        뇌졸중 후 일반적 특성과 관련된 성기능의 변화

        Purpose: The physical deficits after stroke have been well documented, but there is little information on the sexual functions of stroke patients. This study examined the associations of the general characteristics with the poststroke changes in sexual functions. Methods: A survey was conducted on 200 stroke patients from November 2008 to February 2009. Among the 200 submitted questionnaires, 147 responded, and the results were analyzed by SPSS for windows version 14.0. Results: A majority of the stroke patients reported a marked decline in all the measured sexual functions, i.e., frequency of sexual intercourse, frequency of kissing and romantic touching, thinking about sex with interest or desire, ability to become sexually aroused and the overall level of sexual satisfaction. Significant differences were observed in age (p<0.01), years since diagnosis (p<0.05), and educational level (p<0.01). There were no significant gender differences (p>0.05). Conclusion: Decreased sexual function is a common problem after stroke. These finding demonstrate a need to develop sexual adaptation programs that can be effective in improving the sexual functions reported by stroke patients.

        보행 시 체중과 굽 높이 변화에 따른 하지의 근활성도 분석

        [INTRODUCTION] The purpose of this study was to analyze lower limb muscle activity according to change weight and high-heeled height. [METHOD] We selected 30 young and healthy women who have been wearing high-heeled shoes for one year or more. The women were divided into 3 groups and walked with 3 high-heeled shoes(4, 7 and 9㎝) on a treadmill for 5min at 2m/sec. These Data were characterized by EMG measurements of three muscles(Tibialis anterior, Gastrocnemius, and Soleus muscle) while they were walking. The resting time was 5 min. [RESULT] In weight change, there was a significant difference in Tibialis anterior(p<.05). In the result of Tibialis anterior multiple comparisons, there was a significant increase in 40kgs as compared to two groups(50kgs, above 60kg). In high-heeled height change, there was a significant difference in Soleus muscle(p<.05). In the result of group multiple comparisons regarding heel hight, there was a significant increase in 9㎝ group compared to two groups(4㎝, 7㎝). [CONCLUSION] The less weight was, the more tibialis anterior was acted. The higher heel height was, the more soleus muscle was activated on walking than Gastrocnemius. [서론] 본 연구의 목적은 체중과 굽 높이에 따른 하지의 근활성도를 분석하는 데 있다. [방법] 본 연구의 대상은 높은 굽 신발을 1년 이상 착용한 젊고 건강한 여성 30명이다. 이들을 세 그룹(40kg대, 50kg대, 60kg이상)으로 나누어 세 종류의 높은 굽 신발(4, 7, 9㎝)을 착용하게 하였다. 착용 후 트레드밀 위에서 2m/sec의 일정한 속도로 보행하였다. 이에 대한 자료 값은 보행 시 세 근육(앞정강근, 장딴지근, 비복근)의 EMG 값으로 획득되었다. 휴식시간은 5분을 가졌다. [결과] 체중에 따른 One-way ANOVA 분석결과, 앞정강근에서 유의한 차이가 나타났다. 앞정강근에 대한 체중별 다중비교결과, 40kg 그룹이 50kg대, 60kg이상 그룹보다 근활성도가 유의하게 높은 것으로 나타났다. 굽 높이에 따른 One-way ANOVA 분석결과 가자미근에서 유의한 차이가 나타났다. 가자미근 활성도에 대한 굽 높이별 다중비교 분석결과, 굽 높이 9㎝그룹이 4㎝, 7㎝그룹보다 근활성도가 유의하게 높은 것으로 나타났다. [결론] 본 연구의 결과는 체중이 적으면 적을수록, 앞정강근이 활성화되며, 굽 높이가 높으면 높을수록 가자미근이 활성화 된다는 것으로 사료된다.

        Kaltenborn-Evjenth 정형물리치료에 대한 문헌적고찰(정형물리치료의 역사와 관련된 문헌을 중심으로)

        Orthopedic medicine specializes in the diagnosis and treatment of musculoskeletal conditions. The physical therapy speciality, Orthopedic Manipulative Therapy(OMT) is an important part of orthopedic medicine. Much of OMT is devoted to the evaluation and treatment of joint and related soft tissue disorders and one of the primary treatment methods is mobilization. When examination reveals joint dysfunction, especially decreased range of motion, joint mobilization techniques are often utilized. Soft tissue mobilization techniques are used to improve mobility and other soft tissues. The therapist should have precautions for a patient. These precautions are based on the patient's concurrent medical and surgical diagnosis. Lastly, the literature describing therapeutic interventions in patients with musculoskeletal problem is poor because the painful conditions are poorly characterized and the therapeutic interventions are poorly described; so, where Kaltenborn-Evjenth concept for patient with musculoskeletal problem were identified, the treatment concept are introduced.

        흉추에 적용한 척추교정이 피부온도와 주관적 피로에 미치는 영향

        The purpose of this study was to determine the changes of skin temperature and subjective fatigue of the thoracic vertebrae by the chiropractic adjustments. Stimulation of the sympathetic nervous system will cause the changes in the skin blood flow which can be detected by measuring the skin surface temperature. This study was to see whether chiropractic adjustments could affect the activity of the sympathetic nervous system as reflected by changes in skin temperature of the thoracic. Skin temperature and subjective fatigue on 16 subjects was measured before, within 10 sec, 10min, 20min and 30 min after a thoracic adjustment with Digital Infrared Thermal Imaging(DITI) and Visual Analogue Scale(VAS). The adjustments consisted of a straight posterior-to-anterior high-speed, low-amplitude thrust to the transverse process of T3-T10 using a reinforced hypothenar contact. The average temperature and fatigue changed when the spine was considered as an entire unit. These results illustrate that the blood flow through the soft tissue can be affected by specific adjustments to the spine. This study might be served as an useful baseline data for the changes of the circulation and fatigue after the chiropractic adjustments.

        목 폄 시 탄력성 테이프의 신장력 차이가 전방머리자세 증후군의 근수축 개시시간에 미치는 영향

        Purpose: This study aimed to identify the effect of varying the expansibility of elastic tape on the onset time of muscle contraction during neck extension for forward head posture syndrome. Methods: Forty-five young adults with forward head posture syndrome volunteered to participate and were randomly assigned to one of three groups according to the expansibility of the elastic tape (25%, 50%, 75%). The onset time of muscle contraction for the neck extensor during neck extension was measured using an electromyographic system (Free EMG, BTS, Italy). Multivariate analysis of variance was employed to determine the effect of different expansibilities of elastic tape on the onset time of muscle contraction during neck extension for forward head posture syndrome. When there was a statistically significant difference by MANOVA, Scheffe was used as a post-hoc test. The level of significance was set at α=0.05. Results: In the comparison of the onset time of muscle contraction of varying elastic tape expansibilities and measurement times, there was a significant difference between the groups (Lt. UT, Lt. SCM, Rt. SCM) (p < 0.05), but there was no significant difference in the interaction between the measurement time and the group, between the measurement time (p > 0.05). Conclusion: In the neck extension, 75% of the tape extensibility in the Lt. UT and both SCM shortened the muscle contraction onset time.

        편마비 환자의 신발 높이 조절이 동적체중부하율에 미치는 영향

        The purpose of this study was to determine the effect of lift to the shoe of the affected limb on gait patterns in subjects with hemiplegia. The subjects of this study were 18 post-stroke hemiplegics. For the study, insole of the paretic side was lifted 10 mm higher, and duration of dynamic weight bearing was measured. before and after the lift application. For the measurement of carry-over effect of lift, we got data of there three items prior to and 3 weeks after lift application and 3 days after removal of the lift. Dynamic weight bearing was significantly decreased in heel contact and footflat phases only when just after application of the lift, without any change after 3 weeks application. In heel-off phase, dynamic weight bearing did not show any significant difference between before and just after application of lift whereas significantly decreased after 3 weeks application. According to this study, lift applied to the shoe of the paretic limb was not significantly effect in inducing dynamic weight bearing, but changed a dynamic weight bearing.

        안정된 지지면과 불안정한 지지면에서 팔굽혀 펴기 시 상지 근활성도와 운동학적 변수간의 상관분석

        Purpose: We investigated the relationship between the muscle activity and kinematic variables of the upper extremity during a push-up task on stable and unstable surfaces. Methods: We recruited 15 healthy subjects. Subjects completed the push-up task on stable and unstable surfaces. Surface electromyograms were recorded from the serratus anterior, upper trapezius, latissimus dorsi, infraspinatus to monitor changes in muscle activity. Markers for kinematic changes of elbow flexion, shoulder extension, shoulder retraction and scapular adduction were attached at C7, the T7 spinous process, both acromions, the scapula superior and inferior angle, the humerus lateral epicondyle, and the ulnar styloid process. Correlation coefficients between muscle activity and kinematic variables were analyzed by SPSS for Windows, version 15.0. Results: On the unstable surface, elbow flexion and shoulder extension increased with increasing muscle activity of serratus anterior, upper trapezius and infraspinatus. On the stable surface, shoulder retraction decreased with increasing muscle activity of serratus anterior and infraspinatus. Scapular adduction decreased with all types of increasing muscle activity, regardless of the support surface. Conclusion: Correlations between muscle activity and kinematic variables were observed on stable and unstable surfaces during a push-up task, while correlations between right and left variables were not clear. These finding suggest that it may be used to develop a rehabilitation program which could be effective in improving shoulder function in patients with shoulder problems.

        가상현실기반과 고전적 스쿼트 운동 방법에 따른 정적, 동적 균형지수 간 상관분석

        Purpose : The purpose of this study was to examine the correlation between static and dynamic balance according to the virtual reality-based squat and conventional squat exercise. Method : Twenty four participants were randomly assigned to the virtual reality-based squat (VRS) group (n=12) or conventional squat (CS) group (n=12). The static balance (C90 area, C90 angle, trace length, sway average velocity) and dynamic balance (forward, rearward, leftward, rightward) were measured using a force plate by BT4. The VRS group used the virtual reality system during 4 weeks, while the CS group underwent classical squat training. The collected data was analyzed using the paired t-test for static and dynamic balance comparisons before and after exercise in both groups and Pearson's test for the correlation between static and dynamic balance according to the measured time. The significance level was set to 0.05 Result : There was no significant correlation between group and static and dynamic balance related variables(p>.05). There was a significant correlation between measurement time and static and dynamic balance related variables(p<.05). According to the measurement time, the static balance parameter C90 area in the VRS group after exercise was significantly decreased(p<.05). The values of forward, leftward and rightward in the VRS group were significantly increased after exercise(p<.05). Conclusion : It is suggested that 20 normal healthy adult men and women who have normal balance ability can improve their ability to control their posture by improving the balance ability when applying virtual reality-based squat exercise.
