        가령에 따른 흰쥐 하악과두 섬유층의 미세구조 및 교원원섬유의 변화

        흰쥐에 있어서 가령에 따라 하악과두에 가해지는 기계적 부하의 변화에 따라 하악과두 섬유층의 미세구조적 변화를 관찰, 분석한 결과를 요약하면 다음과 같다. 하악과두 섬유층의 세포는 생후 초기에는 미분화간엽세포 및 섬유모세포로 구성되어 있었다. 가령에 따라 생후 14일, 27일군 등에서는 생후 초기에 비해 교원원섬유를 활발하게 합성, 분비하는 것으로 생각되는 섬유모세포가 발달하였는데 이들 세포는 과립형 물질로 채워져 있는 내강이 대단히 확장되어 있으며, 리보솜이 치밀하게 존재하는 과립세포질세망과 골지장치가 발달되어 있었다. 또한 이들 세포내에는 세포소기관 사이에 세사가 점차로 발달하는 양상을 보였다. 생후 55일군 이후에서는, 가늘고 긴 세포돌기를 가지며, 과립세포질세망 및 골지장치가 발달되어 있어 교원섬유의 합성이 왕성한 것으로 추측되는 세포, 약간 둥글거나 다각형의 모양을 보이며, 세포질내에 사립체, 용해소체 및 골지장치가 발달하였으며, 손상된 교원섬유 혹은 변성된 세포의 흡수에 관여하는 것으로 추측되는 세포, 세포질의 상당부분이 세사로 채워져 있으며, 사립체, 용해소체 및 과립세포질내세망과 같은 세포질내 소기관은 잘 발달되어 있지 않아 가해진 기계적 자극에 대해 적응하였거나 활동이 정지된 세포 등 3종류의 세포가 관찰되었다. 세포간질의 교원원섬유는 생후 초기부터 가령에 따라 점차로 치밀하게 배열되는 양상을 보였다. 교원원섬유의 직경은 생후 1일, 7일군에서는 $38.48{\pm}3.81nm$, $38.06{\pm}3.86nm$로서 유사하였으나 생후 14일군에서는 $50.21{\pm}3.93nm$로 가장 굵은 양상을 보였으며(p<0.05), 그후 생후 27일, 55일군에서는 각기 $40.05{\pm}2.52nm$, $43.63{\pm}1.20nm$로서 점차로 가늘어져 생후 1년군에서는 $37.38{\pm}2.17nm$로서 가장 가늘어 졌다(p<0.05). 그 분포에 있어서는 생후 초기에는 직경이 30-60인 교원원섬유가 대다수를 차지하는 unimordal한 분포를 보였으나, 생후 27일 이후에는 직경이 작은 교원원섬유가 점차로 증가하는 양상을 보였다. 이상의 가령에 따른 결과로 미루어보아 하악과두 섬유층의 미세구조, 교원원섬유의 배열 및 직경의 분포에서의 변화는 가해지는 기계적 부하에 대한 적응한 결과라고 생각된다. The fibrous layer of mandibular condyle of the neonatal, 1-, 7-, 14-, 27-, 55-days and 1 year old rats were examined in the electron microscope with particular attention to the ultrastructure and diameter of collagen fibrils. In the 1-day rats, most of the cells of the fibrous layer were undifferentiated mesenchymal cells and fibroblasts with rough a little developed rough endoplasmic reticulum(RER) and golgi apparatus(GA). In 7-, 17 and 27-days old rats, most of the fibroblast showed well developed GA and RER with widely distended cisternae containing granular materials. In many of these cells contained intracytoplasmic filaments among the cytoplamic organelle. In 55-day and 1-year old rats, three types of cells were observed, ie, cells containing well developed cytoplasmic organelle presumed to be involved in the collagen fibril synthesis, cells showing well developed lysosomes, golgi apparatus, mitochondria and short cytoplasmic process presumed to be involved in the active resorption of the injured collagen fibrils or cellular debris, cells containing many intracytoplasmic filaments and a little organelle presumed to be cells of inactive state. The average diameters of collagen fibrils were similar in 1- and 7-day old rats as $38.48{\pm}3.81nm$, $38.06{\pm}3.86nm$. That was thickest in 14 days old rats as $50.21{\pm}3.93nm$ among experimental groups. They were gradually thinner in 27-, 55-day rats as $40.05{\pm}2.52nm$, $43.63{\pm}1.20nm$ and thinnest in 1-year old rats as $37.38{\pm}2.17nm$. The distribution pattern of diameters of collagen fibrils were unimordal with peak of 30-60nm in rats from 1-day to 17-day old. With aging from 27-day to 1 year old rats, collagen fibril diameters showed wide distribution pattern and percentage of thin collagen fibrils increased. These results may show the functional adaptation of fibrous layer of mandibular condyle to the increased mechanical forces with aging.

        흰쥐의 발치와 재상피화에 따른 상피내 CGRP 면역양성 신경섬유의 분포변화

        The purpose of this study was to investigate the distribution pattern of CGRP immunoreactive nerve fibers in the healing mucosa of extracion wound. Maxillary 1st molars of rats were extracted. All extraction sites and adjacent tissues of 3 groups of rats(1-week, 2-week and 4-week groups) were removed en bloc and processed for immunostaining and were subjected to light microscopic examination. The results obtained were as follows; In 1-week group, there was no difference in the distribution pattern of CGRP immunoreactive nerve fiber between epithelial margin adjacent to extraction socket and normal gingival epithelium. In 2-week group, some CGRP-immunoreactive nerve fibers were seen in epithelial layer. In 4-week group, many intercellular CGRP immunoreactive nerve fibers were abundant in all layers of immature epithelium characterized by scab on the mucosa and thick keratinized cell layer with irregular surface. Intraepithelial CGRP immunoreactive nerve fibers were reduced to normal level in adjacent mature epithelium. These results suggest that density of CGRP immunoreactive nerve fibers are increased transiently in epithelium during reepithelialization process and CGRP released from these nerve fibers may play an important role in the reepithelialization in the wound healing.

        구강저에 발생한 유피낭종

        Dermoid cyst has evolved to represent any cyst filled with sebumlike material and with evidence of specialized skin derivatives from defective embryonic development. Approximately 7% of all dermoid cyst arise in the head and neck region and 0.01% of all oral cysts. The anterior region of the floor of the mouth is the most common site of occurrence in the oral cavity. There is no sex predilection and the age at presentation is usually in the second or third decades. Clinically, the lesions present as a rubbery or doughy mass that may vary in size from a few millimeters to 12 ㎝. An intraoral dermoid cyst usually presents as a slowly growing, painless swelling in the anterior floor of the mouth which may eventually cause elevation and displacement of the tongue resulting in dysphonia, dysphonia and dyspnea. Treatment involves surgical removal and recurrences have not been reported. We report three cases of dermoid cyst that were cared successfully in the department of dentistry, Kyung-pook National University. One case is a sublingual type, another case is a geniohyoid type, and the other is a lateral type. 저자등은 구강저에 발생한 3 증례의 유피낭종 및 유표피낭종을 악설골근을 기준으로 존재하는 위치에 따라 구내 및 구외접근법을 통한 외과적 적출을 시행하여 다른 합병증없이 양호한 결과를 얻었기에 보고하는 바이다.

        하악골에 발생한 양성 섬유성 조직구종의 치험례

        Benign fibrous histiocytoma(BFH) is a mesenchymal cell-originated tumor composed of cells with fibroblastic and histiocytic differentiation. BFH occurs predominantly on sun-exposed skin of extremities. Oral BFH lesions are uncommon. The majority of oral lesions includes the soft tissue but not the jaw bones. The lesion appears as well-defined multilocular radiolucencies associated with bony swelling when it occurs on the jaw. The lesion induces the thinning and expansion of the cortex and shows many thin, indistinct septa in the lesion. Surgical excision is the choice of treatment. The recurrence rate is low and metastasis has not been reported. We report the clinical, radiographic and microscopic findings of a BFH case occurred in the mandible with literature reviews.

        인체 상피세포에서 ras-종양유전자의 발암화가 신호 전달 기작에 미치는 영향

        The present study has attempted to look into the mechanism of ras-induced carcinogenesis in a human epithelial cell system. Human epithelial cells immortalized with Ad12-SV40 hybrid virus were used to assess carcinogenic potential of the ras-oncogene. Cells transfected with pSV2-ras showed characteristics of cellular transformation. The transformation parameters such as cell density, soft-agar colony formation, and cell aggregation were significantly increased in the cells expressing ras oncoprotein. In addition, the duration required for the appearance of foci was shortened in the ras-transfected cells. Consistent with other reports, our results demonstrated an evidence that the ras-oncogene induced the cellular transformation of human epithelial cell system. When a high concentration of glucocorticoid was added into the media, transformation process was accelerated. It is speculated that glucocorticoid may provide an advantageous environment for the proliferation of the transformed cells. The induction of the intracellular free calcium concentrations following agonist treatment was significantly lower in the transformed cells than in the control cells. These effects were more manifested in the presence of extracellular cacium, indicating that the transformation process may alter the influx pathway of extracellular calcium. The induction of IP₃ following agonist treatment was also lower in the transformed cells than in the control cells. Thus, it is suggested that phospholipase C-coupled pathway was down-regulated in the process of the ras-induced transformation. While the levels of TGF-β1 and PAI-2 mRNAs were decreased, the level of fibronectin mRNA was increased. The results indicate that mechanism of the ras-induced transformation may be associated with the altered expressions of growth regulatory factors. The present study demonstrates an evidence that the ras-induced cellular transformation may be associated with alteration of signal transduction and growth regulatory factors. The study will contribute to improve the understanding of molecular mechanism of epithelium-derived cancers including oral cancer.

        DERMOID CYST ON THE FLOOR OF THE MOUTH : A CASE REPORT

        Dermoid cysts located in the floor of the mouth are very rare developmental keratinizing squamous epithelium lined cysts. Anatomically, they are classified as sublingual (median genioglossal), submental (median geniohyoid) and lateral dermoid cysts, and they can be further classified as epidermoid, dermoid, and teratoid cysts by histology. We report a case of sublingual dermoid cyst in a 16-year-old boy presenting as a large sublingual swelling causing speech and swallowing difficulties and discuss the surgical treatment techniques and histopathological features of this lesion.

        쇄골 두개 이형성증 환자의 구강내 증상과 치료-증례 보고

        Cleidocranial dysplasia(CCD) is a rare syndrome usually caused by an autosomal dominant gene or no apparent genetic cause. The skull is large and short with marked bossing of the frontal bone. Closure on the fontanelles and sutures is delayed. The clavicle may be unilaterally or bilaterally aplastic. Oral manifestations include multiple impacted permanent teeth, prolonged retention of primary teeth and multiple supernumerary teeth. There are many difficulties in the early diagnosis of CCD because a majority of the craniofacial abnormalities becomes obvious only during adolescence. Late diagnosis produce occlusional & psychological problem. Therefore early detection and treatment of CCD can reduce the period and the extent of orthodontic and surgical interventions. We experienced CCD patient with multiple supernumerary teeth and will report it with the literature review.

        우측 후하악부에 발생한 낭포성 활액종의 치험례

        Cystic hygroma, a cystic subtype of lymphangioma, is relatively rare tumor of lymphatic origin. The lesion is a benign, painless, soft, compressible malformation of the lymphatic system. They can arise anywhere along the lymphatic system, however they are usually located in the head and neck regions and in most cases appear by the age of 2 years. The cases in the adult is rarely occurred and a few cases are described in the literature. Surgical excision remains the treatment of choice. But complete extirpation of these lesion is often impossible, because the tumor tends to spread along vital structures. Therefore recurrence rates are accordingly high. This is a case report about 19 year old male patient with cystic hygroma on right retromandibular area. We obtained the successful, functional and esthetic result by surgical excision of the mass. Therefore, we report the case with a review of literatures.

        상악골에 발생한 치성 점액종의 치험례

        Odontogenic Myxoma of the jaws is a rare benign tumor derived from embryonic mesenchymal elements of dental anlage. It appears to originate from the dental papilla, follicle or periodontal ligament. Odontogenic Myxoma of the maxilla is less frequent but behaves more aggressively than that of the mandible, because it spreads through the maxillary sinus. Radiographically, the tumors present as unilocular or multilocular radiolucent lesions with well-defined borders with fine, bony trabeculae. On gross examination, the tumor appears as a smooth, glistening, gelatinous, lobulated mass. On microscopic examination, these neoplasms exhibit loose arrangement of stellate-shaped cells. The intercellular substance is a mucinous and homogeneous matrix. We report a case of odontogenic myxoma of the maxilla observed in our clinic with good prognosis after partial maxillectomy

        화학적 발암화에 따른 Protein Kinase C의 발현 변화

        Protein kinase C (PKC) is known to play a pivotal role in neoplastic transformation cells and its high expression is often found in a variety of types of tumors including oral cancer. While PKC is associated with the altered signal transduction pathway of the tumor cells, it is still unclear which isoform is involved in the carcinogenesis process. Since the cellular distributions and the roles of PKC are isoform-specific, it is very important to identify the specific target molecules to improve our understanding of the carcinogenesis processes. Thus, the present study attempted to perform chemical carcinogen-induced neoplastic transformation of human epithelial cells and analyze the specific isoform of PKCs involved in the cellular transformation. The study analyzed overall PKC responses upon MNNG(N-Methyl-N'-nitro-N-nitroso guanidine) exposure with [3H] PDBu binding assay. PKC translocation was observed at high doses of MNNG treatment in the presence of extracellular calcium. Such effects were not observed in the absence of extracellular calcium. Translocational effects with exposure of MNNG was further enhanced in the presence of hydrocortisone. The result suggests that the type of PKC involved may be Ca2+-dependent classical isoform and steroid hormone enhances PKC activation. Among cPKC isoforms examined, only PKC-α and r showed significant translocation of protein levels from cytosolic fraction to membrane fraction, as analyzed by immunoblot. PKC-ε in nPKC class showed an inch·eased translocation, but other forms in this class did not show the effect. None of isoforms in aPKC class was affected by MNNG treat-ment. The study demonstrated that there was a certain specificity in the patterns of isoform induc-tion follwong chemical carcinogen exposure and helped identify all the types of PKC isoforms expressed in human epithelial cells. It was revealed that PKC isoforms were activated in an early resonse to chemical carcinogen, suggesting that PKC be associated with carcinogenesis process from an early stage in this particular cell system. The study will contribute to improving our understanding of chemical-induced carcinogenesis in human cells and may provide a scientific basis to introduce the specific PKC inhibitors as an anticancer drug of epithelial cell-origin cances including oral cancer.
