Horner’s syndrome may rarely accompany herpes zoster ophthalmicus (HZO). A 78-year-old woman suffered from HZO accompanied ipsilateral ptosis and miosis. Before skin eruption, she was diagnosed as a paratrigeminal syndrome of Raeder. She was treated with intravenous acyclovir and prednisone for 7 days. Ptosis and miosis was not completely improved after 5 months of follow-up.

한 정신병원에서 발생한 급성 축삭성 길랑-바레 증후군으로 추정되는 환자들에 대한 임상적 연구

Background : Guillain-$Barr{\acute{e}}$ syndrome(GBS) is characterized clinically by acute flaccid paralysis, areflexia, and albumino-cytologic dissociation. Based on electrophysiology and pathology, GBS can be divided into either predominantly demyelinating or predominantly axonal patterns. Objectives : The clinical and laboratory status of probable acute axonal GBS occurring at a mental hospital was evaluated. Methods : Eight schizophrenia patients with probable acute axonal GBS were analyzed. Results : The mean age of the patients was 38 years old. Most of the patients were men. All patients showed an acute ascending paraparesis and/or quadriparesis with areflexia, and all have a history of schizophrenia for 3~20 years. The diseases occurred predominantly in the summer and electrodiagnostic studies revealed axonal patterns. The patients were treated by supportive care, except one patient with intravenous immunoglobulin. The prognosis was improved in 3 ; no change in 4 and 1 became aggravated. One patient with acute motor-sensory axonal neuropathy had a recurrence after 10 months of the first attack. Conclusions : Axonal GBS has been considered uncommon clinically or electrophysiologically, but 8 probable acute axonal GBSs occurring at a mental hospital have been diagnosed in 3.5 years. 한 정신병원에 장기입원한 정신분열증환자에서 계속 발생한 8명의 급성 축삭성 GBS로 추정되는 환자들의 평균연령은 38세였으며 7명이 남자였다. 모든 환자들은 급성 상행성 양쪽하지 마비나 사지마비를 보이면서 심부 건반사가 소실되었다. 이 병은 주로 여름철에 많이 발생 하였으며 전기생리학적 검사상 축삭이 주로 손상된 소견을 보였다. IVIG치료를 한 1명을 제외한 나머지 환자들은 경제적 사정상 대증요법으로 치료하였다. AMAN형태의 환자 3명 중 1명에서 임상적 호전을 보였고, AMSAN형태의 환자 5명 중 2명에서 임상적 호전을 보였다. AMSAN형태의 환자중 1명에선 10개월 뒤 같은 증상이 재발하였다.

Patent foramen ovale Complicated with Chronic Mitral Valvular Insufficiency in A Mixed Dog

A 12-year-old intact female mixed dog (weighing 6.2 kg) was referred with primary complaints of severe abdominal distension, persistent coughing and exercise intolerance. Diagnostic studies found ascites, jugular distension, loud left and right apical systolic (grade 4/6) murmur, generalized cardiomegaly with caudal vena cava distension and left atrial dilation. Echocardiographic findings were consistent with degenerative mitral and tricuspid valve endocardiosis and bi-ventricular congestive heart failure. There was also a left to right shunting patent foramen ovale. The LV systolic function is depressed relative to the degree of volume overload. Based on diagnostic findings, this case was diagnosed as PFO complicated with mitral and tricuspid valve endocardiosis with ISACHC IIIa heart failure. The dog was treated with furosemide (2 mg/kg, q12hr, PO), sildenafil (1 mg/kg, q8hr, PO), pimobendan (0.3 mg/kg, q12hr, PO), enalapril (0.5 mg/kg, q12hr, PO) and spironolactone (1 mg/kg, q12hr). The clinical signs were gradually improved after medical therapy.

A Case of Thoracic Intercostal Neuralgia due to Ossification of the Ligamentum Flavum

Ossification of the ligamentum flavum(OLF) is not infrequent in the cervical and lumbar regions but is very rare in the thoracic spine. We reported a 62-year-old women with left thoracic intercostal neuralgia due to an OLF. Physical and neurological examination were normal. MRI showed an OLF with compression of the thecal sac in the posterolateral aspect of the T9-T10 level. Laminectomy and removal of the ligament resulted in marked clinical improvement. OLF is known to cause thoracic radiculomyelopathy, but presentation with intercostal neuralgia only is very rare.

Hemifacial spasm (HFS) may develop after Bell’s palsy (BP). But it was not reported that contralateral HFS occurred simultaneously in acute BP. A 25-year-old woman admitted with left HFS occurred simultaneously in acute right BP for 6 days. Past, family, and social history were unremarkable. Nerve conduction studies (NCS) and blink reflex (BR) test showed bilateral facial neuropathies. Brain MRI and cerebral angiography were normal. The symptoms and signs of HFS and BP were improved slowly after acyclovir and prednisolone therapy. Follow-up serial NCS and BR also showed a rapid improvement.
