        위절제술 후 발생한 조기 덤핑증후군에 대하여 한방치료로 호전된 환자 1례 증례보고

        This clinical study describes the effect of Korean medicine as a treatment for early dumping syndrome. The patient was treated with herbal medicine (Sogeonjung-tang and Jeongjeon-gami-ijin-tang), acupuncture, and moxibustion for 34 days. The degree of treatment was measured by the amount of oral intake and the number of vomiting incidents. During the treatment, the patient's symptoms were relieved and the amount of oral intake increased. At first, the patient vomited once or twice per day, and the amount of oral intake was about 600-700 cc. After the treatment, the patient did not vomit and oral intake increased about two fold. This clinical case study shows the positive effect of herbal medicine, acupuncture, and moxibustion on relief of symptoms of early dumping syndrome.

        물순환 기능 향상을 위한 지하개발 평가지표 개발 및 적용 연구– 용인시 구도심과 신도심을 사례로 -

        This study aimed to develop a groundwater soundness index for evaluating underground development from a water cycle perspective, in order to supplement the existing biotop area ratio, which only evaluates water cycle functions from the surface. The development of the groundwater soundness index was carried out through a process of evaluating items related to ground water disturbance caused by underground development, determining whether to apply the evaluation items, categorizing the evaluation items, adding evaluation items for underground development, assessing the possibility of grading, and selecting the final evaluation indicator. The appropriateness of the index was verified by comparing and analyzing the results of evaluating the water cycle function using the biotop area ratio alone and a combination of the biotop area ratio and the groundwater soundness index, after producing both indices for the old and new central districts of Yongin-si, Gyeonggi-do. The results showed that while the former concluded that the water cycle function in the new central districts was superior to that in the old central districts, the latter concluded that the water cycle function in the new central districts was worse than that in the old central districts. Therefore, to improve the water cycle function of the city, the surface and underground water cycle must be balanced, and natural and artificial ground must be properly secured. In addition, ensuring the pitching function of the surface layer is deemed positive. 본 연구는 물순환 측면에서 지하개발을 평가하는 지하 물순환 건전성 지표를 개발하여 지표면 위주로만 물순환 기능을 평가하는 기존 생태면적률을 보완하고자 하였다. 지하 물순환 건전성 지표의 개발과정은 지하개발의 지하수 교란과 관련된 평가항목 검토, 평가항목의 적용 여부 판단, 평가항목 유형화, 지하개발 평가항목 신설, 등급화 가능 여부 검토, 최종 평가지표 선정의 절차로 진행하였다. 지표의 적정성은 용인시 구도심과 신도심을 대상으로 생태면적률과 지하 물순환 건전성지표를 산출한 후 지표 적용 전후 결과를 비교 분석하여 파악하였다. 연구결과, 생태면적률만을 적용했을 시에는 신도심이 구도심에 비해 물순환 기능이 우수하다고 평가되었다. 반면 생태면적률과 지하 물순환 건전성지표를 함께 진행한 물순환 종합평가 시 신도심이 구도심보다 물순환 기능이 열악한 것으로 평가되었다. 따라서 도시의 물순환 기능 향상을 위해서는 지표와 지하의 물순환성이 균형을 이루어야하고, 자연지반과 인공지반을 적절히 확보할 필요가 있다. 더불어 지표면의 투수기능을 확보하는 것이 긍정적일 것으로 판단된다.

        뇌교 출혈 이후 발생한 지연된 중추성 고체온증에 대한 한방치료 1례

        A patient had central fever following pontine hemorrhage. Central hyperthermia caused by stroke is a rare case, and it is difficult to control. There are few case reports about central hyperthermia in Korean medical treatment (KMT). The patient suffered central hyperthermia accompanied by tachycardia, dyspnea, and irritability. However, there was no evidence of infection. Thus, hypnotics, sedatives, and a minor tranquilizer (Lorazepam and Midazolam) was prescribed. Despite a temperature peak of 39.9 ℃, most of the symptoms were alleviated. The patient’s average body temperature was about 37 ℃, which is higher than most people at 36.5 ℃. His symptoms were diagnosed as ascendant hyperactivity of liver Yang (肝陽上亢), and the patient was prescribed Shihogayonggolmoryo-tang. During the 44 days of KMT, there was no change in his average body temperature and no central hyperthermia over 39 ℃. This case report demonstrates the possibility of controlling central hyperthermia caused by pontine hemorrhage using KMT.

      • 도시생태기능 회복을 위한 물순환체계 평가 개선 연구 : 지하개발평가지표 개발과 적용을 통한 구도심과 신도시 비교

        국내의 도시 물순환체계에 대해 공간적으로 평가하는 대표적 지표인 생태면적률은 독일BFF제도로부터 차용되는 과정에서 발생하는 한계를 지닌다. 따라서 현 생태면적률이 지표면 중심으로만 평가되고, 국내에 성행하는 지하개발과 관련된 물순환평가가 부재하다는 문제를 인식하였다. 이에 물순환측면에서 지하개발을 평가하는 지표를 개발하여 지표면 위주로만 물순환 평가가 진행되는 생태면적률을 보완하고자 한다. 본 연구에서 개발한"지하물순환건전성지표"는 연구에서 수행된 지표개발체계를 거쳐 개발되었고, 지표를 구성하는 각 평가항목은 국토지반정보포털, 국가지하수정보센터 등에서 제공하는 지적정보를 융합하여GIS공간분석기법에 적합한 형태로 도면화하였다. 연구방법으로는 기존 물순환체계 평가지표인 생태면적률과 개발된 지표인 지하물순환건전성지표를 GIS도면분석기법을 통해 대상지에 적용 후 분석하였고, 도출된 결과 비교를 통해 지하개발이 물순환기능에 미치는 영향을 평가해보았다. 물순환체계 중 지표면 위주의 평가항목들로 구성된 기존 지표인 생태면적률만을 적용했을 시에는 신도시가 구도심에 비해 물순환기능이 매우 우수하다고 평가되었다. 반면 지표면의 물순환성을 평가하는 생태면적률과 지하개발에 따른 물순환성을 평가하는 지하물순환건전성지표를 함께 진행한 보완된 물순환평가 시 신도시가 구도심보다 물순환기능이 열악할 수 있다고 평가되었다. 이를 통해 고도의 지하개발을 통해 지표면 녹지율을 확보한 신도시가 구도심보다 물순환 측면에서 보다 열악한 도시생태환경을 지닐 수 있다는 결과를 도출하였다. 즉 이와 같은 결과는 기존 물순환평가 방법인 생태면적률이 국내 도시의 물순환기능을 평가하는 통합환경계획지표로써 한계가 있음을 증명하며, 물순환기능을 공간적으로 분석 시, 생태면적률과 지하물순환건전성지표가 함께 진행될 때에 지하개발이 성행하는 국내도시의 물순환기능을 평가할 수 있는 타당한 방법임을 제안한다. The ecological area ratio, a representative indicator of the spatial evaluation of the domestic urban water circulation system, has limitations that arise in the process of borrowing from the German BFF system. Therefore, it was recognized that the current ecological area ratio was evaluated only as the center of the surface, and that there was no water circulation evaluation related to underground development prevalent in Korea. Accordingly, by developing an index that evaluates underground development in terms of water circulation, it is intended to supplement the ecological area rate in which water circulation evaluation is conducted only on the ground surface. The "groundwater circulation soundness indicator" developed in this study was developed through the index development system, and each evaluation item constituting the indicator was drawn in a form suitable for the GIS spatial analysis technique by combining intellectual information provided by the National Geotechnical Information Portal. As a research method, the ecological area ratio, an existing water circulation system evaluation index, and the underground water circulation soundness index, which were developed, were applied to the target site through GIS drawinganalysis, and the results were compared to evaluate the effect of underground development. When only the ecological area ratio, an existing indicator consisting of surface-oriented evaluation items, was applied among the water circulation systems, the new city was evaluated to have a very good water circulation function compared to the old city center. On the other hand, it was evaluated that the water circulation function of the new city could be worse than that of the old city during the complementary water circulation evaluation conducted by the ecological area rate of the surface and the underground water circulation soundness index of underground development. This allows the landscape is highly secure rate of melt the surface through the underground development of new city more poor cities can have the ecological environment in terms of a nasty temper water circulation results. In other words, these results prove that the existing water circulation evaluation method using GIS has limitations as an integrated environmental plan index to evaluated the water circulation function of domestic cities, and suggest that underground development is a valid way to evaluate the water circulation function.

        해면상 혈관종을 동반한 안면신경마비 환자의 한방치험 1례

        Objective: To report the effect of Korean medicine treatment on a facial palsy patient with a cavernous malformation. Methods: The patient was treated with herbal medicine (Jodeung-san, 釣藤散) and acupuncture. The degree of treatment was measured by the Yanagihara-scale and by mobility of the face. Results: During treatment, the patient’s symptoms were relieved and facial movement was improved. Conclusion: This clinical case study showed the effect of herbal medicine and acupuncture on symptoms of facial palsy.

        허혈성 심질환 환자의 두통 및 흉통 한방치험 1례

        Objective: This case study describes the use of traditional Korean medicine to relieve chest pain and headaches in a patient with ischemic heart disease. Methods: The patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and herbal medicines (Gamiondam-tang and Cheonmabanhwa-tang-gagambang). The Numerical Rating Scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain and headaches decreased from 6 to 0. In addition, chest discomfort and palpitations almost subsided. Conclusion: This clinical case study suggested that Gamiondam-tang and Cheonmabanhwa-tang-gagambang could be effective in relieving chest discomfort and headaches.

      • 오령산 투여 및 침치료 후 당뇨병성 신증의 혈청 크레아티닌 수치가 호전된 뇌경색 환자 증례보고

        ■ Objectives This case is to report the effect of Oryeong-san and acupuncture therapy on serum creatinine level of a cerebral infarction patient with diabetic nephropathy. ■ Methods A female Korean patient was treated with Oryeong-san, acupuncture for total 32 days. We observed renal function test, input/output balance, body weight, hand and foot circumferences, and other symptoms like edema, urination, and any adverse event. ■ Results After treatment, serum creatinine level was improved to 2.48mg/dL from 3.45mg/dL at admission, and foot circumference was decreased to 18.5~18.9cm from 22.0~22.5cm without any adverse event. However, we couldn't find any significant differences on input/output balance, body weight, or symptoms of urination. ■ Conclusion This case suggests that Oryeong-san and acupuncture therapy could be effective in improving serum creatinine clearance of cerebral infarction patient with diabetic nephropathy.

        한방치료로 호전된 섬망을 동반한 좌측 중대뇌동맥경색 환자의 치험 1례

        Objectives: The purpose of this case study is to describe the effect of Korean medicine on a patient with delirium caused by a left middle cerebral artery infarct. Methods: The patient was treated with Sopungbosimdodam-tang, acupuncture, and moxibustion. The Korean version of the Delirium Rating Scale (K-DRS), Neelon and Champagne (NEECHAM) Confusion Scale, and Korean Version of the Mini-Mental State Examination (MMSE-K) were used to evaluate the effect of the treatment. Results: After 50 days of treatment with Sopungbosimdodam-tang, the patient's K-DRS score decreased from 16 to 8. The NEECHAM Confusion Scale also improved from 17 to 23, and the MMSE-K score improved from 1 to 7. Conclusion: This clinical study suggested that Korean medicine could contribute greatly to the treatment of delirium caused by a left middle cerebral artery infarct.
