Flabby tissue를 동반하는 골격성 Class III 환자의 양악 총의치 수복 및 적합성/안정성 평가 증례

치조제 흡수 및 광범위한 flabby tissue가 존재하는 Class III 무치악 증례에서 의치의 유지, 지지, 안정을 얻기는 매우 어렵다. 환자는 72세의 남성으로 약 20년전 비의료기관에서 제작된 의치의 잦은 탈락과 잘 씹히지 않는다는 것을 주소로 내원하신 분이다. 본 환자의 임상검사 소견상 상악 전치부에는 심한 flabby tissue 가 존재하였으며, 골격성 Class III에 해당하는 심한 하악 전돌 양상이 관찰되었다. 먼저 임시 의치 제작으로 저작기능을 회복하고 flabby tissue의 무압인상채득을 실시하였으며, 구치부 치아 배열을 고려하여 최종 의치를 제작하였다. 이에 환자의 주소인 의치의 탈락 및 저작시 불편감이 개선되는 양호한 결과를 얻었기에 이를 보고하는 바이다. To obtain denture retention, support, and stability in Class III edentulous cases with flat alveolar ridges and extensive flabby tissue is very difficult. The patient was a 72-year-old male who wore ill-fitting 20 year old dentures made by non-medical institutions. There was flabby tissue on the maxillary anterior ridge. The patient showed Angle Class III skeletal relationship with severe protruded mandible. First, temporary dentures were fabricated to restore the masticatory function, and final dentures were made through non- pressure impression technique and careful the arrangement of the posterior resin teeth. Improvement of the retention and stability of the denture during the occlusal force application is reported.
전통적 방식과 CAD/CAM을 이용한 양악 총의치 수복 증례

자연치아가 모두 상실되면, 저작이나 발음 등의 기능이 불가능할 뿐 아니라 심미성에 있어서도 노인성 안모를 가지게 되어 대단히 비심미적인 외모를 하게 된다. 총의치를 장착함으로써 저작기능도 회복되며, 총의치에 의한 구강 주위조직의 적절한 지지가 이루어짐으로써 심미성의 회복도 가능해진다. CAD/CAM을 이용한 의치는 저장된 데이터를 이용하여 언제든지 신속하게 의치의 재제작이 가능하고, 높은 강도와 밀도를 가지는 의치상이 장점이라고 할 수 있다. 본 증례에서는 최종의치 기공과정에서 악간관계채득 단계까지는 전통적 방식으로 진행한 후, 이후 최종의치 완성 전 기공과정을 전통적 방식과 CAD/CAM을 이용한 방식으로 나누어서 진행하였다. 이후 각각을 평가하여 환자가 만족하는 임상결과를 얻었기에 이를 보고하는 바이다. In edentulous patient, not only mastication and pronunciation but also esthetics are compromised. By restoring an edentulous patient with complete dentures, masticatory function and esthetics can be rehabilitated. The advantages of the dentures using CAD/CAM are that they can be reconstructed ubiquitously with stored data, and the strength and density of CAD/CAM material are clinically acceptable. In this clinical report, interocclusal record was taken in a conventional manner and then two lab processing procedures were performed: conventional method and CAD/CAM method. Two types of processing procedures were compared and evaluated. Fair clinical results were obtained in both ways, satisfying the patient.
