장기적 경구 스테로이드 복용이 녹내장 및 고안압증에 미치는 영향

목적: 경구 스테로이드를 장기적으로 복용하는 환자들에서 녹내장 혹은 고안압증과 관련된 유의한 위험인자를 탐색하고 위험인자를가진 환자들의 안압 관리에 도움이 되고자 한다. 대상과 방법: 2020년 11월부터 2021년 2월까지 경구 스테로이드를 6개월 이상 장기간 복용하고 있는 690명의 환자들을 대상으로의무기록을 단면적으로 분석하였다. 기본 인구학적 정보 및 안압검사, 약제의 종류, 용량, 복용한 기간, 안과적 과거력과 안압하강제사용력을 확인하였고 녹내장 혹은 고안압증과 관련된 위험인자를 분석하였다. 결과: 녹내장 혹은 고안압증으로 진단된 안과 그렇지 않은 안을 대상으로 진행한 일반화 선형모형분석에서 경구 스테로이드의 종류, 나이, 복용한 기간은 두 군에서 차이가 없었으나, 용량은 통계적으로 유의하였다(odds ratio, 1.09; p=0.0294). 결론: 경구 스테로이드를 장기적으로 복용 시 종류, 나이, 복용한 기간에 따른 녹내장 혹은 고안압증의 유병률 차이는 없었다. 그러나경구 스테로이드 복용 용량이 높은 환자들의 안에서 녹내장 및 고안압증의 유병률이 상승된 것으로 조사되어, 가능한 저용량으로복용하는 것이 안압 관리에 있어 보다 더 유리할 것으로 생각된다. Purpose: The aim of this study was to evaluate the risk factors associated with glaucoma or ocular hypertension (OHT) in patients taking oral corticosteroids for extended periods, and to aid in managing intraocular pressure (IOP) in patients with these risk factors. Methods: A cross-sectional study was performed involving 690 patients who visited a tertiary referral hospital and had been using oral corticosteroids for more than six months. Patients' demographics, tonometry results, drug type, dosage, duration, ophthalmic history, and the use of glaucoma eye drops were analyzed to determine the risk factors associated with glaucoma or OHT. Results: In a generalized linear model analysis comparing patients' eyes diagnosed with glaucoma or ocular hypertension to those without such diagnoses, no statistical difference was observed between the two groups in terms of drug type, age, and duration of oral corticosteroid use. However, the dosage was found to be statistically significant (odds ratio 1.09, p = 0.0294). Conclusions: No difference in the incidence of glaucoma or OHT was found based on the type of oral steroid, age, or duration of use. However, a higher incidence of glaucoma and OHT was observed among patients taking higher doses of oral steroids. Therefore, it is suggested that using lower doses of oral steroids may be more beneficial for managing IOP.

The purpose of this paper is to explain the validity rules of the categorical syllogism presented in Aristotelian Logic in the 19th and 20th centuries and to introduce the validity test techniques. First of all, I explained what the word ‘distributed terms’ or ‘undistributed terms’ means. Next, I examined Geach's view and Parsons’ view about the origin of the distribution theory. Geach claims that the concept of ‘distributed terms’ is the result of the use of the concept of ‘terms in the distribution assumption’ in the late Middle Ages, and Parsons claims that the concept of ‘distributed terms’ grew directly in the study of Aristotle's On Interpretation. Subsequently, I explained the basis for the existence of each rule by summarizing the validity rules of the categorical syllogism into affirmation-negation rules and distributed-undistributed term rules. Lastly I introduced techniques for testing validity of categorical syllogism, applying the validity rules to a given syllogism. 본 논문의 목적은 19세기와 20세기의 아리스토텔레스적 논리학에서 제시된 정언삼단논법의 타당성 규칙을 설명하고 타당성 검사 테크닉을 소개하는 것이다. 필자는 우선 ‘주연혹은 부주연의 지위에 있는 명사’라는 말이 무엇을 의미하는지를 설명하였다. 다음으로 필자는 ‘주연의 지위에 있는 명사’라는 개념이 중세 후기의 ‘분포 상정’ 이론에서 등장하는 ' 분포를 상정하고 있는 명사‘라는 개념의 활용에서 나온 결과물이라고 주장하는 기치의 견해와 ‘주연의 지위에 있는 명사’라는 개념은 아리스토텔레스의 <명제 해석론>의 연구에서직접적으로 성장했으며, 분포 상정 연구의 발전과는 독립적으로 (혹은 분포 상정에 관한연구가 시작되기 전에) 생겨났다고 주장하는 파슨즈의 견해를 살펴보았다. 이어서 필자는정언삼단논법의 타당성 규칙들을 긍정-부정 관련 규칙과 주연-부주연 관련 규칙으로 요약하면서 각 규칙의 존립 근거를 설명하였다. 마지막으로 필자는 정언삼단논법의 타당성규칙을 구체적인 사례에 적용하는 테크닉을 소개하였다.

In this study, other than the approach that is mainly accessed in previous studies as a main factor in determining the value of the logistics center, 251 logistics centers in the metropolitan area have been examined after categorizing them into the characteristics of logistics center buildings and facilities and management characteristics, such as economic considerations in terms of operations management. The main results of the analysis are as follows: First, it is confirmed that the logistics center has significant influence on the rent decision in addition to the municipal characteristics of the logistics center, logistics facilities, and logistics facility characteristics. Second, only the building area and the floor area ratio have a significant effect on the rent variable, which is a dependent variable. The above-mentioned results show that the complex and the multifunctional center change in the capital region, and in the logistics real estate. The value of the logistics center, which has been developed and invested, is simply approached in the past by expanding the viewpoint from the point of view of the warehouse to the building property, facility property, and property management characteristics. We expect to present the possibility of access from a more comprehensive viewpoint. 최근 수도권 중심으로 복합적이고 다기능 중심적인 개발 및 시설 투자가 이루어지고 있다. 이에 본 연구에서는 임대료 결정요인을 종합적인 관점에서 접근하고자 입지적 특성 외에도 그 관점을 건물적 특성, 물류시설적 특성과 부동산관리 특성 관점에서 미치는 관계를 파악하고자 하였다. 특히, 본 연구에서는 표본 선정의 제한과 측정 변인의 계량화 과정에서 고려가 미흡했던 수도권 물류센터의 특성을 반영하고자 하였다. 이를 위해 선행연구에서 측정된 주요 변인에 대한 고찰과 함께 수도권 전역의 분포된 물류센터 251개를 대상으로 실증검증하였다. 주요 분석결과, 물류센터의 입지적 특성 외에도 경제적 및 시설적 관점에서 건물적 특성, 물류 시설적 특성도 물류센터 임대료 결정에 유의한 영향을 미치고 있음을 확인하였다. 특히, 건물적 특성변수에서는 건축면적과 용적률 변수만이 종속변수인 임대료 변수에 유의한 영향을 미치는 것으로 나타났다. 마지막으로 최근 복합적이고 다기능 중심으로 개발 및 투자가 확대되는 수도권 물류센터 특성을 고려한 변인의 측정 필요성과 그 가능성을 확인할 수 있었다. 또한 고속도로와 높은 연계성과 수도권 전역으로의 물류배송 특성을 반영한 변수의 고찰(배후지와의 거리) 유의성 검증, 연구대상의 대표성은 향후 관련 연구에 하나의 시사점을 제시할 수 있을 것으로 보인다.
