Purpose: To compare the ocular biometry and prediction of postoperative refractive outcomes using IOL Master??700 (Carl Zeiss Meditec AG, Jena, Germany) and A-scan (Aviso??, Quantel Medical, Cournon-d Auvergne, France) in severe cataract. Methods: Seventy nine eyes with severe cataract under visual acuity of 1.7 logMAR were scheduled for cataract surgery. Axial length and anterior chamber depth were measured preoperatively with IOL Master?? 700 and A-scan. Mean absolute error was obtained with the difference between preoperative predictive refraction and postoperative refraction outcome using autorefraction. Results: Forty three cases received phacoemulsification and 36 received extracapsular cataract extraction. The mean absolute error between IOL Master??700 and A-scan in 20 eyes received phacoemulsification and 11 eyes received extracapsular cataract extraction (ECCE) was not significant (p < 0.001 and p < 0.001, respectively). IOL Master??700 measured mean axial lengths and anterior chamber depths longer than A-scan in phacoemulsification group (p < 0.001 and p < 0.001, respectively). However, there was no significant difference in mean axial length of ECCE group between two devices (p = 0.350). Conclusions: There was no significant difference between IOL Master??700 and A-scan in prediction of refractive outcomes after surgery of severe cataract under visual acuity of 1.7 logMAR. If available, IOL Master??700 can be considered as reliable method for prediction of postoperative refractive outcomes in cases of severe cataract.

초등학생과 부모를 위한 진로집단상담 프로그램 개발

이 연구에서는 초등학교 고학년의 진로발달을 촉진하기 위하여 초등학생과 부모를 위한 진로집단상담 프로그램을 개발하고 효과를 검증하였다. 선행연구들을 분석하고 초등학생, 부모, 청소년상담자를 대상으로 요구조사를 실시하여, 그 결과를 토대로 아동용 6회기, 부모용 4회기의 집단상담프로그램을 개발하였다(1회기는 공동회기). 아동 프로그램은 긍정적인 진로태도 형성, 자아 및 직업영역의 탐색에 대한 역량을 키우는데 목표를 두었다. 이 과정에서 진로발달이 연속적인 학습의 과정이라는 사회학습이론의 가정들이 적용되었다. 부모프로그램에서는 자녀 진로발달 촉진자로서 부모 역할, 진로활동장면에서의 상호작용 기술 등이 다루어졌다. 전국 공모를 통해 선정된 6개의 청소년상담센터에서 참가에 동의한 총 아동56명, 부모58명을 대상으로 실험집단 6개와 통제집단(대기자집단) 6개를 구성하였으며 효과검증 이후 통제집단은 프로그램을 처치받도록 하였다. 사전검사를 공변량으로 사용한 ANCOVA 결과 본 프로그램은 아동의 진로자기효능감 향상과 바람직한 부모의 진로행동을 증가시키는 데에 효과가 있는 것으로 나타났다. 다만 부모프로그램에서 상호작용의 충분한 연습이 필요하다는 피이드백을 반영해 연습 2회기를 추가하였다. 전문가 2인의 감수를 받아 아동6회기와 부모 6회기(필수 4회기, 추가 2회기는 선택)의 최종프로그램을 확정하였으며 이 연구의 결과에 대한 논의와 제한점을 기술하였다. In this study, career group counseling program was developed for elementary school students and their parents. To develop a program, results of relevant studies, and the relevant career counseling programs were reviewed and analyzed. Needs assessment was conducted with elementary school students, their parents, and youth counselors to verify the primary components of career group counseling program. The resulting primary program consists of six 60-minute sessions for children and four sessions for their parents. The goal of the program includes (a) the increase of career recognition of children, (b) the development of positive attitudes toward career, and (c) the development of career aspirations integrating children`s interests and abilities. Throughout the process, the development of children`s learning ability in career was intended. The focuses of parental sessions were given to (a) education on the facilitating roles of parents for children`s career development, and (b) exercises of desired parent-child interaction skills. The primary program was implemented at six youth counseling centers to evaluate its effectiveness. The experimental group consists of 29 children and 30 parents; control group (waiting list) consists of 27 children and 28 parents. The results showed that the program was effective in increasing children`s career self-efficacy, Also, desired career-related parental behaviors was increased. Given the feedback that the number of parental sessions are not long enough to work-through the included contents, the number of parental sessions was modified into six sessions. The implications and limitations of the results are described.

국내 대학 담당자들이 인식한 외국인유학생의 어려움과 심리지원 방안

본 연구는 외국인 유학생의 대학생활 적응을 돕기 위하여 외국인 유학생을 담당하는 현장전문가들을 대상으로 이들의 관점에서 지각한 외국인 유학생들이 경험하는 문제와 효과적인 지원을 위해 필요한 요소가 무엇인지 살펴보고자 하였다. 이를 위하여 외국인 유학생 상담자 7명과 대학 내 외국인 유학생 지원 부서의 실무자 7명, 총 14명을 대상으로 개별 면담을 실시하고 결과를 분석하였다. 연구 결과 전문가들이 인식한 외국인 유학생이 경험하는 문제는 총 29개의 개념, 8개의 하위범주, 2개의 상위범주로 유목화되었다. 두 가지 상위범주에는 환경․맥락적인 차원에서 경험하는 문제, 학업과정에서 경험하게 되는 복합문제들이 포함된다. 또한, 효과적인 지원을 위해 필요한 요소는 65개의 개념과 16개의 하위범주, 3개의 상위범주로 유목화되었다. 세 가지 상위범주에는 상담 및 지원 전문가의 역량, 효과적인 접근 방법, 서비스 제공의 체계성이 포함된다. 본 연구의 결과는 외국인 유학생들을 지원하는 현장의 접점에 있는 전문가들을 대상으로 외국인 유학생의 대학생활을 효과적으로 지원할 수 있는 요구 사항들을 파악하고 실질적인 지원을 위한 기초자료를 제공하였다는 의의를 지닌다. 연구의 의의와 시사점, 구체적인 연구결과를 제시하였다. The purpose of this study was to explore the problems experienced by international students and to provide effective support from their viewpoints who are working in international students support department and international student counselors at the student counseling center. For this purpose, we conducted interviews with seven international students" counselors and seven international students" support staff in universities. As a result, the problems experienced by the international students recognized by the experts were classified into 29 concepts, eight subcategories, and two categories. Specifically, the two categories include problems experienced in the context of the environment, and complex problems experienced in the course of study. In addition, a total of 65 concepts, 16 subcategories, and three categories were identified as necessary for effective support. The three categories include multicultural competency of counseling and support professionals, effective access, and systematic services provision. The result of this study implies that the experts at the contact points of the field supporting the international students are able to grasp the requirements that can effectively support the university life of international students and provide basic data for practical support. The significance of the study, its implications, and the specific results of the study were presented.

임신중 발생한 추간판 탈출증

A herniated nucleus pulposus (HNP) during pregnancy has been a rare occurrence with a reported incidence of 1:10,000 case. It is hard to diagnose because of the potentially hazardous effect of ionizing radiation to the fetus when complaining back pain during pregnancy. In this case, magnetic resonance imaging (MRI) provide safe and useful modality and in general, back pain usually respond readily to conservative treatment. Cauda equina syndrome or severe and/or progressive neurologic deficit is a medical emergency that necessitates prompt surgery during pregnancy. She admitted at 31 weeks gestation for sudden development of right leg paresthesia and ankle motor weakness, an MRI showing compression on right nerve root at the level of L5~S1. After 4 weeks treated with bed rest and analgesics, we delivered by cesarean section and laminectomy at the same time. We have experienced a pregnancy with HNP, so report of this case with brief review of literature.
