      • 고주파 온열치료시 케모포트의 열적 변화 연구

        목 적 : 고주파 온열치료 시 약물투여 및 혈액채취에 사용되는 케모포트로 인한 열적 변화를 알아보고자 한다. 대상 및 방법 : 전극 크기 20 cm인 고주파 온열치료기(EHY-2000, Oncotherm Kft, Hungary)를 이용하여 현재 본원에서 사용 중인 케모포트 중 재질이 플라스틱인 소재, 티타늄을 둘러싼 에폭시 소재와 티타늄 소재 케모포트를 자체 제작한 직경 20 cm, 높이 20 cm 원통형 한천(Agar)팬텀에 삽입하여 온도를 측정하였다. 온도측정기(TM-100, Oncotherm Kft, Hungary) 그리고 Sim4Life(Ver2.0, ZMT, Zurich, Switzerland) 프로그램을 사용하여 실제 측정된 온도변화와 비교 분석하였다. 측정위치는 전극 중심축 및 중심축 측면 1.5 cm지점에 각각 0 cm(표면), 0.5 cm, 1.8 cm, 2.8 cm 깊이별로 하였다. 측정조건은 온도 $24.5{\sim}25.5^{\circ}C$, 습도 30 ~ 32%, 출력 전력 100 W로 5분 간격으로 총 60분을 측정 하였다. 결 과 : 케모포트 미사용, 플라스틱, 에폭시 그리고 티타늄 케모포트를 사용한 경우의 최고온도는 전극 중심축 2.8 cm 깊이에서 측정값 $39.51^{\circ}C$, $39.11^{\circ}C$, $38.81^{\circ}C$, $40.64^{\circ}C$와 모의실험값 $42.20^{\circ}C$, $41.50^{\circ}C$, $40.70^{\circ}C$, $42.50^{\circ}C$이며, 전극 중심축 1.5 cm 측면 2.8 cm 깊이에서 측정값 $39.51^{\circ}C$, $39.32^{\circ}C$, $39.20^{\circ}C$, $39.46^{\circ}C$와 모의실험값 $42.00^{\circ}C$, $41.80^{\circ}C$, $41.20^{\circ}C$, $42.30^{\circ}C$이다. 고안 및 결론 : 고주파 전자기장에 의한 케모포트 주위의 열적 변화량은 부도체 물질인 플라스틱과 에폭시 소재에서 미사용의 경우보다 낮게 나타났으며, 도체 물질인 티타늄 케모포트에서는 약간의 차이를 보였다. 이는 케모포트내 금속 함유량 및 기하학적 구조에 의한 것과 이용 장비의 낮은 고주파 대역를 사용함에 있는 것으로 여겨진다. 즉 본 연구에 사용된 케모포트는 열 변화가 미미하여 장해를 고려하지 않아도 된다고 사료된다. Purpose : This study evaluate the thermal changes caused by use of the chemoport for drug administration and blood sampling during radiofrequency hyperthermia. Materials and Methods : 20cm size of the electrode radio frequency hyperthermia (EHY-2000, Oncotherm KFT, Hungary) was used. The materials of the chemoport in our hospital from currently being used therapy are plastics, metal-containing epoxy and titanium that were made of the diameter 20 cm, height 20 cm insertion of the self-made cylindrical Agar phantom to measure the temperature. Thermoscope(TM-100, Oncotherm Kft, Hungary) and Sim4Life (Ver2.0, Zurich, Switzerland) was compared to the actual measured temperature. Each of the electrode measurement position is the central axis and the central axis side 1.5 cm, 0 cm(surface), 0.5 cm, 1.8 cm, 2.8 cm in depth was respectively measured. The measured temperature is $24.5{\sim}25.5^{\circ}C$, humidity is 30% ~ 32%. In five-minute intervals to measure the output power of 100W, 60 min. Results : In the electrode central axis 2.8 cm depth, the maximum temperature of the case with the unused of the chemoport, plastic, epoxy and titanium were respectively $39.51^{\circ}C$, $39.11^{\circ}C$, $38.81^{\circ}C$, $40.64^{\circ}C$, simulated experimental data were $42.20^{\circ}C$, $41.50^{\circ}C$, $40.70^{\circ}C$, $42.50^{\circ}C$. And in the central axis electrode side 1.5 cm depth 2.8 cm, mesured data were $39.37^{\circ}C$, $39.32^{\circ}C$, $39.20^{\circ}C$, $39.46^{\circ}C$, the simulated experimental data were $42.00^{\circ}C$, $41.80^{\circ}C$, $41.20^{\circ}C$, $42.30^{\circ}C$. Conclusion : The thermal variations were caused by radiofrequency electromagnetic field surrounding the chemoport showed lower than in the case of unused in non-conductive plastic material and epoxy material, the titanum chemoport that made of conductor materials showed a slight differences. This is due to the metal contents in the chemoport and the geometry of the chemoport. And because it uses a low radio frequency bandwidth of the used equipment. That is, although use of the chemoport in this study do not significantly affect the surrounding tissue. That is, because the thermal change is insignificant, it is suggested that the hazard of the chemoport used in this study doesn't need to be considered.

        장골이식 공여부의 합병증 및 후유증에 관한 후향적 연구

        Introduction: The iliac crest has been the accepted place to obtain bone for reconstruction in oral and maxillofacial surgery. The iliac crest has many advantages because of its accessibility, large amount of cancellous bone, relative ease of bone harvest, possibility of two team approach and ability to close the wound primarily. This study evaluated retrospectively the morbidity of bone harvesting from the anterior iliac crest to provide a logical guide for recognizing the complications and morbidities of an iliac crest bone graft. Materials and Methods: Fifty healthy patients (mean age of 35.5 years; range 7 to 59) underwent iliac crest bone harvesting for a maxillofacial reconstruction from January 2007 to September 2009 at the Department of Oral and Maxillofacial Surgery in Kyungpook National University Hospital. Age, sex, size and kind of grafted bone, duration of pain on donor site, duration of gait disturbance, sensory deficit, scar, contour defect were measured in each patients by retrospective research. Results: The mean duration of pain is 6.7 days, and mean duration of gait disturbance is 7.2 days. Most patients were free from gait disturbances and pain within 2 weeks and there was no correlation between the size of the harvesting block bone and the duration of gait disturbance or pain. However, this study showed that the duration of pain is associated with gait disturbance. In addition, most patients had no complaints regarding their surgical scar and contour defect, and only one patient had permanent impairment of the sensory function. Moreover, an iliac bone graft did not extend the length of hospitalization. Conclusion: This study suggests that split thickness bone harvesting from the inner table of the anterior iliac crest is a well accepted procedure with relatively low morbidity

        알츠하이머병 치매에서 대뇌 아밀로이드 축적 여부에 따른 임상적 특성의 차이

        Objective:The present study investigated the clinical characteristics of Alzheimer’s disease (AD) dementia with low brain amyloid-beta (Aβ-AD) burden comparing with AD dementia with high amyloid-beta burden (Aβ+AD). We also developed a prediction model for the amyloid positivity on 11 C-labelled Pittsburgh Compound B (PiB) positron emission tomography (PET) with distinct clinical variables in AD dementia patients. Methods:Fifty-nine clinically defined AD dementia individuals, who participated in the Korean Brain Aging Study for Early diagnosis and prediction of AD (KBASE) study, were included. All the subjects received comprehensive clinical evaluations and PiB-PET. Based on cerebral PiB retention, all subjects were divided into Aβ+AD (n=47) and Aβ-AD (n=12) subgroups. To develop a prediction model for amyloid positivity, stepwise multiple logistic regression analysis was conducted. Results:When compared to Aβ+AD, Aβ-AD showed older age, later age-at-onset, and lower education. In regard of risk factors for dementia, Aβ-AD had higher frequency of hypertension and diabetes mellitus as well as lower frequency of apolipoprotein E (APOE) ε4 allele. Although there was no between group difference in Clinical Dementia Rating (CDR) or CDR sumof-boxes scores, mini-mental state examination and constructional recall scores were higher for Aβ-AD than Aβ+AD. The final amyloid positivity prediction model included APOE4 genotype, hypertension, and diabetes mellitus. Conclusion:The findings from this study indicated that clinically diagnosed AD dementia may have high possibility of not being pathological AD if they have older age and higher vascular risks, and did not have APOE4 genotype.

      • 소년원생들의 우울증 예측에 기계학습을 적용한 예비연구

        연구목적 : 청소년 범죄에서 정신건강의 문제와 범죄가 관련성이 높다는 것은 잘 알려져 있지만 국내에서는 아동청소년 범죄자들의 정신건강에 관한 연구가 거의 없다. 뿐만 아니라 아동청소년정신건강 영역에서 기계학습을 적용한 연구는 아직 초기단계이다. 본 예비연구에 서는 여자청소년 재소자들에서 정신건강 문제 중 우울증의 예측에 기계학습 알고리즘을 적용하여 적합한 지를 알아보고자 한다. 방 법 : 대상자는 청주소년원에 재소중인 87명의 여자청소년을 대상으로 하였다. 대상자들에게 설문지 패키지 (인구학적 정보, 범죄관련 정보, 자기보고척도 설문지(아동기부정적경험 설문지, 벡우울척도)를 배부하여 정보를 수집하였다. 기계학습 기법을 이용하여 수집된 재소자들의 기본 정보를 바탕으로 우울증을 예측할 수 있는 6개의 모델(Logistic regression, Random forest, Supportive vector machine, Decision tree, Nearest neighbor, Adaboost)을 생성하여 각 모델간의 예측 성능을 비교해 보았다. 결 과 : 대상자를 벡우울척도(절단점 13)로 군을 분류한 결과 정상군 18명(21%)과 우울증군 69명(79%)이었다. 6개 모델의 우울증 예측 정확도는 Logistic regression 81.8%, Random forest 81.8%, Supportive vector machine 68.18%, Decision tree 72.7%, Nearest neighbor 77.3%, Adaboost 63.6%였다. 그중에서 Random forest 모델의 AUC score는 0.75로 다른 모델들과 비교하여 가장 높았다. 결 론 : 본 연구는 재소청소년의 정신건강에 중점을 두어 현황을 파악하고,우울증의 예측에 기계학습 기법을 적용을 하여 높은 정확도를 확인하였다는 점에서 의의가 있다. 또한 취약계층의 정신건강 영역에 기계학습 기법을 적용하여 관리 및 감시에 적용 가능성에 대한 근거를 제공하였다. Objectives : It is well known that mental health problems and crime are highly related to youth crime, but there is little research on the mental health of young offenders in Korea. Furthermore, research on the application of machine learning in the mental health of children and adolescents is still novel. This preliminary study aims to investigate whether it is appropriate to apply machine learning algorithms to predict depression among female adolescent inmates. Methods : The subjects were 87 young females in Cheongju Juvenile Center. A questionnaire was distributed to the subjects to gather their demographic information and crime-related information, as well as their adverse childhood experiences and Beck depression inventory scores using self-reported scale questionnaires. Based on the collected information, six models (logistic regression, random forest, supportive vector machine, decision tree, nearest neighbor, Adaboost) that can predict depression were created to compare the predictive performance between models using machine learning techniques. Results : Results showed that 29 victims (25.7%) met the criteria of PTSD and 19 victims (16.8%) met the rigid criteria of PTSD. But, according to the subscales, 41 victims (36.3%) were diagnosed as PTSD. Victims with PTSD had more serious depression, anxiety, sleep disturbance, anger, social withdrawal and life stresses. Conclusion : This study identified the current mental health status of female inmates with high accuracy by applying machine learn-ing techniques to predict depression. The applicability of machine learning techniques to the management and surveillance of mental health in vulnerable groups was also highlighted.

        우측 후하악부에 발생한 낭포성 활액종의 치험례

        Cystic hygroma, a cystic subtype of lymphangioma, is relatively rare tumor of lymphatic origin. The lesion is a benign, painless, soft, compressible malformation of the lymphatic system. They can arise anywhere along the lymphatic system, however they are usually located in the head and neck regions and in most cases appear by the age of 2 years. The cases in the adult is rarely occurred and a few cases are described in the literature. Surgical excision remains the treatment of choice. But complete extirpation of these lesion is often impossible, because the tumor tends to spread along vital structures. Therefore recurrence rates are accordingly high. This is a case report about 19 year old male patient with cystic hygroma on right retromandibular area. We obtained the successful, functional and esthetic result by surgical excision of the mass. Therefore, we report the case with a review of literatures.
