N/A It was recently observed that the hyperimmune response in vitro as in vivo was always accompanied by high levels of neopterin. Indeed in vitro, the stimulation of lymphocytes or macrophages either with viral alloantigens or with chemically modified autologous cells induces a remarkable release of this substance by means of the gamma interferon induction. In the viral infections the increased levels of neopterin could be due to the direct stimulation by the virus on the nucleic acid of the hosts cell (wacher et al ; 1957) Our aim was to verify if during fatty liver or hepatitis, and in cirrhosis of the liver there could be significant variation of serum neopterin levels. We studied 75 patients suffering from hepatic diseases, ranging between 22 and 84 year (mean age 48.96+14.2) and 20 healthy controls homogeneous for sex and age to our patients. The mean values of serum neopterin were 2.17+0.94 ng/ml in patients and 1.59+0.35 ng/ml in the control group. These data was correlated with the mean values of both SGOT and SGPT that were 152.25+278.61 IU/L, 204+405.94 IU/ respectively, and the statistical study showed positive but low correlation coefficient (SGOT:r = 0.28, p<0.05; SGPT:r = 0.30, p<0,05). There was no significant difference of serum neopterin levels between patients with fatty liver and control group (t = 1.057, p>0.05). However, the statistical study showed a significant difference between hepatitis, cirhosis of the liver and the normal control group(p<0.05). Serum neopterin levels and triglycerides were shown to be the best variables for discriminating between the hepatitis and fatty liver patients, neopterin being the more specific of the two. No significant correlation of serum neopterin levels between B hepatitis and non B hepatopathies could be found.

Although peritonitis is an unusual form of tuberculosis, it is one of the most important diseases of the peritoneum in this country. Chylous ascites is developed by the various etiologies, but it is rarely caused by tuberculosis. We experienced a case with chylous ascites following tuberculous peritonitis with lymphadenitis which had not responded to a longterm antituberculous medication. In the findings of the second laparoscopy after six months` treatment, milky-turbid chylous ascites has still resided in the peritoneal cavity with some nodules newly developed over the hepatic surface. Significantly elevated triglyceride level in ascites was identified. The patient was much improved with conservative treatment. A new tumor-associated glycoprotein (TAG-72) has been shown to be expressed in a wide variety of epithelial malignant tissues. We studied the serum levels of TAG-72 antigen in some malignant tumors of digestive system (n= 44), compared with benign diseases of digestive organ (n=35) and the normal subjects (n=35). The results were as follows; 1) The mean value of serum CA 72-4 was 1.69±0,50 U/㎖ in the normal subjects. 2) There was no significant difference of serum CA 72-4 levels between patients with benign disease of digestive organ and the normal subjects (p$gt;0.05). 3) Levels of serum CA 72-4 in patients with gastric caner or hepatoma appeared significant increase than those of either the normal subjects or benign diseases (p$lt;0.05). 4) There was significantly increased levels of serum Ca 72-4 in patients with advanced gastric cancer than those with early gastric cancer (p$lt;0.05). In gastric cancer, CA 72-4 identified 45.8% of all patients (CA 19-9, 22. 2%. CEA 40.0%), and combination of CA 72-4 and CEA detected as many as 60% (p$lt;0.05).
위전절제술 후 발생한 반응성 저혈당증을 α-Glucosidase Inhibitor로 치료한 1예 : A case report

Gastrectomy or vagotomy may result in reactive hypoglycemia, which, in some cases, can reduce the plasma glucose levels to 30∼40 ㎎/dL due to rapid digestion and absorption of food, especially carbohydrate. We treated the patient with frequent episode of severe hypoglycemia. Reactive hypoglycemia is caused by an excessive insulin secretion after a sharp rise in plasma glucose. He had undergone total gastrectomy due to stomach cancer 4 years before. Since nutritional treatment did not successfully manage his reactive hypoglycemia, an α-glucosidase inhibitor, voglibose was administered. His hypoglycemic symptoms disappeared and a rapid change of plasma glucose and insulin levels were attenuated after the α-glucosidase inhibitor treatment. The effects of α-glucosidase inhibitor were documented in a 5-hour oral glucose tolerance test, where not only insulin and C-peptide rise was clearly attenuated, but also the blood glucose concentration did not fall low enough to induce hypoglycemic symptoms. This therapy was very effective and the patient has not had any recurrence of reactive hypoglycemia since the initiation of the therapy.
