        노인부양체계 및 노인부양지표의 개발

        From the standpoint that the fundamental problem in the social care for the aged in Korea stemmed from the discrepancy between the increasing dependency needs of the aged and the insufficient support from the care systems, the main objectives of this study are as follows: (1) to build theoretical framework; (2) to develope indicators relevant to the social care for the aged. This study approached from the conceptual basis of and need-fulfilling of the elderly and identified the following 5 components in analyzing the social care for the aged: (1) Needs of the elderly, (2) Support(need-fulfilling) system, (3) Criteria for need-fulfilling, (4) Unmet needs, (5) Social policy on aging. With the focus on the needs and the level of need-fulfillment of older people, this study classified the indicators into 3 need categories, namely, economic needs, health care needs and psycho-social needs. In addition to these categories, demographic and fiscal /budgetary category was included. The followings are brief description of the need categories. (1) In demographic and fiscal /budgetary category, demographic changes, changes in family structure and fiscal & budgetary changes were included. (2) In economic needs, the need of income security and the need for economic activity were included. Here was also included the housing needs of the elderly closely reflecting econorrnc status in Korea. (3) In health care needs, prevention and health maintenance, utlization of medical facilities and services, medical expenditures and nursing & physical care needs were included. (4) In phycho-social needs, life satisfaction, loneliness, changes of attitude on the care for the elderly, family relationship, helping patterns, kin-friend-neighborhood and social activity were included.

      • Two-track monitoring procedure for interpreting known and unknown fault types

        Multivariate control charts, including Hotelling’s T2 chart, have been widely adopted for the multivariate processes found in many modern systems. Traditional multivariate control charts assume that the control group is the only in-control information that can be used to determine a decision boundary. However, this assumption has restricted the development of more efficient control chart techniques that can capitalise on available out-of-control information. In the paper, we propose the two- track procedure that combine the Hotelling T2 and classification algorithms, while maintaining low false alarm rates. In addition, the proposed two-track procedure gains an increased detection power by taking advantage of the out-of control information, while it can also classify out-of-control group as known and unknown.

      • 노인의 자녀별거 결정요인 분석

        Representative samples of Korean 60 years of age and older were interviewed in the whole country, Korea. Based on the model for the determinants of living arrangements which has been widely accepted and used, this paper explores the economic and health status, preferences, and kin availability which helps to explain whether the Korean elderly live with their children. Two separate logistic regression models for unmarried and married elderly are conducted for the elderly who have at least one living child. The results show that: (1) Among the unmarried elderly, the likelihood of living separate from their children increases when they reside in rural area, own dwellings and prefer for independent living but decreases when they are economically dependent on their children, have more children and have at least one son: and (2) Among the married elderly, the likelihood of living separate from their children increases when they live in rural area and prefer for independent living, but decreases when they are economically dependent on their children and have at least one son, and when the husbands have a difficulty in ADL. Implications for the Korean elderly policies are suggested in the conclusion.

        가족이 수발하는 장기요양보호노인의 보호욕구 사정에 관한 연구

        본 연구는 노령화가 진전되고, 평균수명이 연장됨에 따라 증가하고 있는 장기요양보호노인의 보호욕구(care need)를 사정할 도구의 개발이 필요하다는 문제 의식하에, 1998년에 실시된 '장기요양보호노인의 생활과 서비스 실태조사' 자료를 이용하여 주수발자가 있는 장기요양보호노인의 보호욕구를 측정하여 보았다. Hiroaka, et al.(1996)의 이론적 분석틀을 한국 현실에 맞게 수정·보완하여 1) 노인의 건강상태: 노인의 일상생활수행능력(ADL: Activities of Daily Living)과 도구적 일상생활수행능력(IADL: Instrumental Activities of Daily Living)의 제한 정도와, 2) 비공식적 보호 정도(level of informal care): 주수발자와 노인의 동거 유무, 주수발자의 신체적·정신적 건강상태를 기준으로 장기요양보호노인의 보호욕구를 측정하였다. 본 연구결과, 장기요양보호노인의 절반 정도가 ADL 6개 항목 모두 제한을 느낄 정도로 건강상태가 좋지 못하였으며, 비공식적 보호를 받는 데 어려움이 있는 노인은 85% 정도 되었다. 그리고 장기요양보호노인의 건강상태와 비공식적 보호 정도를 조합하여 노인의 보호 욕구를 살펴보면, ADL에 제한을 받는 정도가 높은 노인의 상당수가 비공식적 보호를 제공받는 데 어려움이 있는 것으로 나타났다. 이러한 연구결과는, 가족보호를 보완할 수 있는 사회적 서비스(social service)의 확대필요성이 크다는 것을 보여주고 있다. This paper aims to measure the long-term care needs of the frail elderly with caregivers, using 'A survey on the status and social service needs of frail elderly' in 1998, in order to develop an assessment tool of long-term care needs in the frail elderly. The model of Hiroaka, et al.(1996) is modified to measure the long-term care needs of Korean elderly. Normative long-term care needs are judged by combination of two factors: 1) the health status of frail elderly, measured by activities of daily living(ADL) and instrumental activities of daily living(IADL), and 2) level of informal care, measured by co-residence with caregivers, physical and mental status of caregivers. It is found that almost the half of the frail elderly have difficulties in 6 activities of daily living and nearly three quarters of them are not being cared adequately by their caregivers. As a result of combining the health status of frail elderly and level of informal care, there are not quite a few frail elderly who have difficulties in ADL and are not being cared adequately. This finding shows social services for compensating the role of family should be expanded.
