The purpose of this study is to suggest a body percussion program to rhythm expression class in elementary school and to investigate the effect of the body percussion program on dance enjoyment. The participants of this study are fifty students from two sixth-grade classes in elementary school in Gangwon province, divided into an experimental group and a control group of twenty five students each. The results of the study are as follows. First, the homogeneity of the experimental and control groups is confirmed in the pre-test between two groups. Second, as the result of comparing the pre-test and post-test of the experimental group, dance enjoyment is significantly elevated in the post-test than in the pre-test, and overall, dance enjoyment, physical and mental health, sociability, and a sense of accomplishment are appeared to be significant. Third, as the result of the post-test comparison between two groups, the experimental group is revealed to have a significantly higher level of dance enjoyment than the control group. Furthermore, compared to the control group, the experimental group appears to have a significantly higher level in both sociability and a sense of accomplishment, whereas there is no significant difference in maintaining physical and mental health. In conclusion, this study has suggested a rhythm expression class using body percussion as a part of elementary school dance class, and it is found that rhythm expression class using body percussion program is positively associated with dance enjoyment.

Cholangitis and cholecystitis are intra-abdominal infections that show poor prognosis upon progression to sepsis and multiorgan failure. Administration of antibiotics with high antimicrobial susceptibility and removal of infected bile at the initial treatment are important. After undergoing ERCP for diagnostic purposes, a 58-year-old man developed acute cholangitis and cholecystitis accompanied by rhabdomyolysis, multi-organ failure, and severe sepsis. Broad-spectrum antibiotics with bedside endoscopic nasobiliary drainage were administered, but clinical symptoms did not improve. Therefore, bedside EUS-guided transgastric gallbladder aspiration and lavage was performed, resulting in successful treatment of the patient. We report the above described case along with a discussion of relevant literature. (Korean J Gastroenterol 2015;65:370-374)

Biodegradable nanofibrous matrix containing insulin was fabricated by simple electrospinning method. The electrospun poly(D,L-lactide-co-glycolide)(PLGA)/Insulin nanofiber structure revealed randomly aligned fibers with average diameter of 700 nm determined and calculated by SEM and image analyzer. ATR-FTIR and ESCA mesurement were used to confirm the presence of insulin in PLGA/Insulin nanofibers. PLGA nanofibers were degraded slowly in phosphate buffer solution and 50 days were required to degrade completely. A 50% of insulin incorporated to PLGA nanofibers was released within 1 day of incubation, and then remained insulin in nanofibers were released slowly to 40 days of incubation. Human fibroblasts` behavior on PLGA/Insulin nanofibrous matrices has been investigated. Fibroblasts were well attached and proliferated faster on the surface of PLGA/Insulin nanofibers than on control PLGA nanofibers, due to the strong cell proliferating effect of insulin, which is one of the growth factors. From the above results, electrospinning of PLGA with insulin is a promising method for application to cell culture matrices.

The electrospun poly(3-hydroxybutyrate-co-3-hydroxyvalerate)(PHBV)/gelatin (1:1) nanofiber structure revealed randomly aligned fibers with average diameter of 600 nm determined and calculated by SEM and image analyzer. ATR-FTIR and ESCA mesurement were used to confirm the presence of gelatin in PHBV/gelatin nanofibers. Human fibroblasts` behavior on PHBV/gelatin nanofibrous matrices has been investigated. Fibroblasts were well attached on the surface of control PHBV and PHBV/gelatin nanofibers. Initial cell attachment on PHBV/gelatin nanofibers were significantly higher than that of control PHBV nanofibers, because gelatin has many RGD moiety to mediate cell attachment. Electrospinning of PHBV with gelatin significantly decreased water contact angle of control PHBV nanofiber. Cells could penetrate into the nanofibers and enabled 3D cell culture, due to the hydrophilic nature of PHBV/gelatin nanofibers. PHBV/gelatin nanofibers facilitated spacial distribution, and initial attachment of cells. From the above results, electrospinning of PHBV with gelatin is a promising method for application to cell culture matrices.

Obesity is a prevalent disease with significant morbidity and mortality. It is a state of chronic low-grade inflammation due to excess body fat. Weight homeostasis is maintained through changes in various gastrointestinal hormones caused by dietary intake. However, being overweight or obese breaks the balance of these appetite-related gastrointestinal hormones and creates resistance to the actions of these hormones. The sensitivity of vagal afferent neurons to peripheral signals becomes blunted. Cytokines produced by excessive fat tissue damage our normal immune system, making us vulnerable to infection. In addition, various changes in gastrointestinal motility occur. Therefore, this review focuses on the various changes in gastrointestinal hormones, the immune state, the vagus nerve, and gastrointestinal movement in obese patients. (Korean J Med 2019;94:403-409)

Although several investigators have reported the coronary artery spasm after coronary artery stenting, all previous reports, coronary spasm after drug-eluting stent (DES) placement was occurred in first generation DES, such as sirolimus-eluting (Cypher®) and paclitaxel-eluting stents (Taxus®). Clinical relevance is unknown, the underlying mechanism may be delayed vascular healing due to toxic effects of the sirolimus and paclitaxel to endothelial cells (EC). The second-generation DES, such as everolimus-eluting (Xience V®) and zotarolimus-eluting stents (Endeavor®), are proving to be significantly more effective and safer than the first-generation DES.1)2) We report a patient with severe coronary artery spasm lead to myocardial infarction after second generation DES implantation at the site of detectable atherosclerotic lesion.

Zollinger-Ellison syndrome (ZES) is characterized by gastrinoma and resultant hypergastrinemia, which leads to recurrent peptic ulcers. Because gastrinoma is the most common pancreatic endocrine tumor seen in multiple endocrine neoplasia type I (MEN 1), the possibility of gastrinoma should be investigated carefully when patients exhibit symptoms associated with hormonal changes. Ureteral stones associated with hyperparathyroidism in the early course of MEN 1 are known to be its most common clinical manifestation; appropriate evaluation and close follow-up of patients with hypercalcemic urolithiasis can lead to an early diagnosis of gastrinoma. We report a patient with ZES associated with MEN 1, and urolithiasis as the presenting entity. A 51-year-old man visited the emergency department with recurrent epigastric pain. He had a history of calcium urinary stone 3 years ago, and 2 years later he had 2 operations for multiple jejunal ulcer perforations; these surgeries were 9 months apart. He was taking intermittent courses of antiulcer medication. Multiple peripancreatic nodular masses, a hepatic metastasis, parathyroid hyperplasia, and a pituitary microadenoma were confirmed by multimodal imaging studies. We diagnosed ZES with MEN 1 and performed sequential surgical excision of the gastrinomas and the parathyroid adenoma. The patient received octreotide injection therapy and close follow-up.

Chitosan has been proposed for a variety of tissue engineering scaffold. In this study, we evaluated the in vivo biocompatibility of chitosan scaffolds with various deacetylated degree(DD) in rat, mouse and rabbit models. The structure of the scaffold was improved by addition of butanol during the lyophilization(-B) and the DD of the chitosan scaffold(CS) was modified by treatment with acetic anhydride to fully acetylated form(ACS) or with NaOH to fully deacetylated form(AFCS). To evaluate biocompatibility of chitosan with different DD, the scaffolds were implanted subcutaneously, and animals sacrificed after 3, 15, 30, and 60 days in rats and after 1, 3, and 9 weeks in mice and rabbits. In rat implantation models, the improved biocompatibility of the ACS was demonstrated by matrix-rich neo-tissue formation with much less infiltration of polymorphonuclear cells(PMNs) and faster degradation after subcutaneous implantation compared to CS scaffolds. However, in mouse and rabbit implantation models, the severe immune reaction of the CS-B was demonstrated by large numbers of infiltration of neutrophils during 9 weeks and the very slow degradation after subcutaneous implantation of ACS-B and AFCS-B was observed. To evaluate the effectiveness of chitosan scaffold as a dermal substitute, the scaffolds were implanted onto the full thickness skin wound in rat and rabbit models. The ability of chitosan scaffold to repair full thickness dermal defects was evaluated by histological examination of transplanted skin. In rat skin wound model, transplantation of ACS as well as CS restored dermis, in contrast, the severe inflammatory reaction that occurred in rabbit skin wound model. In conclusion, we showed that the biocompatibility of chitosan scaffolds can be variable depending on the DDs as well as on animal species. Therefore, more comprehensive comparative studies may be necessary before the clinical application of the chitosan-based scaffold.

Multiple endocrine neoplasia type 1 (MEN1) syndrome is a relatively rare disease, characterized by the occurrence of multiple endocrine tumors in the parathyroid and pituitary glands as well as the pancreas. Here, we report a case of MEN1 with neuroendocrine tumors (NETs) in the stomach, duodenum, and pancreas. A 53-year-old man visited our hospital to manage gastric NET. Five years prior to his visit, he had undergone surgery for incidental meningioma. His brother had pancreatic nodules and a history of surgery for adrenal adenoma. His brother`s daughter also had pancreatic nodules, but had not undergone surgery. The lesion was treated by endoscopic submucosal dissection and diagnosed as a grade 1 NET. Another small NET was detected in the second duodenal portion, resected by endoscopic submucosal dissection, which was also diagnosed as a grade 1 NET. During evaluation, three nodules were detected in the pancreas, and no evidence of pituitary, parathyroid tumors, or metastasis was observed. After surgery, the pancreatic lesions were diagnosed as NETs, with the same immunohistochemical patterns as those of the stomach and duodenum. Genetic testing was performed, and a heterozygous mutation was detected in the MEN1 gene, which is located on 11q13. (Korean J Gastroenterol 2017;69:181-186)
