Acute toxic methemoglobinemia is an infrequent complication of the use of various drugs. Severe methemoglobinemia is very often fatal. Methylene blue is an effective drug in the treatment of methemoglobinemia patients. However, failure to respond to methylene blue has been described in patients with sulfhemoglobinemia, chlorate poisoning, and glucose-6-phosphate dehydrogenase deficiency. It is even possible that hemolysis may occur due to methylene blue treatment itself. We encountered a case of a 71-year-old woman who developed methemoglobinemia caused by alprazolam intoxication. She presented with hemolytic anemia and did not respond to methylene blue. In spite of concerted N-acetylcysteine therapy, the hemolytic anemia became aggravated and the patient died eleven days after intoxication.

Inferior mesenteric arteriovenous fistula (AVF) is a rare vascular abnormality that may be either congenital or acquired. It usually presents with abdominal pain, gastrointestinal bleeding, and a palpable mass with thrill or portal hypertension; it may also elicit more serious complications such as colonic ischemia. We report a case of idiopathic inferior mesenteric AVF in a 75-year-old man who presented with recurrent ischemic colitis. To our knowledge, this is the first reported case in Korea of an idiopathic inferior mesenteric AVF without any surgical or traumatic history.

위에 발생한 점액암종과 대세포신경내분비암종의 충돌종양: 증례 보고

The concurrence of two different pathological tumors of the stomach is infrequent. Even rarer is a gastric collision tumor of both tumor types. Although there have been a few reported cases of gastric collision tumors that consisted of an adenocarcinoma and neuroendocrine carcinoma, to the best of our knowledge, there is no documented case report of a gastric collision tumor consisting of a mucinous carcinoma and large cell neuroendocrine carcinoma. We report a case of gastric collision tumor, consisting of a mucinous carcinoma and large cell neuroendocrine carcinoma that presented as abdominal discomfort in a 64-year-old man. This finding draws attention to the related findings from previous studies on gastric collision tumors. 위암종이 조직학적으로 전혀 다른 두 가지 종양의 동반 사례는 매우 드물고, 충돌종양으로 나타나는 경우는 더욱 드물어 증례보고만 되고 있다. 위에 발생한 선암종과 신경내분비암종의 충돌종양이 국내외적으로 몇 개 보고된 바가 있으나 점액암종과 대세포신경내분비암종의 충돌종양은 우리가 알고 있는 바 문헌에 아직 보고가 없다. 저자들은 두 달 전부터 복부 불쾌감을 주소로 내원한 64세 남자환자에서 위에 발생한 점액암종과 대세포신경내분비암종의 충돌종양이 CT에서 구별되는 증례를 경험하였기에 컴퓨터단층촬영 및 조직병리소견을 문헌고찰과 함께 보고하고자 한다.
