목적 : 최근 학생들 사이에서 교복을 줄여서 꽉 끼게 입는 현상을 흔히 관찰 할 수 있다. 일반적으로 ‘쫄’ 교복 착용이 건강재해를 유발할 것이라는 생각은 많지만 관련된 연구는 드물다. 본 연구는 ‘쫄’ 교복 착용이 소화기계에 어떤 영향을 미치는지 알아보고자 시행하게 되었다. 대상 및 방법: ‘쫄’ 교복 착용에 의한 행동의 부자유도와 옆터짐 정도를 본 연구진이 개발한 INDEX 조사를 통하여 측정하였다. 00여고 2학년 198명에 대해 ‘쫄 교복 착용군 89명, 미착용군 109명을 설문조사하였다. 결과 : 일개 여고생들에게 ‘쫄’ 교복 착용률은 44.9%이었으며 조기 포만감과 상복부 이물감 호소율이 ‘쫄’ 교보 착용군에서 미착용군 보다 유의하게 높았다(P<0.05). 소화기계 증상 발생의 위험 요인에 대한 다중 로지스틱회귀분석에서 줄여 입은 군의 줄여 이비 않은 군에 대한 교차비는 2.01(95% CI 1.05-3.83), 아침을 먹은 군이 먹지 않은 군에 대한 교차비는 0.32(95% CI 0.10-0.99), 스트레스가 있는 군이 없는 군에 대한 교차비는 2.65(95% CI 1.31-5.38)로 조사되었다. 결론 : 여고생에서 ‘쫄’ 착용은 소화기계의 건강장해를 유발한다고 조사되었다. 단순한 유행의 하나로 '쫄‘ 교복을 착용할 수는 있지만 이로 인한 건강장해가 발생한다면 이는 청소년들에서 중요한 문제를 유발할 수 있다. ’쫄‘ 교복착용의 부작용에 대한 다양하고 심도있는 연구가 필요하며 일선 학교에서도 홍보와 계도가 필요하다. Objectives : This study was carried out to evaluate the health effects of a tight school uniform in high school girls. Of the health effects, we focused gastrointestinal disorders. Methods : We conducted a questionnaire survey of 198 students at a girls' high school in Gyeongju. One hundred ninety eight students were divided into two groups by wearing on a tight school uniform(TSU) and a loose school uniform(LSU). We have measured the extent of discomfort related to motion and a garment rips through tightness INDEX developed by the authors. Results : The proportion of students wearing TSU was 44.9%. TSU group complained significantly more early gastric satiety and epigastric foreign body sensation than LSU group (P<0.05). Multiple logistic regression, TSU(OR=2.01, 95% CI:1.05-3.83), regular breakfast(OR=0.32, 95% CI:0.10-0.99), and stress (OR=2.65, 95% CI:1.31-5.38) were significantly associated with gastrointestinal symptoms. Conclusions : The results of this study indicate that the clothing pressure from a tight school uniform has some effects on the gastrointestinal disorder. More extensive epidemiological studies and public activities for adverse effects of a TSU is needed.

고관절 전치환술 혹은 고관절 반치환술 후 발생하는 대퇴골 주위골절은 드물며, 치료하기 힘든 심각한 합병증중 하나이다. 최근 고관절 전치환술 및 재치환술의 빈도 증가로 대퇴골 주위 골절의 빈도가 늘어나고 있다. 대퇴골 주위 골절의 발생시 많은 요소들이 고려되어야 한다. 저자들은 서울 백병원 정형외과에서 경험했던 대퇴골 주위 골절의 결과를 보고하는 바이다. Periprosthetic fracture of the femur after total or semi of hip arthroplasty was uncommon but complications were difficult to treat. The incidence of periprosthetic fracture had increased in recent years. Our policy was to treat this complication operatively, and we have reviewed our results from 1984 to 1997. When these fractures did occur, many factors were involved in the treatment decision.

Xanthoma is a localized collection of tissue histocytes containing lipid. The majority of tendinous xanthomas probably occurs in the setting of hypercholesterolemia especially in bilateral Achilles tendon xanthomas. Xanthoma of the Achilles tendon is a rather rare, interesting orthopaedic condition that has important ramifications in internal medicine and dermatology because the lesion is associated with a specific disturbance of lipid metabolism. We experienced one case of normolipidemic and symptomatic Achilles tendon xanthoma. Surgical intervention was carried out for cosmetic and symptomatic reasons, the patient undergoing total resection and a reconstruction of the Achilles tendon by the combinedV-Y muscle flap and modified Lindholm technique.

The purpose of this study was to analyse forty-five month follow-up results of twenty total joint arthroplasties of eighteen patients with ankylosed hip retrospectively. The patients were operated upon from May 1991 to February 1997. Eleven hips had been spontaneously ankylosed and nine hips surgically fused. The duration of the ankylosis averaged twenty-six years. The preoperative Harris hip score averaged 67.5 points and the preoperative limb shortening averaged 3.9cm compared with the contralateral side. The average postoperative Harris hip score was 85,6 points and sixteen patients(88.9%) were satisfied with the result of the operaion at the last follow-up. The postoperative range of hip flexion averaged 77.6 degrees. The postoperative limb shortening averaged 1.6cm, which was later corrected by the addition of 2.3cm. The postoperatively measured abductor-lever arm ratio averaged 0.48. Of the twenty hips, infection developed in one; recurrent dislocation, in one; aseptic loosening, in one; and heterotopic ossification, in five. Our experience with these results implies that, if the technical difficulties are overcome, total joint arthroplasty of ankylosed hips is an useful operation which can achieve relief of pain, improved mobility and the correction of limb length discrepancy
