        재활.물리치료서비스 수가항목의 인적 투입요소 구조분석 연구

        Fees for medical insurance services in Korea has not being been set based on service costing. Recently in the USA, fees for physican services are determined by developing and applying Resoure Based Relative Value Scales (RBRVS). This study attempts to develop relative value scales for personal factors of rehabilitative and physical therapeutic services. The personal factors were classified into four categories as having been done in the USA;service time, treatment technology and physical efforts, mental efforts and judgement, and stress. Input factors were measured using Magnitude Estimation Method (MEM), and relative value units were calculated for each of twenty eight rehabilitative and physical therapeutic services. Results show that service time surveyed differs from that provided in the public fee schedules in 24 services; the three personal factors but the service time are highly correlated; the physical therapists hold treatment technology and physical efforts to be the most important factor in setting the for services; and that relative values developed for noninsurance services such as Silver Spike Electrode (SSP) and Proprioceptive Neuromuscular Facilitation (PNF) are higher than those of similar insurance services. The policy implications and measures for improvement for the above findings were suggested respectively.

        병원에 종사하는 의료기사 직종의 전문직업성과 조직성과

        본 연구는 35개 병원급 이상 의료기관에 종사하는 물리치료사, 방사선사, 임상병리사, 작업치료사 437명을 대상으로 전문직업성이 조직성과에 어떠한 영향을 미치는지 살펴보고자 하였다. 분석결과 첫째, 의료기사 직종 종사자들의 전문직업성은 직업에 대한 가치 구성변수 중 직무수행능력과 직무의미성?보수의 적절성?직무자율성이 성별과 연령대에 따라 유의한 차이가 있었고, 직무에 대한 영향력은 연령에 따라서만 유의한 차이가 있었다. 둘째, 조직성과 측정변수인 직장에 대한 애착에 가장 큰 영향력을 미치는 변수는 직업에 대한 가치 변수 중 보수의 적절성이었다. 셋째, 직무특성 중 현 병원 근무년수가 높을수록 직장에 대한 애착이 높다는 것을 알 수 있었다. 넷째, 직종별로 살펴본 결과, 의료기사 직종의 조직성과(직장에 대한 애착)에 영향을 미치는 전문직업성 변수들은 의료기사 직종에 따라 차이가 있었다. The objective of this research is to find the influence of professionalism on the organizational performance, where 437 people of physical therapist, radiological-technologist, medical laboratory technologist and occupational therapist working for 35 medical service utilizations were included. The professionalism employed for medical technicians has shown significant difference on the duty performance, job meaning, appropriate salary and job autonomy with sex and ages among the value variables for jobs, and the influence of duty has shown the significant difference only on the ages. Second, the appropriate salary among the variable values for jobs has the most significant influence on the systemicity and the affections to jobs of measurable values. Third, it has shown that the higher years at work among the duty special quality have strong affection to workplace. Fourth, from the result of occupational category, the professional values affecting the organizational performance of medical technicians have difference on the type of medical technicians.

        요양보호사의 직무스트레스와 직무만족 간의 관계

        본 연구는 노인장기요양보험법에 의해 부산지역에서 노인요양과 관련된 기관에서 노인요양 업무를 수행하고 있는 요양보호사 355명을 대상으로 하였다. 분석방법은 소속기관에 따른 요양보호사의 직무스트레스, 스트레스 대처경험과 대처노력, 그리고 직무만족의 차이를 살펴보기 위하여 t-test를 실시하였으며, 소속기관에 따라 요양보호사의 직무만족에 영향을 미치는 요인들을 분석하기 위하여 회귀분석을 실시하였다. 본 연구의 분석 결과를 요약하면 다음과 같다. 첫째, 요양보호사의 직무스트레스는 요양병원 요양보호사가 요양시설 요양보호사 보다 적게 받고 있는 것으로 소속기관에 따라 유의한 차이가 있었으며, 요양보호사의 직무만족도는 요양시설에 요양보호사가 요양병원 요양보호사보다 높은 것으로 소속기관에 따라 유의한 차이가 있었다. 둘째, 요양보호사의 스트레스 대처경험과 대처노력은 소속기관에 따라 유의한 차이가 있었다. 셋째, 요양보호사의 직무만족 영향요인은, 요양병원 요양보호사의 경우 고용형태ㆍ업무 스트레스ㆍ환자와의 스트레스 요인이었으며, 요양시설 요양보호사의 경우는 평균임금ㆍ고용형태ㆍ관리환자 수ㆍ업무 스트레스ㆍ근무조건 스트레스ㆍ환자와의 스트레스이었다. 그러나 소속기관 관계없이 공통적인 영향요인은 고용형태가 정규직인 경우, 업무 스트레스가 낮을수록, 환자와의 스트레스가 높을수록 직무만족이 높다는 것이었다. The research on the analysis of relationship between job stress and job satisfaction has been conducted targeting 355 Geriatric Care Helpers who are caring for the elderly people in the geriatric care centers in the area of Busan by the Act of Long-term Care Insurance for the Geriatric. As the method for the analysis t-test was conducted to investigate their job stress, counter-stress experience and effort, and job satisfaction. Also regression analysis was conducted to analyse the factors having effect on the job satisfaction of geriatric care helpers. The results of analysis can be summarized as following. First, job stress of geriatric care helpers at hospitals has shown significant difference from that at geriatric care centers, while job satisfaction of geriatric care helpers at care centers has shown some difference from that at hospitals. Second, measurement experience and effort of job stress was shown to some difference according to each institution. Third, the effect factors on geriatric care helpers were employment type, job stress, stress with patients in the case of hospitals, while those on geriatric care helpers were average wage, employment type, the number of charging patients, job stress, job requirement stress, and stress with patients. However, regardless of the type of employed institution, job satisfaction was shown high in case of full time job, low job stress, and high stress with patients.

        대학병원과 중소병원간의 협력관계에 관한 연구

        본 연구는 대학병원과 중소병원간의 협력관계의 현황과 내용을 알아보고, 협력관계의 효과와 개선방안등을 확인하여 병원간 협력체계를 활성화 할 수 있는 현실성 있는 방안을 제시 하고자 한다. 연구의 주요결과를 보면 협력관계 체결의 주 이유는 병원의 인지도 및 브랜드 가치 때문이라는 의견이 가장 높았으며, 협력의 내용은 환자 진료 의뢰 및 회송이라는 응답이 가장 높게 나타났다. 협력의 내용 중 환자를 의뢰하는 이유는 응급상황의 경우, 회송하는 이유는 필요한 진료 완료의 경우가 가장 많았으며, 만족도는 보통으로 나타났다. 협력관계 효과 부분은 대외적인 이미지 향상과 위험도 높은 환자의 효과적인 진료가 가능하다는 응답이 보통 수준이었으며, 그 외의 전반적인 분야에서는 효과는 낮게 나타났다. This paper aims at suggesting realistic plans to vitalize the cooperative structures between hospitals by confirming effects and improvements in the cooperative relationships, as well as by identifying the current status and circumstance in the cooperative relationships between a university hospital and hospitals. The major results of this study are as follows; The cooperative relationships are mainly concluded under the highest opinion on the degree of perception and the brand value for hospitals. For details on their cooperation, the request and return on diagnosis and examination for patients were shown with the highest response. The emergency circumstances were the highest for reasons of requests on patients, and the completed cases for the necessary diagnosis and examination for reasons of return on patients were the most cases among details on their cooperation, whereas their satisfaction showed an ordinary level. Regarding the effects of cooperative relationships, responses to the increase of external image and to the possibility of effective diagnosis and examination for patients with higher risk showed an ordinary level, but other effects in the general fields showed lower levels.

        환자평가표에 의한 요양병원 입원 노인들의 일상생활사 의사결정 인식기술 상태와 일상생활수행능력 간의 관계

        본 연구는 2013년 12월 현재 요양병원에 입원생활 중인 노인들 중 환자평가표의 평가구분에서 계속입원중인 환자 평가를 받은 3개월 이상 입원중인 노인들을 대상으로 실질적으로 부양 및 요양간병 시 집중적으로 관심을 가져야 하는 일상생활활동이 무엇인지를 파악해 보고자 하였다. 연구결과, 일상생활사에 관한 의사결정을 할 수 있는 인식기술 상태와 일상생활활동수행능력들 간의 인과관계를 로지스틱 함수를 이용하여 추정한 결과, 일상생활사 의사결정 인식기술 상태가 심하게 손상된 경우에는 노인들에게 실질적으로 부양 및 요양간병 시 ‘식사하기’ 활동과 ‘옮겨 앉기’ 활동에 집중적으로 관심을 가져야 한다는 것을 확인하였다. 또한 ‘식사하기’ 활동과 ‘옮겨 앉기’ 활동 외의 활동들은 일상생활사에 관한 의사결정을 할 수 있는 인식기술 상태보다는 노화나 질병으로 인한 신체적 기능이 떨어져서 나타난 결과로 유추해 볼 필요성이 있을 것이다. 따라서 이 경우에는 부양이나 요양간병 시 도움을 받는 노인들의 인격이나 수치심 등 자존감을 고려하여 수발자들은 행동과 언어에 각별히 신경을 써야 할 것이다. This work has performed to find what activities of daily living are required for the intensive interests when inpatient elderly more than 3 months has been supported and convalescent care, where the inpatient elderly were judged by the inpatient assessment report in the time of December, 2013. According to the estimation with logistic function of the relationship between the state of decision making recognition technology and the Activities of Daily Living(ADL), the intensive cares for the elderly are required in the parameters of Having meal and transferring sitting when they are severed and convalescently cared as the degree of functional independence for ADL are severly proceeded. In addition, the senescence and disease the activities except Having meal and transferring sitting seem to be influenced by the decline of body function more than the state of decision making recognition technology for daily living.

        재활의료서비스 전문성을 위한 물리치료의 직무특성과 임파워먼트 및 조직몰입 간의 관계

        Purpose:This study purposes to suggest some basic data for the efficiency of a hospital management by drawing out the efficient ways to manage manpower to professionalism of rehabilitation service. Methods:The study used the questionnaire written by respondents themselves, and conducted the regression analysis on the relationships among the job characteristics, the empowerment and organizational commitment. Rusults:Among job characteristics, the wage was the influential factor on the empowerment(psychological, structural), and the specialty physical therapist license was the factor affecting on the psychological empowerment. Among the psychological empowerment factors, the higher meaning of his/her existence, the lower self-decision level, the higher structural empowerment are, the higher organizational commitment was. Conclusion:There were not identified any significant job characteristics influencing on the organizational commitment. Among the structural empowerment factors, the apply of job performance was the largest factor affecting on the organizational commitment.

        전기자극이 정상인과 요통환자의 체표면 온도 변화에 미치는 영향

        The purpose of this study is to investigate the relationship between effects of TENS (Transcutaneous Electrical Nerve Stimulation) and IFC(Interferrential Current Therapy) to the change of body surface temperature. Cases are 22 normal persons and 22 patients with low back pain. Digital Infrared Thermal Imaging system was used for the detection of body surface temperature. 50Hz in frequency and 25-35mA in intensity were applied to TENS and IFC, 15 and 10 minutes on each. The results were follows ; 1. TENS and IFC has on effect of decreasing surface temperature, which would be from cardiovascular factors. (P<0.001) 2. The influence of IFC to the body surface temperature is greater than TENS, and it seems to be vasoconstriction of sympathetic activity. 3. There were no significant differences of body surface temperature between the two groups before and after electrical stimulation.

        신이식 100예에서 이식신의 예후에 영향을 미치는 인자

        N/A It has been reported that many factors other than HLA and the mode of immunosuppression influence the results of renal transplantation. The factors are constantly changing with the advances in surgical techniques and introduction of Cyclosporin-A s-A), etc. We analyzed the possible prognostic factors in 100 cases of renal allograft which were performed by the transplantation team of Kosin Medical College from Dec. 1984 to Aug. 1988. Detailed results are presented for the several factors as follows: 1) HLA and the mode of immunosuppression. Excluding 9cases of graft failure due to non-immunological causes, the actuarial graft survival in 3 years was 100% in the E3LA-II) group, 95.1% in the HLA-HID group and 84.6% in the LUR group. In the HLA-HID group, the 3 year graft survival (3YGS) was 96.8% in the Cs A+P treated group and 93.3% in the Aza+P treated group, and the difference in these 2 groups was not significant statistically (p>0.1). Numbers of patients with serum creatinine equal or above 2 mg/dl were 5 of 17cases (29.4%) in the Aza+P treated HLA-HID group, and 8 of 47cases (17.1%) in the Cs-A+P treated HLA-HID group, but the difference was not significant statistically (p>0.1). 2) Pretransplant transfusions. Twenty-seven cases which were transfused with more than 10 units of packed red cells were 100% in 3YGS and the other cases were 97.8% in 3YGS, And there was no significant difference between Aza+P and Cs A+P treated HLA-HID groups in relation to transfusion (p>0.1), 3) Donor and recipient age. The 3YGS in donors older than 50 years and in those 50 years old and under were 93.796 and 94.895, respectively, The percentages of cases with serum creatinine equal or above 2mg/dl were 26. 5% in the older than 50 years group and 12.3% in the other group, but the difference was not significant statistically (p>0.1). Thirteen cases older than 60 years were 10096 in 3YGS. Recipient age had no significant effect on 3YGS between the older than 50 years group and the other group (p>0.1). 4) Donor and recipient sex. The 3YGS was highest (100%) in the male to male group and lowest (87.5%) in the male to female group, but the difference was not significant in these 2 groups (p>0.1). 5) Minor ABO incompatibility. Seventeen cases with minor ABO incompatibility were 100% in 3YGS, and in remaining compatible 74 cases, the 3YGS was 93.4% and there was no significant difference (p>0.1). In summary, the 3YGS was higherst in the HLA-ID group, and there was no signifcant difference in 3YGS between Aza+P and Cs A+P treated HLA-HID groups. And the elder (more than 50 years or 60 years) donor group did not show lower 3YGS than the younger age group despite somewhat worse graft function. There appears to be a minimal effect with mismatch of sex and minor ABO incompatibility. And the fact that there is no significant relation between pretransplant transfusion and 3YGS seems to be due to DST, which was done in all cases except HLA-ID and 2cases of the HLA-HID group.

        응급의료정보센터 직원의 직무인식 및 만족도에 대한 연구

        본 연구는 응급의료정보센터 직원의 직무인식 및 만족도를 조사하여 직원들의 직무만족을 향상시키고, 나아가 응급의료정보센터의 발전에 대한 긍정적인 제안을 위하여 조사되었다. 본 연구의 결과 직원들은 응급환자의 안내 및 상담업무를 가장 중요하고, 잘 수행하고 있다고 응답하였다. 또한 이송중인 응급환자에 대한 응급처치의 지도 및 이송병원의 안내 업무의 난이도가 높다고 조사되었다. 근무 시 업무에 대한 사명감과 직무 만족도는 보통 이상인 편이나, 전체적인 근무조건, 급여만족도는 만족하지 않게 나타났다. 환자 상담시 모두 스트레스를 느끼고 있었고, 특히 환자를 직접 볼 수 없는 상태에서 제한된 정보로 응급실 내원이나 지속관찰 등을 결정해야 하는 면과 근무인력부족에서 스트레스가 많았다. 또한 정보센터의 위상에 대해서는 정보센터의 정체성에 대해 문제가 크다고 생각하고 있었고, 그로 인해 행정기관과 응급의료기관, 119 구급대와의 응급처치지도, 이송정보 제공 및 병원안내와 같은 유기적인 업무협조체계를 구축하는 것이 어렵다고 느끼고 있었다. The research has been investigated to enhance job recognition and satisfaction in Emergency medical information center and to induce some positive suggestions to develop emergency medical information centers in Korea. As a result, the findings found that the tasks which the experts felt are most easily ones are the guidance, counseling, and advice of a patient, while the most difficult tasks are the advice on the first-aid treatment and the guidance of the hospitals. The research has found the findings that most employees feel that they perform their jobs quite well including the guidance and consultation of first-aide patients. They also feel that they are under stress in consulting patients, and in making decisions to visit emergency centers and to observe the patients in limited information without seeing patients in person. They are concerned about the identity of emergency information centers, feeling that it is difficult to construct smoothly the collaboration of such agencies as the administrative, emergency medical centers, 119 resue teams to treat first aide treatments, to provide transportation information, and hospital guidance.
