Drooling causes inconvenience and develops complications, therefore it can be very stressful to patients and caregivers. Drooling caused by neurological disabilities appears to be the consequence of dysfunction in the coordination of the swallowing mechanism. However, there is no explanation about drooling by brain injury in oriental medicine literature. This is the case report about a stroke patient with drooling improved by electro-acupuncture therapy. The patient already applied scopolamine patch, and improved drooling before admission. However, caregiver concerned about side effects, so we stopped using scopolamine patch and applied electro-acupuncture for drooling treatment. The patient was treated by 4 acupuncture points on either side of Yeomcheon(CV 23), and Jichang(ST 4), Hyeopgeo(ST 6) with mixed low frequency electro stimulation. Although we stopped using scopolamine patch, drooling maintained improved state without a change, futhermore after 16 days later, drooling had disappeared until the patient discharged. This result showed electro-acupuncture is useful method to improve drooling caused by brain injury.

This is a case report of a tracheostomized 80-year-old man suffering from sputum after stroke. His symptoms were watery sputum production requiring frequent sputum suction. The subject was diagnosed as having a deficiency of spleen qi and was treated with Gami-ijung-tang, Yukgunja-tang, Soeumin Bojungikgi-tang, and Ijin-tang extract in the herbal prescription known to dispell Dam-eum (phlegm-retained fluid). Frequency of ssputum suction and condition of sputum were checked everyday. Gamiijung-tang, Soeumin Bojungikgi-tang with Ijin-tang extract led to improvement by reducing sputum secretion. There were limitations on frequency of suction and condition of sputum which were thought to be derived from tracheostomy, however. Consequently, pathology and treatment of sputum will be different whether tracheostomy was performed or not.

The purpose of this study is to report the clinical effects of modified Seogakjihwang-tang on a patient who had headache and memory disorder after traumatic brain injury, which were considered as the result of blood stasis. The patient had headache and memory disorder after traumatic brain injury and treated with modified Seogakjihwang-tang three times a day for 60days. As a result, the numerical rating scale and pain rating score of headache decreased and the scores of mini-mental state examination-Korea, the Korean version of Montreal cognitive assessment and the mini-memory test increased during treatment with modified Seogakjihwang-tang. Therefore, this case supports a possibility that modified Seogakjihwang-tang might be an effective treatment to patients with headache and memory disorder after traumatic brain injury.

Objective: The aim of this case report was to describe the effect of Hyeolgusamhwa-tang (絜矩三和湯) on abdominal distension caused by Qi (氣脹) in a patient with functional dyspepsia. Methods: A female Korean patient was treated with Hyeolgusamhwa-tang and acupuncture for 14 days. We observed objective symptoms, such as bloating, nausea, belching, loss of appetite, and heartburn, and we also measured her abdominal circumference. Results: After treatment, her abdominal circumference was decreased from 87 cm to 83.5 cm, and her food intake was improved from the baseline. The other objective symptoms, like general weakness, loss of appetite, abdominal discomfort, and heartburn, were improved or disappeared without any adverse events reported. However, we could not find any significant differences for some symptoms, such as nausea. Conclusions: This case suggests that Hyeolgusamhwa-tang and acupuncture therapy could be effective in reducing functional distension in patients diagnosed with distension caused by Qi (氣脹).

A patients with quadriplegia and a history of pancreatectomy was administered Socheongryong-tang to reduce sputum. The patient's sputum decreased, but a sudden increase in blood sugar was observed. Ephedra is known to stimulate the sympathetic nerves to increase the blood pressure and blood sugar, but in healthy people, changes in blood sugar are not significant due to corrections by hormone regulation. However, this case suggests that ephedra can cause significant blood sugar increases in patients who undergo pancreatectomy or who have diabetes mellitus. Even though ephedra is highlighted for its usefulness in sputum reduction and in weight control, it should be carefully prescribed to patients with diabetes.
