Gastric mucosa-associated lymphoid tissue (MALT) lymphoma represents approximately 9% of all malignant lymphoma in Korean, and its incidence is higher than that of western and other Asian countries. Helicobacter pylori eradication is the first line treatment in limited stage gastric MALT lymphoma. There is no consensus about initial treatment with chemotherapy or radiotherapy for the patient with insufficient response to H. pylori eradication or H. pylori negative. Radiotherapy is a conventional therapeutic modality and showed the excellent long term results. The radiotherapy for limited gastric MALT lymphoma shows that the 4-5-year disease free survival rate is 85-100% and acute toxicities is reasonable. However, only non-randomized studies support a role of local radiotherapy in gastric MALT lymphoma. (Korean J Med 2012;83:712-717)
급성골수성백혈병의 차세대 염기서열 분석을 통한 분자유전학적 위험도 분류

Chromosomal abnormalities are an important prognostic factor in cases of acute myeloid leukemia (AML). Molecular mutations have been reported to contribute to the pathogenesis and prognosis of AML. Next-generation sequencing (NGS) has revolutionized the speed and cost of genomic sequencing and enables the parallel analysis of many genes for molecular risk stratification. The molecular mutations currently included in risk stratification at AML diagnosis are c-kit, FLT3-ITD, NPM1, CEBPA (biallelic), RUNX1, ASLX1, and TP53. The importance of screening for mutations has been further emphasized by introducing novel therapeutic targets for molecular mutations, such as FLT3-TKD, IDH1, and IDH2. Molecular mutations are also used to evaluate measurable residual disease during treatment and to select the intensity of the treatment during consolidation and follow-up. Pretreatment leukemic marrow and blood should be stored at a biobank to perform NGS analysis in cases of AML at diagnosis. Samples from various time points during and after treatment should be obtained and stored under appropriate conditions. (Korean J Med 2021;96:493-500)

Under the influence of an aging society, westernized diets, and environmental factors, the incidence of hematologic diseases Korea has changed over the past decades. Another reason for the increase is that the growth of molecular biology has contributed improvements in the diagnosis and treatment of hematologic disease. Although the incidence of aplastic anemia has recently declined, that of other hematologic diseases, such as multiple myeloma, myelodysplastic syndrome, chronic lymphocytic leukemia, and Hodgkin lymphoma, is rising. The incidence of thalassemia, a rare hereditary disorder of hemoglobin synthesis, is increasing because Korean society is changing to a multi-ethnic society through international marriage. In this article, we describe the recently increasing hematologic diseases in Korea. (Korean J Med 78:557-563, 2010)
동종 골수이식을 시행받은 환자에서 폐색성 세기관지염에 의하여 발생한 자발성 기종격동과 피하 기종

저자들은 만성골수성백혈병으로 동종 골수이식과 이식편 부전으로 인하여 추가적인 말초혈액 조혈모세포이식을 시행 받은 환자에서 만성 이식편대숙주질환과 그 폐 합병증인 폐색성 세기관지염에 동반된 자발성 기종격동과 피하 기종이 병발한 1예를 경험하였기에 이를 보고하는 바이다. Obstructive lung disorders following after allogeneic bone marrow transplantation (BMT) in association with graft- versus-host disease (GVHD) contribute significant morbidity and mortality. We report a case of a 28-year-old man who developed spontaneous pneumomediatinum and subcutaneous emphysema complicating bronchiolitis obliterans after allogeneic BMT. He received an allogeneic BMT for chronic phase of chronic myeloid leukemia. Five months after BMT, he was boostered by allogeneic peripheral blood stem cells from the same donor due to graft failure. One month after the boostering, chronic GVHD developed and were treated with cyclosporine and steroid. The patients developed spontaneous pneumomediatinum and subcutaneous emphysema secondary to severe bronchiolitis obliterans 4 months after boostering donor cells. The air-leak syndromes were recovered by conservative management, including high-flow oxygen.

This study is carried out to find the causes and the quantitative evaluation of sea dike materials loss which is occured during the period of construction works for the tide land reclamation projects on the west coast of Korea. Major subjects of this study are to establish the typical relationships among the tidal flow, the movement of dike materials, the ratio of material movement (losses), construction methods and the ratio of materials movement (losses). Based on the above subjects, this study is made for the purpose of obtaining the following informations ; (1) Collecting and evaluating the data of dike material losses due to the foundation settlement from designed existing dikes on the west coast. (2) By the field investigation at A-San Sea Dike, Pyong Taek Project, the comparison would be made by the relationships between the tide velocity and the movement of dike foundation under the natural conditions and the period of construction so that and out the relationships between the dike materials or fecundation situation and settlements. With regard to the dike construction works, it is so difficult to calculate the exact quantity of materials losses due to the foundation settlement. Losses of the dike materials are ; (1) Topographical variation (2) Sweeping the sectional area of dike by the tide velocity. (3) Dumping riprap to the outerside of dike during the period of construction works. (4) Sectional area losses by the cause of occurence of the new tide channels. (5) Material losses by the heavy storms. (6) Consolidation settlement by the fecundation weakness. (7) Material losses by the earth materials by tide flow. Most high material losses were occured by the consolidation settlement due to the foundation weakness, the maximum tide velocities due to decrease the cross-sectional area of the gaps and erosion of foundation due to the range of tide, inner and outerside of dike, or dike material losses due to the tide flow. Final conclusion would be obtained by the continuous measurement of consolidation settlement at the stage of final closure of the dike. (It is scheduled to clone on the end of 1972). However, intermediate conclusion can be introduced as follows ; (1) The estimation of material (losses) during the period of construction works for the existing sea dikes up to date were only empirical. The material losses at the general closure for design was estimated at 10% of the riprap, 20% of the earth materials, and 20% of the riprap, 40% of the earth materialsat the final closure of the dike. The final closure estimated double quantity to the general closures but it is still doubt. (2) The ratio of consolidation settlements was found smaller than the calculated quantity. It can be foreseen that settlement speeds is higher than the calculated. (3) The movement of dike foundations under the natural conditions was not so depended on the geological conditions of the f3undation. (4) When the tide velocities was estimated 100 at the normal tide, it was estimated 125 at the high tide and 55 at the low tide. The tide velocities at the low tide shows apparently lower than the high tide and the higher velocities at the deep water depth.
