        간호대학생의 시뮬레이션 기반 교육에서 개념지도를 활용한 인수인계 교육 경험

        Purpose: This qualitative phenomenological study aims to understand how nursing students interpret their experiences with handover education using concept maps in simulation-based nursing education. Methods: Data were collected from September 18, 2021 to April 6, 2022, through in-depth one-to-one interviews with 11 nursing students. Interviews, which were conducted once or twice, lasted for 30 to 45 minutes per session. The main interview question was “How do you make meaning of your experiences with handover education using concept maps in simulation-based nursing education?” The collected data were analyzed using content analysis. Results: Three categories of were identified: actively engaged with students, addressed the reality of handover education, and there are two sides to handover education. The participants in this study require systematic education on the take-over method and process that can integrate and structure the continuity and thinking necessary for nursing work. Conclusion: Handover education using concept maps in simulation-based methods should be adapted according to the abilities of nursing students and their contextual needs. Future studies using large sample sizes and multiple settings should substantially evaluate the impact of handover education programs using concept maps on clinical outcomes.

        암 환자의 수면장애에 대한 비약물적 중재 연구동향 분석

        Purpose: This review aimed to explore current publication trends, major concepts, measurements, and intervention types regarding sleep-wake disturbance in cancer patients. Methods: Studies published up to 26 December, 2021 were examined through PubMed, EMBASE, The Cochrane Library, KMbase, CINAHAL, ScienceON and Google scholar using English and Korean keywords with appropriate definitions related to sleep-wake disturbances in cancer patients. A total of 3,542 articles were found and 73 articles introducing intervention to reduce sleep-wake disturbance were included in the final analysis. Results: Interventions for sleep-wake disturbance in cancer patients were classified into the following types: exercise interventions (38.1%), cognitive behavioral therapy (26.2%), complementary and alternative medicine intervention (19.0%), mindfulness-based stress reduction intervention (9.5%), herbal medication intervention (2.4%), dance intervention (1.2%), music intervention (1.2%) and mixed intervention (1.2%). Most of the interventions (52.1%) were performed on breast cancer patients. The Pittsburgh Sleep Quality Index was the most frequently used subjective measurement. Objective measurements to measure sleep quality or sleep disturbance included actigraphy and polysomnography. Sixty-four articles (87.7%) out of 73 found that interventions to reduce sleep-wake disturbance in cancer patients were effective. Conclusion: Nursing intervention to reduce sleep-wake disturbance is important in oncology nursing practice. This literature review emphasizes the need to develop more tailored and suitable interventions considering various cancer patients’ characteristics.

      • Comparative Analysis of Gene Expression Patterns after Exposure to Nonylphenol in Human Cell Lines

        Nonylphenol (NP), a byproduct of industrial synthesis, is quite similar to estrogen in structure, and is known as an environmental estrogen that induces estrogenic disturbances. It has been utilized in several industries for the manufacture of skin cleaning materials, kitchen detergents, cosmetics, fabric detergents, and ink binder. Due to its characteristic strong estrogenic potency, NP is capable of disrupting the reproductive hormone system. Exposure to NP for a prolonged period increases the chances of developing breast and lung cancers. In this study, we conducted a comparative study of expression profiles between HK (Human Kidney cell), MCF-7 (Human breast cancer cell), and LNCaP (Human prostate cancer cell) cells treated with NP at the value of IC20, using Agilent whole genome microarrays. After comparative analysis, we detected some specific expression patterns in each of the cell lines. However, the expression patterns from the HK-2 and MCF-7 cells are quite similar. Interestingly, estrogen receptor 1 and 2 genes were downregulated only in MCF-7 cells, whereas the androgen receptor (AR) gene evidenced overexpression in all 3 of the cell lines. The PDZK1 gene, which has been identified as an estrogen-responsive gene, has also been shown to be overexpressed in all 3 of the tested cell lines. The majority of differentially expressed genes in NP treatment were shown to be involved in cell proliferation, transcription, and signaling. These results may establish a framework for nderstanding the mechanisms underlying the toxicity of NP.

        뇌졸중 표준화 환자를 활용한 시뮬레이션기반 실습교육 경험

        본 연구는 간호학생을 대상으로 뇌졸중 표준화 환자를 활용한 시뮬레이션기반 교육 경험을 심층적으로 이해하고 이를 통해 경험의 본질을 파악하고자 함이다. 자료수집은 2023년 6월 25일부터 자료가포화상태에 이르게 된 2023년 8월 25일까지 간호학생 10명을 대상으로 심층 개별 인터뷰를 통해 수집되었고, Colaizzi가 제시한 현상학적 연구 분석 방법으로 분석되었다. 주요 인터뷰 질문은 “뇌졸중 표준화 환자를 활용한 교육 경험은 당신에게 어떤 의미가 있었나요?”이었고, 수집된 데이터는 내용분석을 이용하여 분석하였다. 연구결과, 총 7개의 의미있는 진술이 추출되었고, 도출된 주제모음은 ‘간호 교육적 차원’, ‘개인적 차원’, ‘간호 직무적 차원’으로 나타났다. 이로써 뇌졸중 표준화 환자 활용은 간호학생들의 실무역량 습득과 뇌졸중 환자간호에 대한 이해를 높였고, 표준화 환자 활용에 있어 긍정적인 요인강화와 제한된 요인들을 파악하여 실무역량을 증진하기 위한 시뮬레이션기반 교육전략이 필요함을 알 수 있다.         뇌경색환자 간호과정 시나리오 개발 및 효과: HPS

        The aim of this study was to develop and apply a nursing process simulation scenario for cerebral infarction patients using HPS for nursing students. The effects of this simulation were evaluated and compared with the effects of traditional teaching methods. This study was conducted on 3rd grade students at the College of Nursing from December 7 to December 26, 2018. This study examined 38 subjects in the experimental group and 39 in the control group. This research went through the analysis phase, design phase, and development phase for the development and application of the scenario. The confidence, knowledge, and performance before and after the intervention in the experimental and control groups were analyzed using a t-test. This study developed a simulation scenario based on a nursing process for cerebral infarct patients and was designed to infer three nursing diagnoses. Both groups showed significant pre-post scores except for the clinical performance. The experimental group had higher post-score scores and greater difference between the post-test and pre-test. This study is meaningful in that a standardized scenario of nursing process of infarct patients was developed. In addition, the developed scenario shows the possibility of developing teaching and learning methods that can integrate theoretical learning and practice. 본 연구는 간호학생을 대상으로 HPS를 활용한 뇌경색환자 간호과정 시뮬레이션 시나리오를 개발하고 적용하여 전통적 강의방식과 그 효과를 비교・평가해 보고자 함이다. 본 연구는 2018년 11월 27일부터 12월 26일까지 간호대학의 4학년 학생을 대상으로 하였으며, 실험군 38명, 대조군 39명으로 최종 77명을 대상으로 하였다. 본 연구는 분석단계와 연구 설계단계를 거쳐 개발단계에서 시나리오를 개발하여 적용하는 과정을 거쳤다. 실험군과 대조군의 중재 전, 후의 자신감, 이론적 지식, 임상수행기술은 t-test를 통해 분석하였다. 본 연구결과 뇌경색 환자에 대한 간호과정을 기반으로 한 시뮬레이션 시나리오가 개발 되었으며, 이는 3가지 간호진단을 유추할 수 있도록 설계되었다. 연구결과 두 그룹 모두 임상수행기술을 제외한 사전사후 점수가 통계적으로 유의하였으며, 실험군과 대조군간 사후점수 및 사전, 사후의 차이검증에서 실험군이 유의하게 높게 나타났다. 본 연구는 표준화된 뇌경색환자 간호과정 시뮬레이션 시나리오를 개발하였다는 점에서 의의가 있다. 또한 개발된 시나리오가 실습 뿐 아니라 이론적 학습까지 통합할 수 있는 교수학습방법으로의 가능성을 보여주고 있다.

        Analysis of Gene Expression in the Testes of Mice Exposed to Bisphenol A and Nonylphenol

        Estrogenic environmental compounds (xenoestrogens) have adverse effects on male reproductive systems, including decreased sperm counts and problems with reproductive development. Although the male reproductive toxicity induced by xenoestrogens has been investigated, the molecular mechanisms by which estrogenic compounds induce toxicity in testes are unclear. This study used a microarray analysis to examine testicular toxicity and gene expression profiles in mice after 30 days of exposure to two estrogenic compounds, bisphenol A (BPA) and nonylphenol (NP). In total, 275 and 729 genes were identified as being either up- or down-regulated, with over 1.5-fold changes, in the testes of the BPAand NP-treated groups, respectively. Differentially expressed genes were classified using a k-means clustering algorithm, and their biological functions and canonical pathways were further analyzed using Ingenuity Pathways Analysis (IPA). Toxicological function analysis characterized the mode of action according to BPA and NP. Pathway analysis identified genes involved in gluconeogenesis and calcium signaling in the BPA-treated group and genes involved in Wnt/β-catenin and estrogen receptor signaling in the NP-treated group. In addition, several differentially expressed genes that may play a role in spermatogenesis, such as Odf1 and the Sox family, were identified. Collectively, these data help to elucidate the molecular mechanism of reproductive toxicity induced by xenoestrogens.

        Development of the Korean Health Behavior for Dementia Prevention Scale for Older Adults

        Purpose: This study aimed to develop the Korean Health Behavior for Dementia Prevention Scale (K-HBDP) and test its validity and reliability. Methods: In this methodological study, the K-HBDP scale was administered to 216 community-dwelling older adults (aged 65 or older) in 2020 in seven cities of western Korea using convenience sampling. Item analysis, construct validity, item convergent and discriminant validity, criterion validity, and internal consistency reliability were evaluated using SPSS for Windows 27.0. Results: The scale includes 23 items and five subscales: cognitive social activities, healthcare management, lifestyle, smoking and drinking, and eating habits, and the cumulative variance explained by the factors was 66.8%. Cronbach’s ⍺ for internal consistency of the total scale was .90 and ranged from .65 to .89 for all subscales. Item convergent and discriminant validity of the K-HBDP were confirmed. Criterion validity was demonstrated with the Health Promoting Lifestyle Profile-II. Conclusion: The findings suggest that K-HBDP scale can be used to measure healthy activities to prevent dementia in older adults. The use of this tool is expected to help develop interventions, educational materials, and training programs to improve nursing practice.

      • Gene Expression Profiles of Nonylphenol as Representative EDCs in Normal Human Kidney HK-2 Cells

        Nonylphenol (NP) is an organic compound of the wider family of alkylphenols and is a product of industrial synthesis formed during the alkylation process of phenols, particularly in the synthesis of polyethoxylate detergents. NP is an endocrine disruptors. Endocrine-disrupting chemicals (EDCs) are exogenous compounds that have the potential to hamper with hormonal regulations and the normal endocrine system and consequently cause health effects. In numerous chemical substances, the action mechanism of an endocrine disruptor is not clearly understood. In the present study, in vitro gene expression profiles were analyzed in nonylphenol-treated HK-2 cells using an Agilent Human 4×44 K whole genome array including 41,000 transcripts. Gene expression profiles were analyzed 3 and 48 hrs after exposure to NP with 2 different doses. We analyzed the gene expression profiles in order to understand the biological effects at level of gene functions and their time-dependent effects. A total of 1,727 genes were identified as being either over or down-expressed over 2-fold changes (P-value⁄0.05) in NP treated HK-2 cells. The functional classification of differentially expressed genes showed that cell death related genes were regulated by NP in HK-2 cells. 79 genes were time-dependent and differentially expressed, while 259 genes were concentration-dependent, all of which were selected using an ANOVA method. These data may support the understanding of the toxicity of nonylphenol in normal human kidney cells.
