Cyclosporin A 반복투여후 랫드 간세포의 미세구조적 관찰

This study was carried out to investigate the fine structural changes of rat hepatocytes by repeated treatment of cyclosporin A that has been widely used for immunosuppressive drug in organ transplantation. Sprague-Dawley rats were kept in experimental circumstances for 2 weeks and 50mg/kg B.W of cyclosporin A was injected once a day subcutaneously for 7 days and sacrificed at 1 hour, 1 day, 3 days, 7 days, 14 days, 28 days after the last injection. Fine structural changes were observed by transmission electron microscope (JEM 1200EX II) and the results obtained were as follows. 1. Accumulation of lipid droplets in hepatocytes was prominently increased in 1 hour and 1 day lapse groups, and this finding was slightly reduced in 3 days lapse group and remarkably reduced from 7 days lapse group enough to be recovered completely in 14 days lapse group. 2. Dilatation of rough endoplasmic reticule cisternae, detachment of membrane bound ribosomes, proliferation of smooth endoplasmic reticula were observed in 1 hour and 1 day lapse groups, and these findings were mild in 3 days lapse group and abruptly reduced from 7 days lapse group enough to be recovered completely in 28 days lapse group. 3. Small myelin figures were observed in 3 days lapse group after CsA-treatment. 4. Swelling of mitochondria and destruction of their cristae were observed in 1 hour and 1 day lapse groups, and these findings were recovered from 3 days lapse group. 5. Dilatation of bile canaliculi and remarkable loss of microvilli in the pericanalicular wall were observed in 1 hour lapse group and the most severe change was shown in 1 day lapse group and lasted to 3 days lapse group, and these findings were reduced gradually from 7 days lapse group enough to recovered completely in 28 days lapse group.
분쇄를 동반한 관절 내 원위 요골 골절의 치료 : 관절경을 이용한 정복 및 핀고정 후 외고정 보강술

목적: 분쇄를 동반한 관절내 원위 요골 골절의 치료로서 관절경을 이용한 정복 및 핀 고정 후 외고정 보강술의 임상적 치료 결과를 보고하고자 하였다. 대상 및 방법: 1년 이상 추시가 가능한 37명의 환자를 대상으로 하였다. 수술 직후 및 골유합 후의 방사선 사진을 비교 분석하였고 최종 추시시 객관적 및 주관적 기능 평가를 하였으며 전반적인 평가로 변형된 Green과 O'Brien의 평가법을 사용하였다. 결과: 전반적인 평가는 우수 11명, 양호 20명, 보통 4명 및 불량 2명이었다. 수술 직후 및 골 유합 후의 요골 단축, 전방 경사각 및 요측 관절면 경사각의 변화는 통계학적으로 유의하지 않았으나(p>0.05), 최종 추시시 전방 경사각은 평균 4.6°로 반대쪽 정상 부위와 차이가 있었다. 최종 추시시 손목 관절의 평균 운동 범위는 반대쪽의 73-92%로 측정되었고, 악력은 반대쪽의 73%였다. DASH와 PRWE 점수는 각각 평균 49.2 (35-67)와 31.0 (13-73)이었다. 결론: 분쇄를 동반한 관절내 원위 요골 골절의 치료로서 관절경적 정복 및 핀 고정술 후 외고정 보강술은 비교적 정확한 관절면 정복과 정복의 유지에 도움이 될 수 있는 시술이라고 사료된다. Purpose: Here we report the clinical results of arthroscopically assisted reduction and pin fixation supplemented with an external fixator for the treatment of comminuted intra-articular fractures of the distal radius. Materials and Methods: Thirty-seven patients with a minimum follow-up period of 1 year were enrolled. Radiographs obtained immediately after surgery and those obtained after osseous union were compared and analyzed. The objective and subjective function of the wrist were evaluated. The overall outcomes were assessed using a modified Green and O'Brien system. Results: Eleven patients had an excellent outcome, 20 good, 4 fair and 2 poor. Loss of radial shortening, radial inclination, step-off, and a gap between the radiographs obtained immediately after surgery and at osseous union were not significant. The volar tilt angle of the final follow-up evaluation averaged 4.6°, which was significantly different compared to the contralateral side. The mean range of motion of the wrist ranged from 73% to 92%. The mean grip strength was 73% compared to the uninjured contralateral wrist. The mean DASH and PRWE outcome scores were 49.2 (range 35-67) and 31.0 (range 13-73), respectively. Conclusion: Arthroscopic reduction and pin fixation supplemented with an external fixator, for the treatment of comminuted intra-articular fractures of the distal radius, allows for accurate articular reduction to be established and maintained.
