전되었으며, 선택적 신경근 차단술을 시행한 5예 중 4예에서 50% 이상의 통증 완화 효과가 있었다. 결론: 대상 포진에 의한 상지 또는 하지의 통증을 호소하는 환자에서 전형적인 피부 병변이 나타나지 않을 경우 신경근증과 혼동을 일으킬 가능성이 높다. 철저한 병력 청취와 신체 검사를 통해 이를 조기에 진단하고 항바이러스제를 투여함으로써 증상 기간을 줄이고 각종 합병증을 예방할 수 있으며, 오진에 의해 수술 등 불필요한 치료가 시행되는 것을 방지할 수 있다. Purpose: To describe the clinical features of herpes zoster that can be easily misdiagnosed as cervical or lumbar radiculopathy. Materials and Methods: We retrospectively reviewed the medical records and diagnostic studies of 7 patients with herpes zoster-related arm or leg pain. Results: Except one immunocompromised patient with suspected postherpetic neuralgia or sequelae of herpetic myelitis, the other 6 patients with herpes zoster complained of very severe initial pain of sudden onset. Three patients did not show skin lesions on initial examination, and 2 of 5 patients with an MRI of the cervical or lumbar spine had findings consistent with their arm or leg pain and may have been confused with radiculopathy. Conservative treatment, including antiviral agents, improved the symptoms of all patients except the immunocompromised one. Selective nerve root blocks of the corresponding dermatomes were performed in 5 patients and extremity pain decreased over 50% in 4 of them. Conclusion: Herpes zoster can be confused with radiculopathy in patients with arm or leg pain if the typical skin lesions have not been developed. Unnecessary treatments including surgery should be avoided by early, correct diagnosis through prudent history taking and physical examination.

목적: 후전방 C형 투시 장치 회전 방법을 이용한 척추 측만증의 수술적 치료에서 흉추 척추경 나사못 삽입의 실용적이고 안전하며 정확한 방법을 알아보고 소개하고자 하였다. 대상 및 방법: 한 명의 숙련된 척추외과 의사가 후전방 C형 투시 장치 회전 방법을 이용하여 45명의 흉추부 만곡이 주만곡인 측만증 환자에서 611개의 흉추 척추경 나사못을 삽입하였다. 흉추 척추경 나사못의 정확한 위치를 알기 위해 술후 컴퓨터 단층 촬영을 시행하였다. 결과: 삽입된 611개의 나사못 중 10개의 나사못(1.6%)이 내측 피질을 천공하였고, 56개의 나사못(9.2%)이 외측 피질을 천공하였다. 21개의 나사못(3.4%)이 추체의 전방 피질을 관통하였고, 시상면 상에서 척추경의 상하 피질을 천공한 경우는 없었다. 신경 혈관계의 합병증이나 나사못을 재삽입해야 하는 경우도 없었다. 결론: 척추 측만증 환자에서 후전방 C형 투시 장치 회전 방법을 이용한 흉추 척추경 나사못 삽입 방법은 회전 변형이 있는 추체에서 회전 변형에 맞춰 C형 투시 장치를 회전시켜 양측 척추경이 대칭으로 보이는 진성 전후면 영상에서 나사못을 삽입하기 때문에 배우기 쉽고, 간편하게 적용할 수 있으며, 안정성이 높은 방법이라고 판단된다. Purpose: This study evaluated a practical, safe and accurate method of thoracic pedicle screw insertion for the surgical treatment of scoliosis using the posteroanterior C-arm fluoroscopy rotation method. Materials and Methods: A total of 611 thoracic pedicle screws were inserted in 45 patients using the posteroanterior (PA) C-arm rotation method. CT scans were taken postoperatively in the transand sagittal sections to evaluate the pedicle screw placement. Results: A mean preoperative curve of 57.7° was corrected to 17.1° (range, 3°-45°) in the coronal plane. The postoperative CT scans revealed that 10 screws (1.6%) had penetrated the medial cortex by a mean distance of 3.0 ㎜ and 56 screws (9.2%) penetrated the lateral cortex by a mean distance of 3.5 ㎜. No screw penetrated the inferior or superior cortex in the sagittal plane. However, 21 screws (3.4%) penetrated the anterior cortex. No neurological or vascular complications were encountered, and none of the screws required replacement. Conclusion: Thoracic pedicle screw insertion in scoliosis patients using the posteroanterior C-arm rotation method is a practical, simple and safe technique that allows the en face visualization of both pedicles by rotating the C-arm to compensate for the rotational deformity.

The correct romanization of Korean is very important in the strategy of globalization. But in reality there is much confusion involved in regard to romanizing personal names, the names of corporations and many other proper names. Many people do not seem to realize that there are rules and regulations. But they know too well that their knowledge on romanization is not accurate. This is all because the proper education of romanization is not provided at school. To settle down the correct romanization, the education of romanization should be included in the school curriculum.
